G. Brown with Commissioner Larry W. Dennison and Commissioner George C. Brown both in attendance as well as Mary Catlin, Administrator/Director of Nursing and Rick Miklich, Director of Environmental Health. Health Officer Peter Geerlofs was not present. APPROVAL OF MINUTES: The Minutes of the October 20, 1987 meeting were approved as read by motion of Commissioner Larry Dennison, seconded by Commissioner George Brown. ADMINISTRATOR'S REPORT: items for the Board: Mary Catlin reviewed the following Model standards program: Mary Catlin asked if the Board would like the Department's annual report presented in the "model standards" form this year? In this format information on the morbidity and mortality of Jefferson County would be presented as well as how the County compares to the rest of Washington State and the Health Board Meeting Minutes: November 17, 1987 Page: 2 country. services that the Ms. Catlin advised that the summary information on by program would still be provided. The Board concurred annual report be submitted in this format. Contracts: Mary Catlin asked if a written policy could be developed which states that all contracts that the Health Department enters into are written with Jefferson County as contractor (not the Health Department). After discussion of the needs of the department and the legalities involved, the Board directed Mary Catlin to develop a written policy for their consideration. The discussion continued regarding the contract with the taverns where the Health Department is placing condom vending machines. Mary Catlin suggested that the contract state the Department receive all the proceeds from sales until the price of the vending machine and the initial supplies has been reimbursed. The machine would then become the property of the tavern and be their responsibility to maintain. Chairman Brown expressed concern with regard to assuring continued maintenance of the machines. He suggested of sharing maintenance costs be explored. The meeting was recessed and then reconvened at 3:00 p.m. so that the Board members could attend a memorial service for PUD Commissioner Charles Botts. Administrator's Job Description: Mary Catlin submitted a revised draft of the Administrator's job description, She reported that all of the changes that were discussed on this description have been incorporated into this revised draft. Action on this matter was deferred until the Health Board Retreat scheduled for November 19, 1987. Computer Time and Accountinq Procedures: The State CHIMUS system used for program accounting is being discontinued on January 1, 1988. Kathy Stafford and Mary have been working on a replacement computer program for this accounting that will meet the State reporting requirements and better serve the needs of the department. Scan Telephone Lines: The State has sent a memo to all Health Departments stating that they will no longer pay for the SCAN telephone line to the department. This is being fought on the State level by several Health Departments. Mary will keep the Board advised of what happens with the SCAN billings. Health Board Meeting Minutes: November 17, 1987 Page: 3 Gene Seton and Duke Shold re: Individual Sewaqe Disposal System Permit; Black Bear Subdivision, Four Corners: Environmental Health Director Rick Miklich reported that he made a site inspection of a lot in the Black Bear Subdivision at the request of Mr. Seton and Mr. Shold. This subdivision was approved in 1983 but the Health Department records are confusing. The soil logs on the site in question are not good because they have an indication of high ground water at 17 and 18 inches. Mr. Miklich then read the letter he wrote to Mr. Seton which provided the following information: Soil log profiles: #1 hole - soil modeled at 17 inches, #2 hole - soil modeled at 17 inches. #3 hole - soil modeled at 18 inches. #4 hole - soil compacted at 17 inches with clay at 28 inches. The letter advised that because of the soil limitations he recommended the use of a curtain drain and mound disposal system. After reading the rest of the letter, Rich Miklich reported, that Mr. Seton has indicated to him that he is not willing, and sees no need to install a mound system. He would be willing to install a partial fill system, however. Gene Seton then explained that he received a letter from the Health Department dated June 13, 1983 which recommended that lots three and four of the subdivision be combined as one building site which would be five acres in size. (This was also stated in the June 7, 1983 letter). This is what was done and the subdivision was approved. Lots one and two of this subdivision have been sold and one of them has a septic system installed on it currently. Having to install a mound system on this lot is a hardship to any purchaser, Mr. Seton stated. Mr. Seton further stated that the approval of the short plat has put a hardship on him. If the short plat had not been approved in 1983 he would not have spent the money to short plat the property at all. Mr. Seton reiterated that he is willing put extra fill on the lot for a partial mound system. Rick Miklich advised that because of the indication of seasonal high ground water on this lot at 17 inches, the regulation 18 inches of separation can not be met. Even for a mound system 18 inches is recommended with the minimum allowable being 12 inches. If the separation is decreased from the 18 inches, some type of system must be installed to improve the quality of the effluent that goes into the ground (sand filter, etc.). The possib~lity of the installation of a curtain drain to divert the water was discussed. Rick Miklich stated that the installation of a curtain drain and then a winter evaluation of the site could possibly solve this problem. The discussion then turned to the subdivision regulations in 1983 and the use of site evaluations. The Board reported that there is a Health Board Meeting Minutes: November 17, 1987 Page: 4 disclaimer on every short plat that states that the short plat approval does not guarantee that the individual lots will be approved for a septic system. In response to a question from Mary Catlin, Rick Miklich reported that a winter evaluation requires that the evaluation is done over a period of time during the "rainy" season. This procedure assumes there will be a rainy season or it would have to be put off until there is one and it is a very time consuming process. Chairman B. G. Brown asked if the mottling seen in the soil gives an indication of how long during a year the water level was at that height or is it just an indication that the water was at that level at least one time? All it indicates, Rick Miklich responded, is that the water has been at that height at one time. There may have been changes in topography or drainage patterns in that area since that mottling occurred, Chairman Brown added, which should be taken into consideration. The discussion again turned to the subdivision approval received in 1983 and the site evaluations that were done before that approval was given. Commissioner Dennison reminded those present that the site evaluation, which is no longer done by the County, was used to give a property owner an idea if any area on a piece of property was suitable for a septic system, it was not an approval for a septic system on the site and a disclaimer was put on the form stating that. The need for a curtain drain on the site was discussed. Chairman B.G. Brown asked if the topography of the site would cause the ground water to move toward the planned septic system area? Rick Miklich reported that he does not remember the topography of this particular site, but he will make another inspection of it before a determination is made on the need for a curtain drain. Gene Seton stated that he will check the soil log holes that are open on the site now, over the "rainy season" to see if there is any water in them, and report to Mr. Miklich with the results. With that information, Rick Miklich can then make a determination regarding the need for a curtain drain on this site. DIRECTOR OF NURSING REPORT: Mary Catlin presented the written monthly report of nursing services provided by the department. Staffinq: Mary Catlin reported that part of the nursing staff positions in 1987 have been partially funded from funds allocated for a Nurse Practitioner. Now that a Nurse Practitioner has been hired that funding is not available. The following programs will not have funding for nursing services: new born program, communicable disease control, TB, and school education. Mary asked that she be advised as early as possible what the 1988 budget level will be so a determination can be made on what programs will have to be eliminated. Health Board Meeting Minutes: November 17, 1987 Page: 5 Commissioner Dennison stated that one of the problems that the Board has had in reviewing the health department budget, is identifying what programs are tied to grant funding. Flu and Pneumonia Clinics: Two more pneumonia and flu shot clinics have been scheduled: Port Townsend on Friday from 11:00 to 1:00 p.m. and one in Quilcene. The pneumonia is a once in a lifetime shot which protects against 22 kinds of pneumococcal pneumonia. Dental Rinse Proqram: The Department received State funding to do a dental rinse program in the schools. The program will be setup this year with the work to be done next year. Vision and Hearinq: Vision and hearing testing is currently being done in the schools. CONTRACT re: Professional Services: County School Districts; #50 Port Townsend, #46 Brinnon, #20 Queets/Clearwater and #49 Quilcene: Commissioner Larry Dennison moved to approve the contracts between the following school districts and the Health Department for professional nursing services as outlined: School District #20 Queets/Clearwater School District #46 Brinnon School District #49 Quilcene School District #50 Port Townsend Commissioner George Brown seconded the motion. CONTRACT re: Professional Services; Dental Hvqienist: Jess Brevik: Commissioner Dennison moved to approve the professional services contract with Jess Brevik to provide services as a Dental Hygienist for the Health Department. Commissioner George Brown seconded the motion. ENVIRONMENTAL HEALTH DIRECTOR'S REPORT: Environmental Health Director Rick Miklich submitted his written report of October's environmental health activities. He noted that the accounting on the number of hours per program is being reported in a more exact form. PUD Contract: The final draft of the PUD contract with the County for municipal oversight of alternative septic disposal systems was then reviewed for the Board. This contract is for the PUD to provide monitoring for all alternative septic systems approved in the County. The PUD will charge the homeowner a fee which they will negotiate for doing the required monitoring. Rick Miklich will be meeting with John Floyd, Manager of the PUD, to work out the final details on this contract. Health Board Meeting Minutes: November 17, 1987 Page: 6 Waiver Reguest; Mound Septic Disposal System Installation; East Quilcene Road; Bill Taylor: The department had Dave Lenning from the State Department of Social and Health Services, Environmental Health Section, inspect the site of Mr. Taylor's mound system, because of concern regarding the slope on the lot. Mr. Lenning has suggested that due to the slope of the lot being in excess of that allowed by the State guidelines and the cut for the driveway being located too close to the reserve drainfield area, the County should apply for a State waiver for this system and have the property owner re-route his driveway away from the reserve drainfield area as a condition of approval of the system. Rick Miklich advised that he does not feel the County needs to apply for a State waiver for this system for the following reasons: 1) If the primary system fails and the reserve area is needed the driveway can be moved at that time, and 2) there is approximately a 30% slope which means that the guidelines require 60 inches of soil. There is 33 inches of soil on this slope, which is why the mound system is required. Rick Miklich advised that the 33 inches with a mound system is acceptable. He recommended that the system be approved by the County with these two conditions, without referring this permit to the State. Commissioner Dennison moved to approve the variance and the mound system permit with the conditions as recommended by the Environmental Health Director. Commissioner George Brown seconded the motion. Household Hazardous Waste Day: The Health Department will be the lead agency for the Household Hazardous Waste Day on December 12, 1987, Rick Miklich reported. This project is estimated to cost between $7,000.00 and $8,000.00 ($40.00 per cars). Chem Pro Company will be here that day to containerize and ship these hazardous wastes for disposal. The Health Department has funding set aside in their budget to pay for this project, Mary Catlin, Administrator reported. This agreement becomes effective on the first day of September, 1987 and continues until the last day of August, 1988. WITNESSETH Whereas I Brinnon School District #46 is in need of professiona': nursing services to meet state regulations regarding the health of students and to promote the health of students and, Whereas, Jefferson County Heal th Department referred to as the Contractor is prepared to provide professional nursing and services, Whereas, Jefferson County Health Department is willing to assume the responsibilities as set forth within this agreement: It is agreed between both parties and named herein as follows: I. SERVICES A. 1) The Health Department will coordinate mandated screening programs for vision, hearing (WAC 248-148) sco.d.osis and kyphosis (WAC 248-150). (Vision screening for kindergarten, 1, 3, 5, 7, 10; hearing screening for grades: kindergarten, 1, 3, 5, 7, 10. Scoliosis screening for grades 5 - 10.) 2) The nurse wi 11 train volunteers and/or designated staff to assist in the above screening projects.
