Reinforcing steel was of proper size, grade, lap, spacing, location, clearance, and secure prior to placement of concrete. Concrete Placement: Continuous Inspection Observed the placement of 102 CY of concrete mix design #0255AC into Slab on Grade at the location noted above. One set of 5 (4” x 8”) cylinders were cast for compressive strength. Reference MTC Compressive Strength Report No. C259148 for more information. Concrete was placed via concrete pump truck, and finished by Hand. To the best of MTC inspector's knowledge, the above-described work was performed in general accordance with project specifications and approved plans. Upon arrival, MTC checked in with the contractor and went over the inherent limitations of GPR. MTC scanned the accessible areas at the NE and SE building corners (wall and floor). Signals indicating reinforcement were observed, marked on the surface in crayon, and discussed with the contractor. In general, signals near the corner were approxiamtely 6 inches apart. To the best of MTC inspector's knowledge, the above-described work was performed in general accordance with project specifications and approved plans. The contractor consulted with the engineer via phone and was informed that two 7" embeds ~4inches apart would be acceptable in place of the one 9" embed.
