N A... ,4.. .W E-. o if; 0 f-, = H Z CA , , Department of Public Works O Consent Agenda Page 1 of 1 Jefferson County Board of Commissioners Agenda Request To: Board of Commissioners Mark McCauley, County Administrator From: Monte Reinders, P.E., Public Works Director/County Engineer Agenda Date: June 27, 2022 Subject: Change Order No. 5, 6, and 7, for SR 116 and Cedar Avenue Pedestrian - Bike Improvements Project, Project No. 1802057, County Road No. 637109, WSDOT Project No. HLP-SR19(006) Statement of Issue: Change Orders No. 5, 6, and 7 with Interwest Construction, Inc. of Burlington, Washington. Analysis/Strategic Goals/Pro's £t Con's: These change orders consists of: Change Order No. 5: Removal of a segment of cement conc. driveway entrance and traffic curb and gutter at the 1ST Street and Cedar Ave. intersection. This work is necessary to correct the Plan dimension of a too-high gutter-lip, correct omission of the detectable warning surfaces by the contractor, and allow for construction of an ADA compliant cement conc. driveway entrance and traffic curb and gutter, for the two proposed pedestrian crossings at that intersection. Change Order No. 6: Removal of unsuitable native material from the Multi-use Path location, and replacement with crushed surfacing top course material to provide for a sound base for the path. Change Order No. 7: Part 1: Addition of crushed surfacing top course to the Vegetated Filter Strip to provide a firmer surface. Part 2: Adjustment of the existing water valve casing to finished grade in the Multi- use Path. Part 3: Adjustment of the vehicle swing gate at the west end of the Multi-use Path to provide clearance above the asphalt pavement. Fiscal Impact/Cost Benefit Analysis: Change Order No. 5 adds $2,742.60 and 1/2 Working Day. Change Order No. 6 adds $5,141.14 and no Working Days. Change Order #7 adds $3,565.06 and no Working Days. This project is funded at 100% by a WSDOT Safe Route to School Program (State) grant. Department of Public Works O Consent Agenda Page 2 of 2 Recommendation: Public Works recommends that the Board authorize Change Order No. 5, 6, and 7 with Interwest Construction, Inc., and return two originals of each to Public Works for further processing.
