address above. Enclosed is a copy of the Department of Labor and Industiies, Factory
Assembled Structures Section approved plans for your use. Attached is a list of items
that will require review, inspection and approval by Jefferson County. Blazer
Industries is the manufacturer, they may be contacte d at 503-749-1900. The
Department of Labor and Industries, Factory Assembled Structures Section has
informed the manufacturer that permits, review and inspection of on site construction,
conditions and completion of the building will be required by the Jefferson County. If

Hi Scott,
Let me start by apologizing for the confusion this permit, BLD18-00489, has created. David is a land use planner, he is
not a building inspector. He would only do inspections for parking, landscaping, and possibly signs. Jim Coyne or Nathan
Whitmire would be the only people able to do an inspection for the structure and anything having to do with
fire/life/safety. There was a temporary occupancy issued for this building permit, but there has never been a permanent
certificate of occupancy. Typically we would not have issued a temporary occupancy certificate unless all of these other
items had been signed off so I am unsure how this one slipped through. The septic has been signed off, as well as all land
use requirements. I did speak with Jim and Nathan and they both said they inspected the temporary unit for these items
but not the permanent modular building. I believe all the remaining inspections should be quick and not take up too
much of your time but unfortunately we are unable to issue the permanent certiflcate of occupancy until these items
have been inspected.
Again, I apologize for any inconvenience this has caused.

Morning Jodi,
Earlier this year the PUD installed a permanent modular at 310 Four Corners Road. As we installed the modular we had
Jim Coyne come out to do some inspections we also had David Wayne Johnson come out to do some inspections as well
David Wayne Johnson was the inspector that entered the new modular and checked for the items you seem to be
delinquent ol David actually suggested moving the handicap parking to a better location. At that point the only hold up
for final occupancy was the septic as built that needed to be drawn up by Mike Deeney contract septic engineer for the
DCD offers an online scheduling calendar to request appointments, submit questions, and pay consultation fees on our
website front page. Appointment times for this new service are available Tuesday-Thursday from 1pm-4:30pm.
Customers must request appointments at least 48 hours in advance. Walk-in hours are 10:30am-12pm Monday-
Thursday. All walk-in meetings as well as appointments require a minimum S50 for 30 minutes or S1O0 for t hour.

PUD. This is why PUD got a temporary occupancy. The email below was from susan Porto explaining she had finally
gotten the necessary drawing from Mike Deeney and the process was signed off.
I thought I received a certificate of occupancy for 310 Four Corners Road. I am trying to locate this document. Please
give me a little time to find this if I actually have it.
I do have in my hand a final occupancy for 210 Four Corners Road, this is a temporary modular installed while PUD adds
on to our facility at 310 Four Corners.

. Application process: Type I Building Permit for commercialconstruction with SEPA.. The PUD has a Type III Conditional Use Permit (ZON 15-00023) approved by the
Jefferson County Hearing Examiner for operating a Utility Development, Major in a rural
residential zone. The approved conditional use brought the PUD into compliance with
current zoning which allows them to expand the facility through a Type I Building Permit
with no further hearings required.. The SEPA handbook states in section *2.3.3.5 Tips" that SEPA is required for any
addition when the total square footage of the building (old plus new) exceeds the
threshold adopted by the local jurisdiction. Per JCC 18.40.750 the maximum exempt
level for a commercial building is 12,000 square feet with associated parking for up to 40
automobiles. The proposal is for a 12,543 square feet building and 6l parking spaces
o Landscaping: Per the Conditional Use Permit the existing trees and shrubs meet or exceed the
standards to fulfill landscaping or screening requirements.

. Parking: Parking for Utilities Uses is to be determined by the administrator, 6l spaces shown on

Hi Paul and Mark.
Randy Marx here, I am the health department guy who didn't make it to your pre app.
When the building application comes in for the expansion, it will be routed to health for review and sign off. We
will require a septic system designer look at the current septic system use, and show that the septic system has
capacity for the proposed project before we could sign off our review. The designer will need to write up a
justification that shows the septic system has capacity for the proposed use. The justification must include:
o The current use of the septic system and determine the remaining capacity of the septic system. They
will be able to download data from the control panel and see what the current peak use is, From this
the designer will tell you how much capacity you have left.
o The current number of employees.
. The proposed level of staffing the expansion will bring.
Once capacity has been addressed we look at the proposed construction. Looking at plans submitted with the pre
app I do see a couple issues. I do realize this wasn't the purpose of those plans, however:
o You show a new water and a new sewer Iine entering the existing building and have them
crossing. WAC246-272A,Table IV requires a 10'separation. You will need to un-cross this connection
to the building and separate the lines by 10.'
o To add a new connection to the existing septic system we would require a "Modification" permit, This
will need to be completed by a licensed septic system designer.
. The modification permit will need to address the new water line crossing the existing sewage transport
line.
o You need to show and label the existing septic system:the 3 tanks, the primary drainfield, the reserve
drainfield, and all transport lines. Attached is a pdf of the record drawing showing these items,
o Show how you are protecting the septic system components from traffic.
o The existing septic system will need to have had a monitoring inspection by a Jefferson County licensed
O&M provider within l2months of the building permit application. Your septic system designer can
also perform this inspection.
Please let me know if you have any questions along the way. Also please note on the record drawing, off the east
end of the existing building is a decommissioned septic tank. Typically tanks are pumped out then the lid broken in
and filled with soil in place. My records don't indicate it was removed.
