STATEMENT OF ISSUE:
Jefferson County Public Health (JCPH) requests Board approval of Consolidated Contract Amendment #6
between JCPH and State of Washington Department of Health (DOH); January 1, 2022 — December 31, 2024;
additional $1,433,000 for a total of $4,542,110.
ANALYSIS/STRATEGIC GOALS/PRO'S and CONS:
The purpose of this agreement is to provide public health services to the people of Washington State. This
Amendment provides additional funding ($1,433,000) for "Foundational Public Health Services." Foundational
Public Health Services (FPHS) are described as core services which the governmental public health system is
responsible for providing in a consistent and uniform way in every community in Washington.
The grant provides funds for the following programs:
For Local Health Jurisdiction (LHJ) priorities for activities to maximize equitable,
effective and efficient delivery of FPHS to every community in Washington .................. $520,000
Providing flexible funds to each LHJ to meet locally identified needs .................................... 60,000
To support any CHA/CHIP activity or service and may be used to contract with other
agencies for staff time or services................................................................................ 30,000
To increase assessment and epidemiology capacity to meet locally identified needs............ 150,000
To establish a model program for State Environmental Policy Act reviews ............................. 47,000
To fund education, communications and response needs for wildfire smoke and harmful
algalblooms . ............................................................................................................. 80,000
To develop a model program for homelessness and community engagement strategies......... 75,000
To increase Data Management Teams at the local level ...................................................... 55,000

Total consideration for this Contract Amendment is an additional $1,433,000 for a total of $4,542,110. The
Consolidated Contract is funded by DOH, and comprises both Federal and State funds.

PURPOSE OF CHANGE: To amend this contract between the DEPARTMENT OF HEALTH hereinafter referred to as
"DOH", and JEFFERSON COUNTY PUBLIC HEALTH, a Local Health Jurisdiction, hereinafter referred to as "LHJ",
pursuant to the Modifications/Waivers clause, and to make necessary changes within the scope of this contract and any
subsequent amendments thereto.
IT IS MUTUALLY AGREED: That the contract is hereby amended as follows:
