Environmental Assessment Specialists, Inc. (EAS) has completed a Phase I Environmental Site
Assessment (ESA) for the above referenced site in accordance with the 2022 Masters Services
Agreement between EAS and AT&T Mobility, LLC, (AT&T) and in substantial compliance with
the scope and limitations of the American Society of Testing Materials (ASTM), Standard
Practices for ESAs: The Phase I ESA Process, Designation E1527-21. Per AT&T Mobility, LLC
request, this Phase I Environmental Site Assessment has been modified to include the
construction of a proposed wireless antenna site design and its relationship to any Recognized
Environmental Conditions, if any.
The Property consists of a forest land parcel that is occupied by an existing telecommunications
facility with associated antenna structure, antennas, and equipment by others, an existing diesel
powered back-up generator and associated diesel belly tank by others, an existing
telecommunications fenced compound enclosure by others, gravel-covered areas, and a gravel-
paved access driveway. The proposed AT&T Site is to be constructed within a portion of the
existing telecommunications compound by others, on the northwestern portion of the existing
telecommunications compound by others. The Property is located in an area comprised primarily
of forest land and sparse residential development. EAS walked the Property, visually observing
all accessible features. At the time of the site visit, the Property appeared to be in good condition
and well maintained.
According to the provided construction drawings dated December 08, 2021, AT&T proposes the
installation of a new unmanned telecommunications facility at this location. Proposed is the
installation of new antennas, RRH’s, microwave dish, and surge suppressors to be placed on the
existing 150’ monopole by others. A new diesel back-up generator and a new walk up cabinet
will be placed on a new concrete pad located within the existing fenced telecommunications
compound buy others, on the northwestern portion of the existing compound. The proposed
Power and Fiber sources, and proposed utility runs were not identified in the provided Site Plans.
Based on a site reconnaissance and a review of physiographic, historical, and regulatory
information, there is no evidence of current recognized environmental conditions (as defined by
ASTM standards) in connection with the proposed lease Site. No visual evidence of asbestos-
containing material or lead-based paint was observed in the proposed lease Site. This assessment
has revealed no evidence of recognized environmental conditions in connection with the
Property or proposed Client lease areas.

1.0 INTRODUCTION
The purpose of this Phase I ESA was to identify recognized environmental conditions associated
with the AT&T Mobility, LLC lease area. To achieve this objective, the Phase I ESA included
visual observations of the property and observations of the surrounding properties, a visual survey
for suspect asbestos-containing materials/debris piles/lead-based paint, limited historical land use
review, review of regulatory database listings, and reviews of readily available geologic and
hydrogeologic data. This report represents a summary of these findings. A topographic map,
aerial photos, site plan, site photographs, and a regulatory database report are included as
attachments to this report.
EAS visually observed the Property to identify potential sources or indications of chemical
contamination such as underground storage tanks (USTs), aboveground storage tanks (ASTs),
polychlorinated biphenyls (PCBs), chemicals and hazardous waste materials, areas with surficial
staining or distressed vegetation, and visual evidence of asbestos containing materials (ACMs)
and/or lead-based paint.
Lands immediately adjacent to the Property were visually inspected for possible sources of
contamination or environmental impairment, which could migrate to the Site via surface water
runoff, groundwater transport, and other pathways. EAS conducted a regulatory records review,
reviewed historical aerial photographs, building permits, and historical maps, if available, and
contacted regulatory agency personnel.
2.0 SITE LOCATION and DESCRIPTION
According to the provided site plans, the proposed AT&T lease Site is located at the listed
address of 393 Old Coyle Road, Quilcene, Jefferson County, Washington 98376. According to
the Jefferson County Assessor’s Office, the Property is listed as being located within Assessor’s
Parcel Number (APN) 601222001 and is identified as an unaddressed forest land parcel.
According to the Jefferson County Assessor’s Office, the closest parcel to the Property with an
address is identified as a single-family residence at 81 Glacier Ridge Road, which is located
approximately 740’ east of the Property. For the purpose of this report, the Property will be
identified as being located at the address listed on the provided Site Plans.
The Property consists of a forest land parcel that is occupied by an existing telecommunications
facility with associated antenna structure, antennas, and equipment by others, an existing diesel
powered back-up generator and associated diesel belly tank by others, an existing
telecommunications fenced compound enclosure by others, gravel-covered areas, and a gravel-
paved access driveway. The proposed AT&T Site is to be constructed within a portion of the
existing telecommunications compound by others, on the northwestern portion of the existing
telecommunications compound by others. The Property is located in an area comprised primarily
of forest land and sparse residential development. Access to the Property is gained via an existing
gravel-paved access road on the west side of Coyle Road. EAS walked the Property, visually
observing all accessible features. At the time of the site visit, the Property appeared to be in good
condition and well maintained.

3.0 PHYSICAL SETTING
Based on information obtained from the EDR FirstSearch Report, the proposed AT&T Site is
located at approximately 47°44’09.81” North Latitude, -122°47’46.53” West Longitude as
depicted on the United States Geological Survey (USGS) Seabeck, WA 7.5- Minute Quadrangle
(1:24,000). The Property is located at an elevation of approximately 495’ above mean sea level
(amsl), with a gradient descending toward the southwest. Storm water runoff is expected to flow
off the Property toward the southwest.
According to the Washington Division of Geology and Earth Resources Geologic Map of the
Seabeck and Poulsbo Quadrangles, the Site appears to be underlain by Vashon Ice-Contact
Deposits consisting primarily of cobble and pebble gravels, sand, ablation fill, flow till,
lacustrine mud, and rare boulders.
According to the Washington State Department of Ecology Groundwater Division personnel,
groundwater in the immediate area surrounding the property is anticipated to be over 20 feet
below ground surface (bgs). According to the surface topography for the site area, as interpreted
from the USGS topographic map, groundwater in the Property vicinity is anticipated to generally
flow towards the southwest. Therefore, in assessing potential external environmental impacts,
properties located directly northeast of the site (referred to as up-gradient sites) are of primary
concern. However, actual groundwater flow direction is often locally influenced by factors such
as rainfall, geologic structure, seasonal fluctuations, soil and bedrock geology, production wells,
and other factors beyond the scope of this study. The actual groundwater flow direction under the
site can be accurately determined only by installing groundwater monitoring wells, which was
beyond this scope of this project.
4.0 SITE RECONNAISSANCE and OBSERVATION
On January 26, 2022, EAS personnel conducted a site reconnaissance of the AT&T lease area (see
Topographic Map and Site Plan in Appendices). The site reconnaissance is documented in the site
photographs (Appendices). The EAS representative was unaccompanied during the site visit. In
addition, a walk along the perimeter of the Property and a drive around roads in the immediate area
were conducted.
The purpose of the site reconnaissance was to visually and physically observe the site, site
structures, and adjoining properties for conditions indicating an existing release, past release, or
threatened release of any hazardous substances or petroleum products into structures of the site,
or into soil and/or groundwater beneath the site. This would include any evidence of

contamination, distressed vegetation, petroleum-hydrocarbon staining, waste drums, illegal
dumping, or improper waste storage/handling.
The Property consists of a forest land parcel that is occupied by an existing telecommunications
facility with associated antenna structure, antennas, and equipment by others, an existing diesel
powered back-up generator and associated diesel belly tank by others, an existing
telecommunications fenced compound enclosure by others, gravel-covered areas, and a gravel-
paved access driveway. The proposed AT&T Site is to be constructed within a portion of the
existing telecommunications compound by others, on the northwestern portion of the existing
telecommunications compound by others. The Property is located in an area comprised primarily
of forest land and sparse residential development. Access to the Property is gained via an existing
gravel-paved access road on the west side of Coyle Road.
The proposed lease Site is bordered in all directions by portions of the existing
telecommunications compound by others, forest land, and vacant un-paved areas. EAS walked
the Property, visually observing all accessible features. At the time of the site visit, the Property
appeared to be in good condition and well maintained.
Underground Storage Tanks/Aboveground Storage Tanks (UST/AST)
No visual evidence (e.g. pipes, vents, pumps, and stains) that would indicate the past or present
use of petroleum hydrocarbon AST’s/UST’s on or immediately up-gradient of the lease area was
readily apparent during the site visit. Reviews of the EDR FirstSearch government database
report, the Jefferson County Fire Protection District #2 records, and the Washington State
Department of Ecology records indicated that there are no current or historical UST’s identified
on the Property, and a small quantity diesel AST located on the Property. This AST is associated
with an existing back-up generator for the existing telecommunications facility by others, and
there have been no reported leaks, spills, or violations associated with this small quantity diesel
AST. Based on the status of this small quantity diesel AST, it is not considered to represent a
recognized environmental condition for the Property or proposed AT&T lease area.
Leaking Underground Storage Tanks (LUSTs)
No evidence for the presence of leaking underground storage tanks on or immediately up-gradient
of the lease area was observed during the site reconnaissance.
Dry Cleaners
No dry-cleaning activity was observed on or immediately up-gradient of the lease Site during the
site reconnaissance.
Landfills
No evidence of solid waste disposal was observed on or adjacent to the lease area during the site
reconnaissance.

Polychlorinated Biphenyls (PCBs)
No leaking or stained equipment that would have the potential to contain PCBs (e.g.,
transformers, capacitors, light ballasts, hydraulic equipment) was observed at the lease area
during the site reconnaissance.
Waste Management and Chemical Handling
No hazardous materials/substances, evidence of hazardous waste storage or disposal, or
petroleum products were observed on or adjacent to the lease area during the site visit. However,
it is assumed from general industry practice that the existing cellular equipment contains a battery
gelpack with sulfuric acid solution contained inside. No evidence of leakage was observed on the
adjacent ground surface at the time of the site visit. Based on a visual inspection of the existing
equipment area, it is not considered to represent a recognized environmental condition for the
proposed AT&T lease area.
Asbestos-Containing Material/Lead-Based Paint (ACMs/LBP)
As the proposed AT&T Mobility, LLC antennas are to be located on an existing antenna
structure, and equipment on a new concrete pad, asbestos-containing materials and/or lead-based
paints are not expected to be encountered during construction activities.
Naturally Occurring Asbestos (NOA)
Serpentine rock has been identified by the Washington State Department of Health and the EPA
as having the potential to contain naturally occurring asbestos. The State DEH has identified
naturally occurring asbestos as a toxic air contaminant. EAS contacted the DEH in efforts to
identify the potential presence of Serpentine rocks in the immediate and surrounding area of the
proposed lease area. According to the DEH personnel, they did not indicate the potential presence
of underlying serpentine, and Ultramafic rock below the Property.
Vapor Intrusion Condition (VIC)
As the Property address and the adjacent property addresses are not identified in any Leaking
UST, CERCLIS, or Institutional/Engineering Controls databases, and no evidence for the
presence of leaking USTs or significant hazardous materials usage, storage, or disposal was
observed within a 100-foot radius of the lease Site during the site visit, a potential vapor intrusion
condition (VIC) does not appear to exist in connection with the lease Site.
Radon
Radon gas is a naturally occurring radioactive gas that is invisible and odorless. It forms from the
radioactive decay of small amounts of uranium and thorium naturally present in rocks and soils,
so some radon exists in all rocks and soils. Because radon is a gas, it can easily move through
soil and cracks in building slabs or basement walls and concentrate in a building’s indoor air.
However, as the proposed AT&T lease Site will consist of an unmanned facility and the U.S.

EPA’s recommended radon guidelines apply to residences and not to businesses, radon gas
should not affect the operations at the proposed AT&T lease Site.
Underground Oil or Gas Pipelines
No evidence of oil or gas pipelines was observed within the lease Site during the site
reconnaissance.
Stained Soil or Asphalt Patches
No stained soil or asphalt patches were observed within or adjacent to the lease area during the
site reconnaissance.
Demolition Debris
No evidence of demolition debris was observed on or adjacent to the lease Site during the site
reconnaissance.
Pits, Ponds, or Lagoons
No pits, ponds, or lagoons were observed within the lease area during the site reconnaissance.
Clarifiers or Sumps
No clarifiers or sumps were observed or noted within or next to the lease area during the site
reconnaissance.
Air Emissions
No air emissions were observed or noted to be emanating from the lease area during the site
reconnaissance.
Pesticides/Herbicides
No pesticides or herbicides were observed being stored or used within the lease area at the time of
the site reconnaissance.
Cisterns/Cesspools
No visual evidence of cisterns, or cesspools was observed within the accessible portions of the
Property or lease area at the time of the site reconnaissance.
On-Site Wells
No on-site wells were observed or noted within or next to the proposed lease area during the site
reconnaissance.

Septic Systems
Septic systems are important, as they can be a venue for mismanagement of hazardous or
regulated wastes. No evidence of septic systems were observed or noted within or next to the
proposed lease area during the site reconnaissance.
5.0 ADJACENT PROPERTIES
EAS observed lands adjacent to the Property to identify environmental concerns. The Property is
bordered to the north, south, and west by forest land, and to the east by Coyle Road followed by
A residence. EAS conducted a reconnaissance of the adjacent properties to evaluate the potential
for off-site impacts. These would include evidence of improper chemical storage or usage,
surface staining or leakage, distressed vegetation, or evidence of dumping. A visual observation
from the public right-of-way did not reveal any evidence of concern. Based on these observations,
adjacent properties do not appear to represent a recognized environmental condition for the
proposed lease Site.
6.0 REGULATORY RECORDS REVIEW
EAS reviewed available databases from federal and state regulatory agencies to identify use,
generation, storage, treatment and/or disposal of hazardous materials and chemicals or release
incidents of such materials, which may have impacted the Property. The regulatory databases
were provided to EAS from EDR. The EDR Firstsearch Report is included in the Appendix B.
The environmental and regulatory databases that were included in this review follow the ASTM
standard E1527-21 guidelines.

Information obtained from the EDR FirstSearch Report indicated that the Property has not been
included on any institutional/engineering control databases that track activity and use limitations
on properties.
The date of the most recent database update and a plotted map of the aforementioned listings, if
any, depicting their location relative to the proposed AT&T Mobility, LLC. lease area are
included in the Appendices of this report. Based on the nature of the proposed construction, the
regulatory status, distance, anticipated depth to groundwater, and/or hydraulic gradient, no
facilities identified in the EDR FirstSearch Report (if any) are suspected of having had a negative
impact on the proposed AT&T Mobility, LLC. lease area.
Orphan Sites
Orphan sites are sites that cannot be plotted due to errors or missing information in the regulatory
records. There were no sites listed as an unmapped or nongeocoded site in the EDR FirstSearch
report.
7.0 INFORMATION AND INTERVIEWS FROM LOCAL RECORDS AND SITE
CONTACT
Jefferson County Department of Community Development (JCDCD)
EAS contacted the JCDCD to obtain building permits for the Property in an effort to obtain
information in regard to the past and present uses of the Property. According to the JCDCD, no
records of concern are on file for the Property or proposed AT&T Site.
Washington State Department of Ecology Toxics Cleanup Program (WDETCP)
EAS contacted the WDETCP for files related to possible environmental concerns for the Property
and surrounding properties, including USTs, LUSTs, and hazardous materials use, storage, and
disposal. As of the date of this report, this agency has not responded to our request. Should
receipt of a response change the conclusions of this report, AT&T will be notified in writing by
EAS. Based on observations made during the site visit, the absence of an onsite listing within the
regulatory database report indicative of open case/pending spills, releases, or contamination
conditions, and the proposed Site design, the lack of response from this agency is not considered
a significant data gap and does not represent a concern at this time.
Washington State Department of Natural Resources (WDNR)
EAS contacted the WDNR for files related to possible environmental concerns for the Property
and surrounding properties. As of the date of this report, this agency has not responded to our
request. Should receipt of a response change the conclusions of this report, AT&T will be

notified in writing by EAS. Based on observations made during the site visit, the absence of an

EAS contacted the JCFPD for files related to possible environmental concerns for the Property
and surrounding properties, including USTs, LUSTs, and hazardous materials use, storage, and
disposal. According to the JCFPD, no records of concern are on file for the Property or proposed
AT&T Site.
Site Contact Interview
EAS made several attempts to contact the site representative to conduct an interview in regard to
potential environmental conditions associated with the Property, including the use, storage, or
disposal of significant quantities of hazardous materials/substances or hazardous waste. The
property representatives did not respond to the request for information; thus, the site interview
was not able to be conducted within the time constraints of this report. However, based on
observations made during the site visit and the status of onsite listings in a database indicative of
spills, releases, or contamination conditions within the regulatory database report, the lack of a
Property representative interview does not represent a concern at this time and is not considered a
significant data gap. Should receipt of a response change the conclusions of this report, AT&T
will be notified in writing by EAS.
Former Site Contact Interview
EAS was unable to ascertain or contact previous Property owners for an interview regarding
whether any recognized or potential recognized environmental conditions were associated with
the Property during their occupancy in time for review and incorporation into this report. No
recognized environmental conditions from historical on-site land use were discovered within the
proposed AT&T Mobility, LLC lease area or remainder of the Property during the historical
research process of the preparation of this report.
Previous Environmental Reports
No previous environmental reports, including Phase I ESA’s or asbestos/lead-based paint
surveys, were provided to EAS during the course of preparation of this report.
Specialized Knowledge
There is no specialized knowledge about the subject property or proposed AT&T lease area as
provided by the User (AT&T) of this Environmental Site Assessment. Therefore, no specialized
knowledge was supplied to the environmental professional.

Chain-of-Title
At the Client’s discretion, a chain-of-title report was not provided to EAS for review and
incorporation into this report.
8.0 HISTORICAL USE INFORMATION REVIEW
Aerial Photographs and Topographic Maps
Aerial photographs and topographic maps were reviewed to identify historical land development
and any uses, which may have impacted the Property and proposed lease areas. Photographs and
maps provided by EDR and the USGS were reviewed at approximately 5-10-year intervals dating
back to 1951 (See Appendices). Historically, the Property has been occupied by forest land from
1951 to 1990. From 2006 to 2016 the Property appears as present-day use. No recognized
environmental conditions for the Property were identified during the aerial photograph review.
Sanborn Fire Maps
As the Property has been sufficiently covered by other standard historic information sources,
Sanborn Maps were not consulted as part of this assessment.
Oil and Gas Fields
Based on Washington State Department of Natural Resources Oil, Gas, & Geothermal Resources
Division personnel, no production wells are currently, or have historically been located on or
adjacent to the Property.
City Directories
As the Property has been sufficiently covered by other standard historic information sources, city
directory information was not consulted as part of this assessment.
Historical Data Gaps
During the historical research process of the preparation of this report, there were no gaps
exceeding five years in which EAS was unable to ascertain the probable on-site land use.

9.0 CONCLUSIONS AND FINDINGS
EAS performed a Phase I ESA of the AT&T lease area, BR0266 located at 393 Old Coyle Road,
Quilcene, Jefferson County, Washington 98376. Any exceptions to or deletions from this practice
are described in Section 10.0 of this report.
Based on a site reconnaissance and a review of physiographic, historical, and regulatory
information, there is no evidence of current recognized environmental conditions (as defined by
ASTM standards) in connection with the proposed lease Site. No visual evidence of asbestos-
containing material or lead-based paint was observed in the proposed lease Site. This assessment
has revealed no evidence of recognized environmental conditions in connection with the
Property or proposed Client lease areas.
10.0 LIMITATIONS
The professional opinions contained in this report are based solely on the laws, regulations, and
technical data known to EAS at the time of report preparation. The conclusions of this assessment
rely on reasonably obtainable information from site reconnaissance, interviews with onsite
personnel and public officials, and public records. No warranty is made regarding the accuracy of
the publicly documented information, or the opinions of officials or personnel consulted for the
study. All known information has been disclosed and a good-faith effort has been made to consult
pertinent sources.
It should be noted that all environmental assessments are inherently limited in the sense that
conclusions are drawn, and recommendations developed, from information obtained from limited
research and site evaluation. Subsurface conditions were not investigated as part of this study and
may differ from the conditions implied by visual observations. Additionally, the passage of time
may result in a change in environmental characteristics at this site and on surrounding properties.
This report does not warrant against future operations, activities, or conditions that may occur. This
report is not a regulatory compliance audit. A regulatory compliance audit of the tenant operation
would analyze compliance of the operation with regulatory requirements and accepted industry
practices. The scope of the Phase I ESA focused on the likelihood or potential presence of
recognized environmental conditions at the subject property, according to ASTM standards.
Contents of onsite containers were not inspected; however, detailed information regarding container
contents was not provided by the tenant operator or AT&T Mobility, LLC.
This study is not intended to assess or otherwise determine if any soil contamination, waste
emplacement, or groundwater contamination exists on the subject property. This investigation
has been based only upon prior site history, previous documentation, and observable conditions.
Existing hazardous materials and contaminants can escape detection using these methods. The
results of this study suggest that it is possible that hazardous materials contamination exists at the
subject property and that further investigation (regulatory file review, subsurface testing) may be
necessary to make a definite assessment. Our conclusions regarding the potential environmental
impact from off-site facilities near the subject property are based on readily available information
from the environmental databases and the assumed groundwater flow direction. A detailed file
review of each facility was beyond the scope of work.

NPL: NPL National Priorities List (Superfund). The NPL is a subset of CERCLIS and identifies over 1,200 sites
for priority cleanup under the Superfund Program. NPL sites may encompass relatively large areas. As such, EDR
provides polygon coverage for over 1,000 NPL site boundaries produced by EPA’s Environmental Photographic Interpretation

NPL Delisted: Delisted NPL The National Oil and Hazardous Substances Pollution Contingency Plan (NCP) establishes
the criteria that the EPA uses to delete sites from the NPL. In accordance with 40 CFR 300.425.(e), sites may
be deleted from the NPL where no further response is appropriate. Delisted NPL - National Priority List Deletions
CERCLIS: SEMS SEMS (Superfund Enterprise Management System) tracks hazardous waste sites, potentially hazardous
waste sites, and remedial activities performed in support of EPA’s Superfund Program across the United States.
The list was formerly know as CERCLIS, renamed to SEMS by the EPA in 2015. The list contains data on potentially
hazardous waste sites that have been reported to the USEPA by states, municipalities, private companies and private
persons, pursuant to Section 103 of the Comprehensive Environmental Response, Compensation, and Liability Act
(CERCLA). This dataset also contains sites which are either proposed to or on the National Priorities List (NPL)
and the sites which are in the screening and assessment phase for possible inclusion on the NPL. SEMS - Superfund
Enterprise Management System
NFRAP: SEMS-ARCHIVE SEMS-ARCHIVE (Superfund Enterprise Management System Archive) tracks sites that have no
further interest under the Federal Superfund Program based on available information. The list was formerly known
as the CERCLIS-NFRAP, renamed to SEMS ARCHIVE by the EPA in 2015. EPA may perform a minimal level of assessment
work at a site while it is archived if site conditions change and/or new information becomes available. Archived
sites have been removed and archived from the inventory of SEMS sites. Archived status indicates that, to the
best of EPA’s knowledge, assessment at a site has been completed and that EPA has determined no further steps
will be taken to list the site on the National Priorities List (NPL), unless information indicates this decision
was not appropriate or other considerations require a recommendation for listing at a later time. The decision
does not necessarily mean that there is no hazard associated with a given site; it only means that. based upon

or entities that move hazardous waste from the generator offsite to a facility that can recycle, treat, store,
