SUBJECT: Agenda item — Amendment 1 to Fleming Professional Services Agreement for
Jefferson County's Behavioral Health Consortium Project Director Services;
October 1, 2022 — extended to August 31, 2024, Additional $10,000 for a total
of $136,000.
STATEMENT OF ISSUE:
Jefferson County Public Health (JCPH) requests Board approval of Amendment 1 to the Professional Services
Agreement between Lori J. Fleming and JCPH to provide a No-Cost Extension (NCE) for direction and
management of the Behavioral Health Consortium, and all of its ancillary sub-groups, and submission of all
required elements of Health Resources and Services Administration (HRSA) RCORP-Implementation grant
GA1RH39564.
ANALYSIS/STRATEGIC GOALS/PRO'S and CON'S:
HRSA's approval of an NCE for this grant has prompted changes to be made to this contract to support the
timeline, budget, and scope of work outlined in Exhibit C of the Amendment.
FISCAL IMPACT/COST BENEFIT ANALYSIS:
This extension is fully funded by HRSA Grant GA1RH39564. There is no fiscal impact.
RECOMMENDATION:
JCPH management requests approval of Amendment 1 to the Professional Services Agreement between Lori
J. Fleming and JCPH for management and coordination of the Behavioral Health Consortium, and all of its
ancillary sub-groups; October 1, 2022 — August 31, 2024, Additional $10,000 for a total of $136,000.
