PROJECT DESCRIPTION: Maintenance and repair of a deteriorated effluent line
which serves the primary clarifier project
The development is proposed to be within PORT TOWNSEND BAY
and/or its associated wetlands.
LEGAL DESCRIPTION: Parcel 001 152 006 in Section 15, Township 30 North,
Range 1 West, WM. At the Port Townsend Paper Mill.
FINDINGS:
1) The proposal is consistent with Exemptons Section 3.402(6) of the
Jefferson -Port Townsend Shoreline Management Master Program.
2) A site inspection was conducted by Permit Center staff on 3/26/98.
3) All portions of the above ground pipeline will be removed and replaced
with new piping. The portions of the pipeline which are located below
ground will be refurbished with insertion of a resin liner within the
existing pipe.
NOTICE: This exemption does not excuse the proponent from complying
with other local, state, and federal ordinances, regulations, or statutes
applicable to the proposed development.
Development pursuant to this permit shall be undertaken subject to the
applicable policies and performance standards of the Jefferson County
Shoreline Management Master Program.

1) This approval pertains to Jefferson -Port Townsend Shoreline
Management Master Program requirements only. The approval of any
other agency with jurisdiction or any other required permits shall be
obtained prior to undertaking the project.
2) Work within the jurisdiction of the Shoreline Program other than as
described above shall receive separate review from this office.
3) No fill or other material may be placed in the waters or intertidal
areas of Jefferson County.
4) Prior to commencing construction, proponent should contact the
Washington State Department of Fisheries at (360) 895-4757 to
determine whether this action is subject to the Washington State
Hydraulics Code.
5) The proponent shall notify the Jefferson County Permit Center, (360)
379-4450, Development Review Division 48 hours in advance prior to
commencing construction in order to allow monitoring and ensure
conformance with project conditions.
6) The applicant shall provide the Permit Center with a receipt from
the Olympic View Sanitary Landfill, which verifies all pipe waste
materials were properly disposed of.
7) The temporary by-pass shall not be installed over water.
8) The effluent line shall be flushed with clean water for a minimum of
20 minutes to insure that no waste is released to the shoreline
waters while the pipe is being removed.
9) Spill catchment containers shall be placed under the joints (weak
points) of the piping during removal.
10) Should an accidental spill occur during this maintenance activity,
the applicant shall fully implement the "Port Townsend Paper
Corporation Environmental Response Manual" Attachment B, dated
Revised 1996.

Dear Ms. Mark:
Regarding our Shoreline Exemption Permit application for the Rebuild Effluent Line to
Primary Clarifier project I would like to offer the following clarifications:
1. Our original plan to abandon the underground portion of this piping system is no
longer valid. We will now be installing a resin liner in the existing underground
concrete cylinder pipe. The above ground portion will still be replaced with new FRP
pipe.
2. We will remove the above -ground concrete cylinder pipe and dispose of it at Olympic
View Sanitary Landfill facility in Port Orchard, Washington.
3. The effluent line will be flushed with clean water for twenty minutes after the effluent
is removed so that no waste will be released to the bay while the pipe is being
removed.
4. The temporary by-pass will not be installed over water but will follow a land route to
the northwest corner of the primary clarifier where it will connect with the 36" clarifier
discharge line.
I have also included select portions of our Spill Prevention Control and Countermeasure
Plan and Environmental Response Manual for your review. Complete copies of this
document are on file with Bob Minty Jefferson County Emergency Management Director,
Jefferson General Hospital and Bob Larson, Jefferson County Fire District # 6. All
construction crews involved in this project will be provided this document and instructed
as to how to summon the mill's Emergency Response Team in the event of an
environmental incident. Additionally, the contractor will be provided with a supply of
absorbent pads to clean up any residual moisture encountered while working on this line.
Also enclosed are the pipe calculations for the 16" HDPE bypass line indicating that this
system is capable of handling our peak flows of 8500 gpm. This line will be pressure
tested at maximum system pressure for twenty minutes before being put into service.
Finally, I have included the letter from Port Townsend Paper Corporation responding to a
request by the Washington Department of Ecology for additional information. Our
commitment to the D.O.E. regarding repair of this system is indicated in this letter.
Subsequent to this letter, the shut -down schedule for the mill has been finalized and our
involvement is reflected in our scheduled installation dates from May 4 to May 15, 1998. I
did not include the piping diagrams referenced in this document as they have been
submitted with the JARPA application.

Subject: PTPC Response to WDOE Request for Additional Information Dated 21 July 97
Dear Marc,
In response to your request for additional information dated 21 July 97 regarding the process
sewer line incident of 23 April 97 at Port Townsend Paper Corporation's (PTPC's Port
Townsend, WA facility. We are providing this information within the 30 day time period
specified in your letter.
1. Please provide a map of the waste water line. The map should identify those areas
with "similar characteristics to the section that ruptured"! A date for completion of
the "semi -permanent overwrapping" of those areas must be provided.
Enclosed please find a series of maps (piping diagrams) related to the referenced waste
water line. As requested, sections we believe having "similar characteristics to the
section that ruptured" are highlighted. These piping diagrams were prepared by Mr. Ed
Bellero of PTPC's Engineering staff.
Mr. Bellero has been obtaining cost estimates and schedule availability. In discussions
with contract fiberglass companies, we would expect to have the necessary fiberglass
"semi -permanent overwrapping" of potentially impacted sections completed by
31 October 97.
2. A date for completion of the line replacement/in situ lining must be provided. The
date must be specific for the line replacement/in situ task, rather than linked to
another project.
In order to inspect, in situ line and/or replace sections, we will have to "cut off' waste
water flow from the Main Station to/through the Primary Clarifier. The Primary Clarifier
contains piping directly connected to the waste water system. As stated in my 10 July 97
letter to you,

"Detailed inspection of the line between the Main Pump Station and the
Primary Clarifier in its entirety are not possible during routine operations,
or even during a "partial down". According to Gelco, a contractor who
was brought in to consult on repairs and options, a "video camera"
inspection of the interior line would require drainage of the line and access
ports being cut for insertion of the robot camera."
We acknowledge that the Process Sewer Line repair is a separate project; however, we
believe a more complete analysis and repair of this section of the process waste water
system can be performed during the planned "major work" on the Primary Clarifier,
rather than "piecemeal". We plan on and can commit to completion of the line
replacement/in situ lining AND any necessary Primary Clarifier system maintenance by
31 October 98.

MILLWIDE POLLUTION CONTROL RESPONSE
It is very important to report all incidents which can have environmental impacts at the
mill. If you become aware of such an incident, please follow the outlined procedure.
1. Initial contact person: Pulp Mill Foreman
For any spill or unpermitted release to the bay, beach, atmosphere, etc., the first person
that should be contacted is the Pulp Mill Foreman. The Pulp Mill Foreman will evaluate
the situation and take initial steps to stop and contain the spill and to protect property,
people, and the environment. He or she will also contact:
2. The Power and Recovery Superintendent who is general coordinator for spill
response. If he is not available, then the Pulp Mill Superintendent or, in both of their
absences, the Back End Manager will assume these duties. For names and phone
numbers see the call list in the front of this book. The coordinator will oversee and
coordinate actions to:
a) Stop the spill
b) Contain the spill
c) Clean up the spill
d) Notify the necessary agencies
He will have two people to assist him in carrying out these duties; one from the
Environmental Department and one from the Maintenance Department. For names and
phone numbers see the call list.
The designee from the Environmental Department will assist in:
a) Contacting the necessary governmental and local agencies
b) Determining immediate and long term hazards
c) Determining appropriate clean up procedures
d) Collecting samples and information necessary for a complete and accurate report.
The designee from the Maintenance Department will assist in:
a) Directing actual containment and clean up activities
b) Mobilizing men, equipment and supplies, and directly supervising the work.

3. If the environmental incident results in a violation of our permits, the Department of
Ecology must be notified. Before reporting, refer to the `CERCLA / SARA Title III'
section to determine if SARA spill reporting is required. Normally the Environmental
Department person contacted by the general spill coordinator calls the governmental
agencies as appropriate. The Department of Ecology must be notified with the following
information:
a) Your name and phone number.
b) Name of material and circumstances leading to our permit violation (e.g. equipment
failure, broken sewer line, pump station failure, ruptured tank, fire, process upset,
etc.).
c) Estimate of quantity released. _
d) Time release began.
e) Duration of release (NOTE: If release is still occurring, say so).
f) What actions have been undertaken to stop, contain and clean up release.

4. If you are unsure of the impact of any incident, log it in and call anyway. A11 persons
failing to report a violation of our permits are subject to civil and criminal penalties. It's
better to report it.
Quick response in reporting and correcting any environmental mishaps goes a long way
to reducing the amount of any penalty assessed.

OIL SPILL RESPONSE PROCEDURES
1. Note the time you have been notified of an oil slick.
2. Get an overview of the situation including:
a) size of slick.
b) source of spill.
c) type and quantity of oil.
3. Contact the General Spill Coordinator, the Power and Recovery Superintendent.
If he is not available, then contact the Pulp Mill Superintendent or, in both their
absences, the Back End Manager will assume these duties. For names and phone
numbers see the call list in the front of this book.
4. Have crew deploy boom to contain spill and begin the clean up. If necessary pull
fire alarm 10-1-1 located on the east side of the dock, 150 feet north of oil
unloading station to obtain necessary manpower to implement containment
procedures.
5. Follow the Incident Call List within 45 minutes of being notified of the spill:

MAINTENANCE DEPARTMENT MINOR OIL SPILL PLAN
1. Contain oil in small area by use of oil absorbent materials such as "PIG SOCKS",
"ZORB-ALL" and absorbent pads. Use containment materials to prevent oil from
escaping into sewer or bay.
2. Dispose of oil that can be picked up as a liquid in the used oil barrels by oil house.
3. Oil soaked "PIG SOCKS", pillows, mats, clay absorbent (e.g. "ZORB-ALL") are to
be disposed of in dumpsters or "PIG" refuse containers.
4. Finish cleaning floor with biodegradable cleaner.
5. Immediately notify your supervisor if oil goes down drain and/or into bay. During
swing and graveyard shifts and on weekends, notify pulp mill shift supervisor.

POLYCHLORINATED BIPHENYL (PCB) CLEANUP
The new rules on PCB spills went into effect as of December 21, 1989. Essentially these
rules address our response and cleanup in case of such a spill.
Possible PCB sources on our mill site which are covered by this law include transformers,
capacitors, switches or any other oil filled electrical devices.
In the event that any leaks from these sources are discovered, please do the following
immediately:
1. Check to see if spill is contained. Do Not handle the PCB fluid.
2. Cordon off the area with a 3 foot buffer so that no one will inadvertently be
exposed to the PCB's.
3. Contact the General Spill Coordinator, the Power and Recovery Superintendent.
If he is not available, then contact the Pulp Mill Superintendent or, in both their
absences, the Production Manager will assume these duties. For names and phone
numbers see the call list in the front of this book.
4. The Spill Coordinator will then contact:

5. Fred or George will estimate the quantity of materiel spilled. Any spill or fire
involving one pound or more must be reported immediately to the following:

EMERGENCY NOTIFICATION UNDER EPCRA, CERCLA AND SARA TITLE III
HOW TO DETERMINE IF A REPORTABLE RELEASE HAS OCCURRED
The Emergency Planning and Community Right -To -Know Act (EPCRA); The
Comprehensive Environmental Response., Compensation, and Liability Act (CERCLA)
and the Superfund Amendments and Reauthorization Act (SARA) Title III contain
emergency notification requirements in the event of releases of hazardous substances.
We are subject to these requirements. A listing of potential or possible releases by
PTPC is summarized Table 24-1-A of the SPCC Manual.
A chemical release is defined as any spilling, leaking, pumping, pouring, emitting,
emptying, discharging, injecting, escaping, leaching, dumping or disposing into the
environment (including the abandonment or discharging of barrels, containers, and other
closed receptacles).
Releases of CERCLA hazardous substances, in quantities equal to or greater than their
reportable quantity (RQ), are subject to reporting to the National Response Center.
Releases of Anhydrous Ammonia and Sulfuric Acid must also be reported to the State
and Local Emergency Response Commissions (SERC and LERC) under EPCRA.
Releases to the environment of these hazardous substances must be reported immediately
(less than one hour) to the appropriate agencies. Failure to report a release in a timely
fashion could subject Port Townsend Paper to a $75,000 per day non-compliance fine.
If a release remains on mill property and results in exposure solely to mill personnel and
does not have the potential to leave the millsite, the release does not have to be reported.
For example, if a white liquor spill occurs which exceeds the reportable quantity of 125
cu. ft. (935 gal) and is contained by the mill sewer system, dikes or other functional
barrier, a CERCLA/SARA reportable release has not occurred. If the same spill should
directly enter Port Townsend Bay or soak into the soil, it has left the mill site and must be
reported.
WHAT TO DO IF THERE IS A RELEASE:
Contact the general spill response coordinator, (the Power and Recovery Superintendent).
If he is not available, then the Pulp Mill Superintendent or, in both their absences, the
Production Manager/Back End Superintendent will assume these duties. For names and
phone numbers see the call list on Page B-3 of this book.
We are required by law to "immediately" contact the appropriate emergency response
authorities with technical information. Immediately means as soon as reasonably
possible at least within an hour. As stated in the general spill response plan, the
Environmental Department will assist the spill response coordinator in notifying the
necessary agencies.

To properly report a chemical release under CERCLA/SARA regulations the following
information must be assembled:
a) The name of the chemical released.

FIRE AT CHIP PILES
The fire crew at the Port Townsend Paper Corporation consists of a minimum of 12
people. This crew receives monthly training in the operation and maintenance of the
mill's fire protection safety systems.
High pressure pumps and hose stations are located throughout the mill in the event of a
fire. In the event of a small fire, chemical fire, or a fire that should not be fought with
water, fire extinguishers are also located throughout the mill. At a minimum, these
extinguishers are located at or near the automatic emergency alarms listed in Table 24-2
of the Spill Prevention Control and Countermeasure (SPCC) Plan.
1. In the event of a fire within the chip piles, the following emergency procedures
should be followed:
2. Pull the nearest fire alarm (see Table 24-3 of the SPCC Plan). This will bring the
mill's emergency response team to the area.
3. Notify the Person -in -Charge (see Table 22-1 of the SPCC Plan for a listing of
qualified Persons -in Charge and their phone numbers.)
4. Secure the area.
5. Assist the fire brigade where possible. Immediately adjacent to the fire -hose
connections are lockers marked "Fire -hose". These lockers contain 50-foot sections
of fire -hose.
If additional assistance is necessary, notify the Port Townsend Fire Department telephone
911) and other appropriate agencies as listed in Section 8 of the SPCC Plan

THE AFOREMENTIONED LINE STARTS AT THE MAIN PUMP STATION AND TRAVELS NORTHEAST TOWARDS SHORE FOR APPROXIMATELY 350 FEET, AT WHICH POINT IT GOES
UNDERGROUND. THIS PORTION OF THE PIPELINE 15 SUPPORTED BY PILING AT INVERT ELEVATION = 15.0', ABOVE THE HIGH WATER LINE OF 10.0'. TO ALLOW FOR CONTINUED
OPERATION OF THIS SYSTEM, WE WILL INSTALL A TEMPORARY BY-PASS LINE TO THE PRIMARY CLARIFIER.
WE WILL ALSO BE REPLACING THE UNDERGROUND PORTION OF THIS LINE FROM THE BEACH TO THE CLARIFIER. THIS PORTION WILL RUN UNDERGROUND FOR
APPROXIMATELY 350 FEET TO THE PRIMARY CLARIFIER. THE EXISTING LINE WALL BE DRAINED AND CLEANED TO PREVENT SOLID OR LIQUID EFFLUENT FROM ENTERING THE
BAY. EROSION CONTROL WILL BE IN EFFECT DURING THE COURSE OF THE PROJECT.
WE WILL BE REPLCACING THE UNDERGROUND PORTION OF THE 6' SLUDGE LINE AT THIS TIME. IT CURRENTLY FOLLOWS THE PATH OF THE EFFLUENT LINE AND WILL DO SO IN
THE NEW INSTALLATION.

Please find our enclosed JARPA Form for a Shoreline Exemption permit for our Rebuild
Effluent Line to Primary Clarifier project. We are seeking exemption on the basis that this
project involves replacing an existing effluent pipeline system and requires that no new
structures be built. This project is being done per our agreement with the Washington
State Department of Ecology, which requires that our work is completed by October of
this year.
As some of the work requires a shut -down of the mill effluent treatment system, we are
obligated to conduct our installation during our annual mill -wide shut -down, May 4
through May 11, 1998. I would therefore request that you proceed as quickly as possible
processing this application.
