Any future application will be separately judged by the rules and laws in effect at that time. All construction and development activities must comply with all permit conditions, state and local codes, and Recommended Standards and Guidance documents in effect when the permit is issued. The property owner is responsible for the accurate location of all property lines. Any removal of or major disturbance of soil in the primary or reserve drainfield area may create site conditions that are unacceptable for the installation of a sewage disposal system Any change in drainfield or tank location may invalidate this permit unless prior approval is obtained from the Jefferson County Environmental Health Division. lf during excavation or development of the site an area of potential archeological significance is uncovered, all activity in the immediate area shall be halted, and the UDC Administrator shall be notified at once. Permit issued to CONSTRUCT, ALTER, REPAIR OR MODIFYAN INDIVIDUAL SEWAGE DISPOSAL SYSTEM IN JEFFERSON COUNTY, WASHINGTON This permit is issued for a period of three years (unless otherwise stated above) in accordance with Jefferson County Rules and Regulations for On-Site Sewage Systems, codified in JCC 8.15 as amended. H - The existing smallA-frame building is to be demolished as shown on the approved design OR permited as a non-living structure through the Building Department. 2.) Approval/issuance of a sewage disposal permit or installation of a septic system does not guarantee the approval of other development or a building permit on this site. Future buildings that require connection to an on-site sewage system (OSS) shall only be approved if the OSS meets the current standards and codes in effect at the time of the building application. 3.) Designer must be contacted prior to start of construction and for inspections during installation. DESIGNER IS REQUIRED TO DO A PRECOVER INSPECTION ON ALL TYPES OF SYSTEMS. 4.) Notification of the start of construction shall be faxed or emailed to Jefferson County Public Health ONE WORKING DAY prior to start. 5.) H - This permit is granted based on the recorded Statutory Warranty Deed that ties together lot 64 and tax 4 underAFN 296107. Separating these properties will void septic permits SEP2021-00121 & sEP1985-00107. 6.) H - An asbuilt drawing and certification of completion by the Designer is required prior to final approval. 7.) Before final approval is given, the designer shall provide an operations and maintenance manual to the property owner and the Health Department. The manual must instruct the owner of the on site sewage system on the ways to properly operate and maintain all components of the system. 8.) When/if designated reserve area is utilized an alternative system providing an additional level of treatment may be required and must comply with code at the time of application for use. 9.) Divert all sources of drainage, including roof drains away from septic tank and drainfield area. 10 ) Approval of this sewage disposal permit does not preclude the permit holder from complying with the Unified Development Code for other/future development on the site. 11 ) This system as designed and approved is sized for only one single family residence. lt is not sized for an Accessory Dwelling Unit (ADU). The minimum daily design flow per residence is 240 GPD. 12.) The project shall adhere to the Best Management Practices (BMPs) for the control of stormwater, erosion and sediment during construction. BMPs shall address permanent measures to stabilize soil exposed during construction, and in the design and operation of stormwater and drainage control systems. 13.) All construction and development activities must comply with all permit conditions, Washington State and Jefferson County Codes and Recommended Standards and Guidance documents in effect when the permit is issued. 14 ) H - AS PER WAC 246-272AAND JEFFERSON COUNTY CODE 8.15 ALL ONSTTE SEWAGE SYSTEMS REQUIRE THATA RESTRICTIVE COVENANT REGARDING THE MONITORING OF THE ONSITE SEPTIC SYSTEM BE RECORDED TO THE PROPERTY TITLE. THE PROPERTY OWNER SHALLASSURE THAT MONITORING IS PROVIDED BYAN APPROVED ENTITYAT THE FREQUENCY DEFINED PER STATE WAC 246.2724AND JEFFERSON COUNTY CODE 8.15 AS ADOPTED OR AMENDED. A COPY OF THE RECORDED OPERATIONS AND MONITORING AGREEMENT IS REQUIRED PRIOR TO FINALAPPROVAL OF THE SEWAGE DISPOSAL PERMIT 15 ) This APPROVAL is for a new septic system only. The revised site plan as submitted with the new septic system application on September 201h, 2021 has been reviewed for consistency under the UDC by Jefferson County Department of Community Development. Any modifications, changes, and/or additions to the site plan shall be resubmitted for review and approval by Jefferson County Department of Community Development. 17 ) The Geographic lnformation Systems mapping review identified the following CriticalAreas on the subject property: Fish Bearing Stream, Wetlands, Geological Hazardous Area (Seismic Hazard Area), "A" Flood Zone, and Susceptible Critical Aquifer Recharge Area. The proposed disturbance area is located outside of the fish bearing stream buffer, wetland area, and "A" flood zone. 18.) A Geotechnical Report Waiver request was submitted on September 241h,2021to be waived from the requirement to submit a Geotechnical Report. The request was approved on October 6th,2021 by the UDC Administrator. The applicants have submitted a notarized hold harmless compliant with Jefferson County Code (JCC) 18.22.550 on October 14th, 2021. The document is recorded under AFN 648673. 19 ) Per JCC 18.22.530(c), clearing and grading shall be limited to the period between May 1st and October 1st, unless the applicant provides an erosion and sedimentation control plan prepared by a professional engineer licensed in the state of Washington that specifically and realistically identifies methods of erosion control for wet weather conditions. 20.) Per JCC 15.15.080, no work or fill shall occur within the Federal Emergency Management Act (FEMA) regulated flood zone, as shown on the approved septic site plan, without review and approval from the Jefferson County Department of Community Development. Work includes, but is not limited to, the temporary storage of debris. lf work or fill occurs within the FEMA regulated flood zone, the applicant shall immediately contact the Jefferson County Department of Community Development (360-379 -4450). 21 ) Washington State Department of Archaeology and Historic Preservation (DAHP) has identified parcel 995400061 as a high risk area of containing archaeology. DAHP has instructed Jefferson County to implement an lnadvertent Discovery Plan in lieu of an archaeological excavation permit and survey. lf potential archaeological sites are unearthed or discovered on the parcel during any ground disturbance or construction, the contractor and property owner must immediately halt all work, secure the location, and contact DAHP. 22 ) All contractors and personnel shall be familiar with the inadvertent discovery plan as attached to this permit. lf any possible historic, archaeological and/or cultural artifacts are inadvertently discovered, the applicant shall immediately stop all work on the project and shall notify the Washington Department of Archaeology and Historic Preservation, Jefferson County Department of Community Development, and affected tribes. sEP2021-00121 \\tidemark\data\forms\F_SEP_Permitmod. rpt Page 3 of 3 11t3t2021
