SUBJECT: Agenda Item —Amendment #2 to the Professional Services Agreement— with
Concerned Citizens; to provide Early Intervention Services for Children - Birth to
Three; September 20, 2021 — June 30, 2023; an additional $16,125.00 in funding
for a total of$30,624.30 for this agreement.
STATEMENT OF ISSUE:
Jefferson County Public Health, Developmental Disabilities Division, is requesting Board approval of
Amendment #2 to the Professional Services Agreement with Concerned Citizens. They will provide early
intervention services for children, aged birth to three years, who experience a developmental delay; in
Jefferson County, Washington, September 20, 2021 — June 30, 2023; an additional $16,125.00 in funding for a
total of$30,624.30, for this agreement.
ANALYSIS/STRATEGIC GOALS/PRO'S and CON'S:
Amendment #2 to the Professional Services Agreement with Concerned Citizens; will fund early intervention
services for children, aged birth to three years; who experience a developmental delay in Jefferson County.
Jefferson County Public Health, the Developmental Disabilities Division and the Jefferson County
Intellectual/Developmental Disabilities Advisory Board, recommends funding this agreement.
FISCAL IMPACT/COST BENEFIT ANALYSIS :
Funding for Amendment #2 to the Professional Services Agreement with Concerned Citizens; is through DSHS,
Developmental Disabilities Administration; under Child Development Services in the County DD Program
budget.
RECOMMENDATION:
Jefferson County Public Health, the Developmental Disabilities Division, requests approval of Amendment #2
to the Professional Services Agreement with Concerned Citizens; to provide Early Intervention Services;

Contract Amendment#2
Between
Concerned Citizens
And
Jefferson County Public Health
Developmental Disabilities Program
WHEREAS, Concerned Citizens (Subcontractor) and Jefferson County (County) entered into an
agreement on September 20, 2021 for Professional Services; that provide Early Intervention Services for
children aged birth to three years, who experience a developmental delay; in Jefferson County,
Washington.
WHEREAS, the parties desire to amend the terms of that agreement.
IT IS AGREED BETWEEN BOTH PARTIES AS NAMED HEREIN AS FOLLOWS:
1. The term of the above referenced agreement ends June 30, 2023.
2. Subcontractor's contract will be increased by $16,125.00 in funding for services rendered
during the term of July 1, 2022 through June 30, 2023. Total compensation under this
Agreement shall not exceed $30,624.30, without express written amendment signed by
both parties.
3. Work performed between July 1, 2022 and the execution of this Agreement that is
consistent with the provisions of this Agreement is hereby ratified.
4. All other terms and conditions of the agreement will remain the same.
