HISTORY: New Manufactured Home. Vacant property. Applicants have applied for a well decommision (USR2022-00037). Septic approved
under SEP2019-00081. No applicable plat conditions (tax parcel).
Parcel Tag: 2022: "The existing abandoned well must be decommissioned in accordance with WAC 173-160-381: Any well which is unusable,
abandoned, or whose use has been permanently discontinued, or which is in such disrepair that its continued use is impractical or is an
environmental, safety or public health hazard shall be decommissioned. The decommissioning procedure (as prescribed by these regulations)
must be recorded and reported as required by the department. Contact Jefferson County Public Health or a licensed well driller to obtain
directions for decommissioning".

900 feet away. No further action is required.

GEOHAZARDS: There is a Geologically Hazardous Area (Seismic Hazard Area) located on-site. The proposed NSFR is located more than 200
feet away from the hazard aream and the general area is mapped as "very low" liquefaction susceptibility. No geohazard report is required.

FOREST LANDS: DNR Response on 10/11/2022: "The L/O sent me an email stating that they aren’t removing timber and the area is already
clear. From looking at 2017 imagery, there was a structure on-site and the area was already non-forestry use. From looking at google earth
imagery (2021) the structure was removed to include scattered trees widening the clearing. Those trees were most likely danger trees within the
distance of old structure. Since this clearing/footprint already had a structure and no forestland, I don’t see any triggers for an FPA/N as long as
no additional trees are removed from site (The clearing I see as of google earth 2021 imagery was already converted to a non-forestry use). As
per FPA 266814, it does not appear that clearing limits crossed parcels".
