CONTRACT FOR PROFESSIONAL SERVICES
Environmental Health Specialist — Health Department
This agreement is made and entered into by the Bremerton - Kitsap County
Health Department (hereinafter referred to as Kitsap Health) and the Jefferson
County Health Department (hereinafter referred to as Jefferson Health).
WITNESSETH:
WHEREAS, Jefferson Health is in need of the services of an Environmental
Health Specialist II (EHS II); and
WHEREAS, Kitsap Health has personnel skilled in environmental health and
is prepared to provide the services of an EHS II to Jefferson Health;
NOW, THEREFORE, IT IS AGREED BETWEEN BOTH PARTIES AS NAMED HEREIN AS FOLLOWS:
SECTION I. SERVICES PROVIDED.
Kitsap Health shall provide the services of an EHS II to Jefferson Health
for the provision of environmental health services. Such services shall
be provided within the scope of the regulations, policies, and procedures
of Jefferson Health and under the general supervision of the Jefferson
Environmental Health Director.
SECTION 2. RESPONSIBILITIES OF JEFFERSON HEALTH.
Jefferson Health shall reimburse Kitsap Health the sum of $2,154
for the salary and fringe benefits of an EHS II. Jefferson Health will
pay directly to the EHS Il the following: travel costs at 20� per mile
for properly documented on- the -job mileage, mileage and ferry fares for
a once -a -week round trip from Port Townsend to the EHS II's home, and
reasonable costs for lodging.
SECTION 3. EFFECTIVE DATES.
This agreement will be in effect for the following inclusive 8 hour
workdays: June 8 -11, June 21 -25, June 28 -July 2, July 6 -9, and July 12 -15, 1982.
SECTION 4. INSURANCE.
Both Kitsap Health and Jefferson Health will maintain comprehensive
general and professional liability insurance.

SECTION 5. AMENDMENTS.
This agreement may be amended by written consent of both parties,
and all amendments shall be attached to this agreement as a part
thereof.
