Observed the placement of ASTM A615 grade 60 reinforcing steel at the above-mentioned locations. Reinforcing steel was of proper size, grade, lap, spacing, location, clearance, and secure prior to placement of concrete(see note). Anchor Bolts: 3/4", 1", 1-1/8" Anchor Rods With 15" Embedment. Concrete Placement: Continuous Inspection Observed the placement of 39 CY of concrete mix design #0255AC into grade beam at the location noted above. One set of 5 (4” x 8”) cylinders were cast for compressive strength. Reference MTC Compressive Strength Report No. C259148 for more information. Concrete was placed via concrete pump truck, mechanically consolidated with a high-cycle vibrator and finished by hand. Noteworthy Items Grade beams were placed 8" below top of slab. Top 8" will be placed monolithically with slab on grade per stamped letter from engineer of record dated 6-29-2021. At grid lines 2,3/A the footings are not per structural drawings they are however as shown on anchor rod schedule on VP building as build drawings dated 3-21-2021. Footings under grade beams were not inspected by MTC due to concrete only needing to be 2500 psi per general notes. To the best of MTC inspector's knowledge, the above-described work was performed in general accordance with project specifications and approved plans.
