Subject: Final Special Inspection Letter

Dear Ms. Charnas:
In response with the request and authorization of MasTec Network Solutions, Krazan & Associates
has provided special inspection services for the Mt Octopus MW Hub-CX project located at 11235
Hoh Mainline, Forks, WA 98331. Our services were performed by our staff under the supervision
of a State of Washington registered engineer.
Our observations were documented in our inspector’s daily field reports, which have been sent to
the County for review. Krazan provided special inspection for the following types of construction
at the project site:
Proprietary Anchor Inspection (Mechanical)
Visual Observation of New Antenna Attachments
In accordance with Chapter 17 of the 2015 International Building Code, the work requiring special
inspection was performed in conformance with approved plans and applicable standards of
workmanship at the time of completion.
LIMITATIONS
We have performed our services in a manner consistent with the standard of care and skill
ordinarily exercised by firms of our type practicing under similar conditions at this time and
locality. Please recognize that construction monitoring is a technique employed to reduce the risk
of problems arising during construction. Provision of construction monitoring by Krazan &
Associate’s personnel is not insurance, nor does it guarantee construction of any type. In all cases,

the contractor shall retain sole responsibility for the quality of work, adhering to plans and
specifications and for repairing defects, deficiencies or omissions, regardless of when they are
found. Our services were limited to the work documented in our reports. We do not undertake
the guarantee of construction or completion of a completed project conforming to the plans and
specifications. No warranty express or implied.
