The meeting was called to order at the appointed time by
Chairman B. G. Brown. The following Board and staff members were
present: Commissioner Larry W. Dennison and Commissioner George
C. Brown; Kathy Stafford, Irene Senior-pomeroy, and Karen Chang.
Dr. Addison, and Maureen Linehan, Program Manager/Planner, Olympic
Area Agency on Aging were also present.
APPROVAL OF MINUTES: commissioner George Brown moved to
approve the Minutes of september 19, 1989 as written. Chairman B.
G. Brown seconded the motion, due to the temporary absence of
Commissioner Dennison. The motion carried by a unanimous vote.
ADMINISTRATOR'S REPORT: Administrator Kathy Stafford
reported that the City will pay the Health Department for their
1989 services when they have a copy of the contract. There has not
been a contract for these services in the past, Kathy Stafford
added, but the City Treasurer, Dave Grove, thought he had a copy
of one for 1989. He will send a copy of ~be contract to the Health
Department when he finds it.

Chimacum & Port Townsend School District Contracts: Both the
Chimacum and Port Townsend School District contracts have been
signed by their respective School Administrators, Kathy Stafford
reported. The contracts are the same as the 1988-89 contract
except for an increase in fee of $.50 per child. The school
districts have agreed to this fee increase. These contract will
be considered by the Board of commissioners at their next meeting.
cape Georae Area Residents Meeting with Dave Lennina of the State
Deoartment of Social and Health Services: Myron Offstein has
asked, Kathy Stafford noted, to be taken off today's agenda because
he understands that the Health Department is in contact with Mr.
Lenning and a meeting will be arranged sometime in the future.
NURSES REPORT: Nursing Director, Irene Senior-Pomeroy,
reported that the Nursing staff will participate in a retreat with
Health Officer, Dr. Geerlofs, in the near future to work on issues
such as program clarifications.
Senior citizens Health Screenina Prooosal: Irene Senior-Pomeroy
presented a proposal for a Senior citizens health screening
program. This program could be accommodated in the Health
Department space as well as at off site locations. Kathy Stafford
asked the Board for approval to proceed with this program. A
letter of support was submitted from the Olympic Area Agency on
Aging and Kathy Stafford reported that Dr. Geerlofs has reviewed
and approves of this program.
commissioner B.G. Brown asked how the medical community feels about
this type of program being taken on by the Health Department?
Kathy Stafford reported that the Hospital doesn't want to provide
these services. Dr. Geerlofs will make a presentation to the area
doctors and ask for their input before the program is implemented.
Commissioner Dennison moved to approve the program concept and
direct the Health Department staff to proceed with planning the
program. Commission George Brown seconded the motion.
Dr. Addison stated that he is a retired physician who practiced
in Iowa for 33 years and then moved to California. He moved here
six months ago. Dr. Addison stated that he has done family
practice for the past two or three years in California, and he is
well aware of the screening and tests that would be used in this
type of program. The tests would be used to refer the Senior
patients to a doctor for treatment, if necessary. One to four
exams could be done in about 3 hours per week (12 hours per month).
Mal Practice insurance must be provided and there are special rates
for part-time physicians as well as for physicians who are over 65.
The cost of the insurance would be approximately $935.00 per year.
The hours required for the program will be dictated by demand.

Visits will be made to other communities such as Chimacum and
Brinnon on a set schedule. A contract for services must be written
for his services and a copy sent to the insurance carrier.
commissioner George Brown asked what per hour charge Dr. Addison
would consider? He reported that he would leave this up to the
Board, but $40 per hour was suggested. Kathy Stafford reported
that the proposal originally included the services of a physician
on a volunteer basis, with the Department paying for the mal
practice insurance and reimbursing the physician for mileage to
other sites.
Maureen Linehan stated that the Olympic Area Agency on Aging
supports the program, but does not have any funding to provide for
it. Commissioner Dennison stated that the Board has agreed to the
program in principle and the details will have to be worked out.
Chairman B. G. Brown called for a vote on the motion. The motion
passed by a unanimous vote.

Alternative On-site Sewage Disposal System: Sa.ple's PrODertv.
Harrowstone Island: Robert Leach, Manager of Jefferson County PUD
#1 explained that the A1RR Wastewater System installed on the
Sample's property on Marrowstone Island several years ago was not
approved by the State, at the time of installation, because of the
treatment medium used in the system. After changes were made to
the system by the manufacturer, it was approved by the State as a
sand filter alternative system.
Commissioner Dennison asked what difference there is in this new
agreement for provision of municipal oversight for an on-site
septic system and the previous agreement? Bob Leach stated that
this agreement is a standard agreement for alternative septic
systems and is less extensive than the previous agreement.
Commissioner Dennison moved to approve the change in Agreement for
the Samples Alternative Septic System as presented by the PUD.
Commissioner George Brown seconded the motion which carried by a
unanimous vote.
Resoonse from ADDlicant for Environmental Health
Director: The Health Department has extended the County's
employment offer to an applicant for the position of Environmental
Health Director, but the man says he can not do the job for less
than $38,500.00. The top of the salary Grade for this position is
$31,400.00. The Board will consider if they want to make a counter
offer to this applicant. The position will be re-advertised in the
Seattle Times and the Environmental Health journals.

ProDOsed Chanaes to the Environmpntal Health Fee
Schedule: Kathy Stafford presented a worksheet that indicated the
comparison of the Jefferson County fees for Health Department
environmental health services to what is charged in other counties,
and lists proposed new fees. The main changes are:
Food Service Program: Restaurants increased $20 to $35
depending on size. Addition of a fee for a satellite kitchen
and a fee for food caterers.
Retail Food markets are charged according to the number of
checkstands. There is an additional fee for re-inspections
and administrative hearings for the food service program.
Sewage Disposal Fees: Water samples went up $1.00 because the
laboratory fee for processing the samples increased. A
separate price for alternative system permits as been added.
The fee for evaluation of an existing system has been
increased to $60.00. A fee for short and long plat review has
been added. Currently there is no charge for short plat
review. The suggestion is to charge a $50 fee which would be
applied to the cost of the actual permits. This will be
discussed at the next staff strategy meeting so that Planning
Director David Goldsmith can give his input on this item.
Dr. Geerlofs reviewed these proposed fees and recommends that they
be approved as soon as possible.

Dear Mr. & Mrs. Samples,
Robert A. Leach, District Manager, authorized by
Resolution No. 88-272 adopted by the Board of commissioners of
Public Utility District No. 1 of Jefferson County on AprilS,
1988, has approved and signed the Agreement For provision of
Municipal Oversight For An On-Site Septic System to provide
monitoring services to your On-site Septic System. A copy is
enclosed for your records.
One fact we want to make you aware of is that this
entails an encumbrance that goes with the land; should you sell
this property the buyer must be made aware that he is also
assuming this encumbrance.
We also must be notified and provided with buyer's name
and contact address_
According to the site monitoring schedule (Appendix II)
there will be regularly scheduled site monitoring visits
conducted by PUD personnel, one at six months and one at twelve
months after the date of final approval by the Jefferson County
Health Department; monitoring visits will be conducted according
to Appendix II of your contract thereafter. You will receive a
copy of each monitoring visit report. Billing will be processed
at time of visit.

AGREEMENT FOR PROVISION OF MUNIeIPAe OVERSIGHT ~~~
FOR AN ON-SITE SEPTIC SYSTEM ~~
THIS AGREEMENT is made this ~~day of rCJ~ ~, 19~qbY and ~
between CHARLES R. & BETTY L. SAMPLES, hereinafter referred to as
the "Owner", and Public Utility District Number One of Jefferson
County, a political subdivision of the State of Washington,
hereinafter referred to as the "PUD", to wit:
WHEREAS, CHARLES R. & BETTY L. SAMPLES is the owner of a certain
tract of land particularly described as follows:
PARCEL #021321021, KNOWN AS TAX 45 WHICH IS LOT 42 IN GRIFFITH
POINT ESTATES, SECTION 32, TOWNSHIP 30 NORTH, RANGE 1 EAST, W.M.,
JEFFERSON COUNTY, WASHINGTON.
and,
WHEREAS, a sewage collection, treatment and disposal system, as
described in the Operations and Maintenance Manual(s) and As-
Built Plans attached hereto as Appendix I, has been installed
thereon, and
WHEREAS, the Owner is in need of a governmental agency to assume
municipal oversight responsibility for the continuous and
satisfactory operation and maintenance of the sewage system, and
WHEREAS, the PUD is willing to assume the responsibility for
providing such municipal oversight, as authorized by Chapter 54
RCW, on the termS and conditions hereafter set forth.
NOW THEREFORE, in consideration of the premises and the mutual
promises and covenants hereafter set forth, the parties hereto
agree as follows:
1. Construction -- The Owner has constructed a sewage
collection, treatment and disposal system together with
appurtenant facilities, with capacity and design satisfactory
to the Jefferson County Health Department. The plans and
specifications for this construction have been approved by
the Jefferson County Health Department prior to construction
and have been inspected by same following construction.
2. Riqhtof Inspection -- The PUD shall have the right to
inspect the Owner's Sewage System in accordance with the
schedule set forth in Appendix II. The cost of such
inspections shall be borne solely by the owner. Said costs
include, but are not limited to, labor (including all fringe
benefits) and mileage.
3. Certification -- The PUD will assume no responsibility for
the Owner's Sewer System until the system is complete and the
Owner furnishes the PUD with a certification by the Jefferson
County Health Department that said system has been

constructed in accordance with the approved construction
plans and specifications therefore.
4. Easements -- The Owner shall furnish the PUD with
perpetual easements for all portions of the Owner's Sewer
System lying outside of dedicated pUblic rights of way.
5. Conveyance -- The Owner shall retain ownership of all
facilities and appurtenances that together or individually
constitute the Sewer System. The PUD shall provide municipal
oversight services only.
6. Reversion -- No reversion shall be necessary as ownership
shall remain with the Owner.
7. Additions -- No additional service areas outside the
property described in the recitals hereto shall be connected
during the life of this agreement to the Owner's Sewer System
without the express written approval of the PUD and the
Jefferson County Health Department.
8. Ooeration and Maintenance -- The PUD shall provide the
personnel, vehicles, additional equipment other than provided
for above, and materials necessary to provide municipal
oversight for the Owner's Sewer System and shall keep an
accurate record of the costs thereof. Such records shall be
open to the inspection of the Owner.
9. Fees -- The PUD shall be entitled to compensation for its
services herein in accordance with the following schedule:
Thirty dollars ($30.00) per each monitoring visit,
to be billed at time of visit.
Such schedule of charges shall remain in force and effect
until such time as a revised schedule of charges is approved
by resolution of the Board of PUD Commissioners after public
notice of the intention to revise charges and public hearing
thereon. Effective date of revised charges shall be as
provided by such resolution. Collection of the fees in
accordance with the above schedule shall be the
responsibility of the PUD.
10. Term -- This agreement shall remain in full force and
effect until the date of notification to the PUD by the
Jefferson County Health Department that the Owner's Sewer
System has been intercepted and connected to a general sewer
system.
11. Tax -- The Owner shall be responsible for paying all
real, personal and other taxes as they come due and are owing
on the Owner's Sewer System or real property. In the event
that any taxes are not paid when they come due and are owing,
the PUD shall have the right to immediately terminate any of
its obligations under this agreement to provide municipal
oversight services for the Owner's Sewer System upon thirty
(30) days notice to the Owner, during which time the Owner
may cure said breach, and pursue any other legal remedies
available for the Owner's breach of this provision.
12. Venue -- Venue for any action hereunder shall be in
Jefferson County, Washington. If any term or provision of
this agreement is in whole or part held invalid or
unenforceable by any court of competent jurisdiction, the
remainder of this agreement shall continue in full force and
effect.
13. Liability. The Owner agrees that the PUD shall have no
liability to the Owner on any matter related to the sewage
collection, treatment and disposal system and appurtenant
facilities; approval of plans and specifications for
construction of the same; certification that the Owner's
sewer system shall meet standards established by the
Jefferson County Health Department or State of Washington; or
from any termination or restriction of such sewage
collection, treatment and disposal system or appurtenance
thereto or repair, replacement of maintenance as required by
the PUD in connection with its municipal oversight
responsibilities under the terms of this agreement.
The Owner shall hold the PUD harmless for all costs and
expenses, including attorney fees, incurred by the PUD as a
result of any claim for loss, damage or otherwise made by any
person or entity as a result of improper design or
construction; failure to obtain or maintain certification
from the Jefferson County Health Department or State of
Washington; or resulting from any termination or restriction
of said sewage' collection, treatment and disposal system or
appurtenance thereto, or repair replacement or maintenance of
the same as required by the PUD in connection with its
municipal oversight responsibilities under the terms of this
agreement.
14. Life of Covenants and Bindinq Effect -- All of the
promises, covenants, terms and conditions of this agreement
hereunder, and this agreement

Inspection Schedule
RECIRCULATING SAND FILTER SYSTEM
with a
PRESSURIZED DRAINFIELD
The PUD will monitor the performance of the system
described in Appendix I on the following schedule:
X six months from the date of final system approval by JCHD
X one year from the date of final system approval by JCHD
X three years from the date of final system approval by JCHD
X five years from the date of final system approval by JCHD
X TEN years from the date of final system approval by JCHD
X TEN year intervals thereafter
X additional monitoring as required in the event of problems
NOTE: Owner must pump tank out according to requirements
of Jefferson County Health Department.
Following each site visit, the PUD's system operator will
prepare a report which will include the following:
(1) Nuisance factors noted (such as odors or user
complaints, if any); (2) Any mechanical malfunctions noted; (3)
Any electrical malfunctions noted; (4) Any evidence or notice of
material fatigue or failure; (5) Any evidence or notice of
neglect or improper use of the system; (6) Any installation
problems, such as compaction of displacement of soil; (7) State
of maintenance of the septic tank; (8) State of maintenance and
condition of the pump chamber; (9) Any evidence of ponding,
evidence of clogging, or reduced infiltration rates; (10) Any
evidence of malfunctions of the siphons or pump(s), including
dosing volume problems, pressurization problems, breakdown,
burnout or cycling problems; and, (11) Any evidence of overflow
and/or seepage problems.
One copy of the report will be placed in the PUD's files,
one will be sent to the Jefferson County Health Department and
one will be provided to the owner.
