Hi David & Nicole,
Please publish a notice of application for MLA05-00276 (SDP05-00014 & ZON05-00027) in the
Wednesday, June 8, 2005 edition of the Port Townsend Leader (if possible).
1. Add the activities for noticing to both cases, but publish the notice from SDP05-00014. (It's a Type III
permit while the zoning is a Type II.)
2. Please let me review the notice prior to publishing and mailing to make sure the standard language on the
form generated is accurate.
3. Mailing:
. Notice & map (Log #22, p 4 of 4) only to APO's & newspapers
. Agencies to copy have already been entered in ZON05-00027 and are on the review form. Please add
the agencies to SDP05-00014.
. Include with the notice for all agencies except the newspapers please

2. Master Permit App (#22)
3. JARPA (#23)
4. SEPA checklist info (#24)
5. Stormwater Plan (#25)
6. Conditional Use Permit application (#26)
7. Master Shoreline Narrative (#27)
. Send the Notice for posting and Affidavit of posting to Jim Parker, representative for PUD #1.
