All provisions in the Agreement and Supplemental Agreement Number I remain in effect,except as expressly modified by this Supplemental Agreement Number 2. The changes to the Agreement are described as follows: I. No. 637109 HLP-S19(006), LA-9732 Statement of Issue: Standard Professional Services Supplemental Agreement with Clark Land Office, PLLC of Sequim, WA. for surveying services for the SR 116 and Cedar Avenue Pedestrian/Bike Improvement Project. Analysis/Strategic Goals/Pro's £t Con's: This project is included in the 2022-2027 Transportation Improvement Program and the 2021 Annual Construction Program as Item No. 3. The goal of this project is to improve pedestrian and bicycle safety along Cedar Ave. between SR 116 and Hayden Street, provide a multi-use path for students from Cedar Ave. to Chimacum Creek Primary School, provide a pedestrian sidewalk from Cedar Ave. to the Jefferson County Regional Library, and to provide a pair of pedestrian activated Rectangular Rapid Flashing Beacons at the school entrance on SR 116. This Supplemental Agreement Number 2 extends the Completion Date from December 31, 2021 to December 31, 2023, providing the time required to complete the construction surveying work included in Supplemental Agreement Number 1, executed on April 26, 2021 with Clark Land Office, PLLC. This supplemental agreement amends the original Professional Services Agreement with Clark Land Office, PLLC executed on November 18, 2019. Fiscal Impact/Cost Benefit Analysis: The maximum amount payable for consultant services under this Professional Services Supplemental Agreement is unchanged and remains at $52,264.88. This project has WSDOT Safe Routes to School (State Funding) grant (100% grant ratio). Recommendation: The Board is asked to execute the three (3) originals of the Professional Services Supplemental Agreement 2 with Clark Land Office, PLLC and return two (2) signed originals to Public Works (attn. John Wayand).
