HomeMy WebLinkAbout032 75RESOLUTION NO.J WHEREAS, the Pacific Northwest Evangelical Free Church Homes (Lagoon Nursing Home) and Jefferson General Hospital (Hospital) require the services of a social worker, and �Z EREAS, the County can provide the services of such a social worker, and WHEREAS, the parties requiring the services will reimburse the County for those services, and WHEREAS, the agreements between the County and the Lagoon Nursing Home and the Hospital were signed by the County Commissioners on May 1975, and whereas both agreements have been fully executed by all parties (true copies of which are attached), and WHEREAS, it appears in the public interest that these services should be provided by the County as set out in the attached agreements. NOW, THEREFORE, IT IS RESOLVED that the County should enter into an agreement With Pacific Northwest Evangelical Free Church Homes and with Jefferson General Hospital to provide the services of a social worker; IT IS FURTHER RESOLVED that the execution of the said agreements by the Board of County Commissioners on May �j , 1975, is hereby ratified. Mercer, 14ember Attest: J� T 1�, -(!bunt for and ' f io Clerk Board Resolution No. AGREEMENT THIS AGREEMENT is entered into thisa, f day of May, 1975, by and between JEFFERSON COUNTY, MM1IINITY COUNSELING SERVICE, hereinafter referred to as the "County," and PACIFIC NOICHWEST EVANGELICAL FRS CHURCH HOMES, hereinafter referred to as the "Lagoon Nursing Home." WHEREAS, the Lagoon Nursing Home requires the services of a social worker, and WHEREAS, the County can provide the services of a social worker, Now, Therefore, it is agreed: (1) Effective Date - The effective date of this Agreement is June 1, 1975. This Agreement shall continue in effect for a period of one year, unless terminated in accordance with Paragraph (2), and shall continue in effect there- after until terminated. (2) Termination - This Agreement may be terminated by either party on thirty (30) days written notice. (3) Amount of Time - The County shall provide the Lagoon Nursing Home with the services of a social worker for eight (8) hours per week. (4) Educational Background - The social worker provided by the County shall hold, as a mininnn educaticnal requirement, a baccalaureate degree in sociology, social work, psychology or other related field. A minimum of two (2) years of paid experience in the social service field may be substituted for a B. A. major other than those prescribed. The social worker shall be under the supervision of Robert Goehring, County Mental Health Director, or other individual holding a Master's Degree in an appropriate academic field. (5) Duties - The social worker shall counsel patients and their families regarding changing family patterns and responsibilities, chronic illness, death and such other problems as may require counseling, serve as an information and referral person to patients and staff regarding commuri.ty resources, prepare patients social histories and determine financial resources, plan for patient discharge and perform such other duties as are required by the Lagoon Nursing Home and as are within the background and experience of a social worker. (6) Paynent - The Lagoon Nursing Home shall pay the County at the rate Agreenent - P. 1 Of SIX and 00 /100 Dollars. ($6.00) per hour for the social worker. The Lagoon Nursing Home shall pay for eight (8) hours services each week. (7) Billing - The County will bill the Lagoon Nursing Home ironthly for social worker services. (8) Responsibility For Work Assignments - The Lagoon Nursing How Administrator or Director of Nursing Services shall advise the social worker of the work to be performed. The social worker and his supervisor shall make all decisions regarding the manner in which that work is to be acccuplished. When work is assigned by the Lagoon Nursing Home Administrator which cannot be ac=rPlished by the social worker in eight (8) hours per week, the lagoon Nursing Hone Admudstrator may determine priorities for the work assigned. (9) Extra Costs - All costs and expenses incurred by the social worker for long distance telephone charges, travel and other direct expenses related to work performed by the lagoon Nursing Home as authorized by the Administrator shall be reimbursed by the Lagoon Nursing Home. (8) Responsibility For Work Assignments - The Hospital Administrator shall advise the social worker of the work to be performed. The social worker and his supervisor shall rrake all decisions regarding the manner in which that work is to be acosplished_ When work is assigned by the Hospital Altninistrator which cannot be acocnplisbed by the social worker in twelve (12) hours per week, the Hospital At=istrator may determine priorities for the work assigned. (9) Extra Costs - All costs and expenses incurred by the social worker for travel, clerical work, long distance telephone charges and other direct expenses related to work perfored by .the Hospital shall be reimbursed by the Hospital.
