de&Oribedlo~locks: Portions of the recorded plat of Ludlow Bay Vil1ag~ (AF#372517) under
Port Ludlow Associates LLC Ownership. See Exhibits A & B attached.
The reason for requesting the amendment is as follows: Plat Alteration via Major Revis-ion in conjunction
- with _~he Resort Supplemental Environmental Impact Statement process.
If the subdivision is subject to restrictive covenants and the proposed amendment would ~su1t in the violation of
the covenant(s), explain here and attach agreement signed by all parties subject to the covenant stating that the
parties agree to the amendment or agree to terminate the relevant covenant(s) to accomplish the purpose of the
subdivision amendment. N / A -
