SUBJECT:
Application to Open Right-of-way, Karl Jacobson
It is my understanding that Al Scalf has informed your Department that we will accept the SEP A fees, do
the noticing requirements, mail-ou,ts and track the case, but that your Department will do the threshold,
staff report, and final threshold on the above proposal.
Before we can proceed, tl).e following items are needed:
1. Original SEP A checklist,
2. APO list prepared by a Title Company doing business in Jefferson County, per the Land Use
Procedures Ordinance,
3. Map
4. Fee in the amount of $398'.00.
Once the above items are received, we will assign a case number and forward the checklist to you for a
threshold determination.
