Statement of Issue:
Standard Professional Services Supplemental Agreement with Clark Land Office, PLLC of Sequim,
WA. for additional surveying services for the SR 116 and Cedar Avenue Pedestrian/Bike
Improvement Project.
Analysis/Strategic Goals/Pro's a Con's:
This project is included in the 2021-2026 Transportation Improvement Program and the 2021
Annual Construction Program as Item No. 3. The goal of this project is to improve pedestrian and
bicycle safety along Cedar Ave. between SR 116 and Hayden Street, provide a multi-use path for
students from Cedar Ave. to Chimacum Creek Primary School, provide a pedestrian sidewalk
from Cedar Ave. to the Jefferson County Regional Library, and to provide a pair of pedestrian
activated Rectangular Rapid Flashing Beacons at the school entrance on SR 116.
This agreement will provide additional preliminary engineering surveying for utilities relocation
work and construction surveying services by Clark Land Office, PLLC. This supplemental
agreement amends the Professional Services Agreement with Clark Land Office, PLLC executed
on November 18, 2019.
Fiscal Impact/Cost Benefit Analysis:
The maximum amount payable for consultant services under this Professional Services
Supplemental Agreement is $29,190.71, to be paid from Safe Routes to School (State Funding)
grant and from Jefferson County Local Road Funds.
Recommendation:
The Board is asked to execute the three (3) originals of the Professional Services Supplemental
Agreement 1 with Clark Land Office, PLLC and return two (2) signed originals to Public Works
(attn. John Wayand).

SUPPLEMENTAL AGREEMENT
The Local Agency of Jefferson County Public Works hereby amends the agreement with Clark
Land Office, PLLC (“the Agreement”), executed on November 18, 2019.
All provisions in the Agreement remain in effect, except as expressly modified by this
Supplemental Agreement.
The changes to the Agreement are described as follows:

SCOPE OF WORK
Project Title: SR 116 and Cedar Avenue Pedestrian/Bike Improvements
Location of Project: Cedar Avenue from the Jefferson County Library driveway entrance to Hayden Street and
the proposed Multi-use Path from Cedar Avenue to the Chimacum Primary School parking lot, Port Hadlock,
Jefferson County, WA.
Task 1: Preliminary Engineering Surveying for PUD Utilities Relocation Work
This work consists of surveying for installation of new and relocated, underground and overhead, utilities to be
installed by Public Utility District No. 1 of Jefferson County (PUD), in conjunction with Jefferson County
Public Work’s (JCPW) Cedar Ave. project. PUD will be installing underground and overhead utilities prior to
the Cedar Ave. project construction work. The PUD utilities need to be installed at locations where they will
not conflict with the Cedar Ave. project design, so the Cedar Ave. project design needs to be marked on-site
prior to the installation of utilities.
General
Review of project construction plan and coordination of work with JCPW and PUD. On-site meeting as needed.
Cedar Avenue
1. Calculations for staking and project data setup in surveying instruments.
2. Recovery of previously established and new horizontal and vertical surveying control on-site.

Path, Multi-use Path Bypass; and School Service Driveway. This work also includes re-staking as needed of
points previously marked for the Task 1 preliminary engineering surveying for utilities.
