SUMMARY
Develops, coordinates, implements and monitors the Intellectual/Developmental Disabilities
(IDD) Transition Program for students from school to work. Provides outreach and case
management for students and families. Builds transition program capacity in partnerships with
education, services providers, state agencies and others. Receives guidance from the
Developmental Disabilities Program Coordinator.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may
be assigned.
Identifies, advocates for, and provides information, education, and outreach to the community
including individuals 13-21 years of age with intellectual and developmental disabilities, their
families, schools, and advocates. Applies and integrates guiding values of the developmental
disabilities system within program services and activities.
Collaborates with community partners to improve service system understanding, access, and
outcomes for individuals with developmental disabilities across the lifespan.
Develops professional, collaborative relationships with the Jefferson County School Districts to
assist DDA-eligible students in connecting to appropriate post-graduation/adult services and
resources to support transition from school to community.
Builds capacity within the local community through active partnerships with education, service
providers, State agencies, and others to develop inclusive supported employment and ensure
clients are prepared for employment upon graduation from transition programming.
Coordinates and collaborates with other State service agencies including Developmental
Disabilities Administration (DDA), Department of Vocational Rehabilitation (DVR), and
Department to Services for the Blind (DSB), to support seamless transition for clients from
school to work.
Provides outreach and case management support for individual students and families to ensure
successful outcomes related to employment and connections to both informal and formal support
systems; leads and works through partnerships, multidisciplinary teams, and community groups
to address issues related to transition services.

Provides Community Outreach/Information and Education to connect with communities and
schools across the county in order to start and grow the School -to -Work program.
Monitors contracted qualified providers' transition and supported employment programming
frequently. Provides ongoing monitoring of individual students' plans and participates in quality
assurance monitoring reviews with contracted vocational agencies.
Coordinates County -funded transition programming. Conducts application process and outreach
to applicants. Monitors, coordinates services and approves billing. Tracks job stabilization and
works closely with schools, providers, transition programs, and DVR.
Collaborates with the County Developmental Disabilities Coordinator to continuously evaluate
program and engages in program development to ensure quality, appropriate services are being
provided.
Represents the County Developmental Disabilities Program in a professional, collaborative
manner at community events and beyond, acting as a primary source for outreach and
information in order to connect families to DDA, DVR, the County and Transition services.
Participates in regional and statewide transition networks; plans and coordinates meetings to
promote collaboration and to keep abreast of best practices related to transition and service
delivery.
Attends community, regional and statewide training/conferences, professional seminars and staff
meetings as required.
Designs and implements projects, training materials, workshops, events, and presentations for
professionals, schools, community partners and students/families related to transition, best
practices for service delivery, resources such as Social Security, employment and job
development, housing, and transportation; and other topics relevant to supporting youth with
developmental disabilities, by using a variety of formats and media.
Researches contemporary practices and techniques in the field of transition services and
creates/edits/maintains training manuals/literature on the topic.
Performs literature reviews and pursues professional development as needed. Maintains
knowledge of conditions, systems, and policies affecting the field of disability.
Keeps current on best practices, new system programming, legislative activities and community
resources and activities pertinent to transition services and supported employment.
Develops project specific strategic plans and budgets. Prepares and presents written reports and
correspondence.
Coordinates with the Behavioral Health/Developmental Disabilities Coordinator, the Finance
Team and Public Health Director within the Department, as requested.

Supports the development of key indicators that contribute to the reduction of system and
structural barriers that perpetuate inequities for people who experience Intellectual/
Developmental Disabilities.
Contributes to the intentionality of imbedding diversity, equity, inclusion, accessibility, social
and environmental justice within the efforts of the County Developmental Disabilities Program.
Participates in performance measures and quality improvement through the development of data
tracking indicators and monitoring processes for the Job Foundations Program, School to Work,
DVR/County Employment Programs and other services connected to High School Transition
Students who experience Intellectual/Developmental Disabilities.
Provides data collection and reporting by working with DVR to show progress relative to
School -to -Work efforts, including but not limited to; sharing data regarding Students' progress in
the School -to -Work program, Students' progress towards employment, and reasons why.
Develops and establishes methods to increase and ensure accessibility to programs and
information including, but not limited to (a) Translation of transition documents into various
languages; (b) Engaging interpreters; and, (c) Setting up appropriate communication methods to
fit the needs of students (i.e., in -person, over -the -phone, and/or video remote interpreter
services).
Develops, monitors and updates website to include School -to -Work information. This content
must be accessible to all, including, but not limited to, language translation, and JAWS -
compatible.
Promotes the practice of respect for people with openness, trust, safety, transparency,
collaborative problem solving, in the pursuit of incremental continuous improvement and
assurance.
Other duties may be assigned at the discretion of the Developmental Disabilities Behavioral
Health Coordinator or Public Health Director.

MINIMUM QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Bachelor's Degree involving major study in Public Health, Human Services, Community
Development or closely related field,

And, A minimum of two years'experience working in health services, social services,
education, policy development, community engagement and mobilization;
OR, Six- years' experience working with system partners, non-profit agencies, service
providers and community groups in a social service field.
OR, An equivalent combination of education and experience may be substituted which
provides the skills, knowledge and abilities to perform the essential functions of the
position.
LANGUAGE SKILLS
Effective communication skills, both orally and in writing: Ability to read, analyze, and
interpret common scientific and technical journals, general business periodicals,
professional journals, technical procedures, or governmental regulations. Ability to write
reports, business correspondence, County plans and procedure manuals. Ability to
effectively present information and respond to questions from community groups, school
districts, agencies, service providers and the general public. Ability to make effective
presentations.
MATHEMATICAL SKILLS
Ability to work with mathematical concepts. Ability to apply concepts such as fractions,
percentages, ratios, and proportions to a practical situation.

Strong analytical skills with exceptional attention to detail. Strong critical thinking skills.

Experience working with diverse populations and/or with individuals who experience
intellectual/developmental disabilities and other disabilities.
Ability to maintain confidentiality related to a student's/family's situation and any
information related to their developmental disability.
Ability to understand how behavior, attitudes and values relate to the impacts of
disability, socioeconomic status, race, ethnicity, mental or physical ability, religion, or
gender.
Ability to understand and apply pertinent RCW's and WAC's, HIPAA policies,
procedures and Public Health core functions.
CERTIFICATES, LICENSES, REGISTRATIONS
Must have a valid Washington State Driver's License.

PHYSICAL DEMANDS/WORK ENVIRONMENT
The work environment characteristics described here are representative of those an employee

Travel to attend meetings and other events throughout Jefferson County, the State and outside
