SUBJECT: Agenda Item — Consolidated Contracts Amendment #7 with the Department
of Health; January 1 2022 — December 31, 2024; decrease of $437,599 for a
total of $4,104,511
STATEMENT OF ISSUE:
Jefferson County Public Health (JCPH) requests Board approval of Consolidated Contract Amendment #7
between JCPH and State of Washington Department of Health (DOH); January 1, 2022 — December 31, 2024;
decrease of $437,599 for a total of $4,104,511.
ANALYSIS/STRATEGIC GOALS/PRO'S and CON'S:
The purpose of this agreement is to provide public health services to the people of Washington State. This
Amendment identifies statement of work and funding for the following programs:
Adds a Statement of Work (SOW) for the following program:
• Public Health Emergency Preparedness (funding of$34,384)
Amends SOW for the following programs:
• COVID-19 Mass Vaccination-FEMA, extends period of performance
• Culturally and linguistically responsive testing to limit spread of COVID-19 (decrease of$2,281)
• Foundational Public Health Services, changes period of performance (decrease of$550,000)
• Maternal & Child Health Block Grant, adds activities, extends period of performance (additional
$36,700)
• On-site sewage system repair, to protect threated shellfish area, extends period of performance
(decrease of $1,654)
• Promotion of Immunizations to Improve Vaccination Rates, revised activities
• Sexual and Reproductive Health Program, revises SOW language (additional; $39,652)
• WIC Nutrition Program (additional $5,600)

FISCAL IMPACT/COST BENEFIT ANALYSIS :
Total consideration for this Contract Amendment is a decrease of $437,599 for a total of $4,104,511. The
Consolidated Contract is funded by DOH, and comprises both Federal and State funds.
RECOMMENDATION:
JCPH Management recommends BOCC approval of Consolidated Contract Amendment #7; January 1 2022 —
December 31, 2024; decrease of $437,599 for a total of$4,104,511.
