Peter Geerlofs, M.D. David Goldsmith, Adminis- trator CITY OF PORT TOWNSEND REPRESENTATIVE: Glenn Ison *********************************************************************** ******************************************************************* The meeting was called to order by Chairman B. G. Brown. Commissioner Larry W. Dennison as well as all other Board members were present. NURSING SERVICE REPORT: Public Health Nurse Charleta Handly reported that two resignations have been received from members of the nursing staff: 1) Beverly Rich (backup Nurse) and 2) Linda Baker (she handled the Well Child clinics). A State consultant for the Well Child program has volunteered to do these clinics for the next three weeks. There is currently only one full time and one part time Nurse on staff. These nursing positions are being advertised. Most of the school screening (vision and hearing) has been done so the staff will only have to provide the minimum service to the schools until these positions are filled. The following nursing service activities have occurred since last month: * Charleta met with the members of every school faculty, but one. This has aided the service that is being provided to the schools because now the teachers and administrators know who they are dealing with. The schools are responding to a survey asking what they see as health problems and what priorities they would put on addressing them. The survey results will be tabulated and the Health Board Minutes: October 21, 1986 Page: 2 * information used to set priorities for extra services to be provided by the Health Department, as time permits. Dental Health Program: A special grant has been applied for to fund a dental health program. A dental hygienist has volunteered to provide some training for 2 to 4 hours per week in the schools regarding dental hygiene. Flu Clinic: 575 flu shots were given during the six scheduled clinics. A $5.00 donation was requested and the average received was $4.71. 14% of the risk population received vaccinations. In addition 17 shots were given in the Health Department with 26 being given in the schools. A possible measles case was reported in one school and eleven students were excluded from school because they didn't have their immunizations. One case of Hemophilus Influenza Meningitis was diagnosed in Port Townsend. Letters were sent to 17 pre schools and day care centers advising them of this one case and that the Health Department has vaccine available. Pneumococcal vaccine has arrived and will cost $10.00 per dose. * * * * Environmental Health Report: Environmental Health Specialist Randy Durant reported the following activities during the last month: * 26 Food Program inspections were done during the month. Temporary food service permits and inspections were up with the Quilcene Fair and the Kinetic Sculpture Race. DSHS is currently conducting a Food Program audit. Jeanne Chao will be leaving at the end of the month. The Port Townsend Paper Company Sludge Application project concluded during the month. It was successful and it will be a number of years before the sludge must be pumped out of the pond again. The Pope and Talbot Sludge Application Project is now being reviewed. Olympic Testing Lab update: Randy Durant will be meeting with representatives of DOE on site, this week, to lock the building. The DOE needs to be urged to contact the owner to clean up the site and dispose of the hazardous materials properly. The next step would be on-going monitoring of the drainfield, well and the headwaters of Donovan Creek. Randy Durant has been working with the Port Townsend School District on safety in the schools shop facilities. 4 to 6 animal bites were investigated by the Health Department. A food service establishment operating without a permit was closed. Mound System Guidelines have been revised and will be available by December 1, 1986. The major change is a decrease to 18 inches of soil required for a mound system installation (from 24 inches). This can be reduced to 12 inches provided there is a sand filter system planned in conjunction with a mound. Environmental Health Section fees during the first 9 months of 1986 have totaled approximately $50,000.00. * * * * * * * * Health Board Minutes: October 21, 1986 Page: 3 ADMINISTRATOR'S REPORT: Administrator David Goldsmith reported that three of the four County School Districts have accepted and approved the Health Department's contract for services during the 1986-87 school year. After reviewing the services to be provided, Commissioner Dennison moved to approve the contracts with Port Townsend School District #50, Chimacum School District #49, and Brinnon School District #46. Chairman Brown seconded the motion. Michele IsseI, previous Director of Nursing, has agreed to contract with the Health Department to provide help during the period that the County is without a Nursing Director. She will provide phone counseling and will meet with the staff one day per week. The contract is for $150.00 per week which includes travel expenses, and will terminate on November 28, 1986. Commissioner Dennison moved to approve the Personal Services contract with Michele IsseI. Chairman Brown seconded the motion. The shortages in personnel have necessitated cutbacks in the clinics offered in Quilcene and Brinnon, but bus fare is being offered to anyone in that area who wishes to come into the Health Department for the services normally provided in these clinics. A part time Environmental Health Specialist position has been requested in the preliminary 1987 budget. There are a couple of people who may be interested in working on a part time basis for the County. Randy Durant reported that budget cuts in Clallam County may effect the services provided to the West End of Jefferson County for environmental and nursing, that the Health Department contracts with their staff to provide. Peter Simpson, Director, Clallam-Jefferson Community Action Council submitted a letter regarding the raise in flu vaccination fees. His concern is that the new fees may preclude some low income senior citizens from obtaining these shots. The Health Department policy is such that no one would be denied service, whether they could afford the fee or not, David Goldsmith stated and reported that Dr. Geerlofs was going to advise Peter Simpson of this policy. Low Income Access Task Force, North OlvmDic Health Plannina Council: The Health Board is represented on the Low Income Access Task Force by Dr. Geerlofs and Glen Ison, Mr. Ison reported. This program is an attempt to systematize access to health services by people who may otherwise fall through the cracks. The next meeting is October 30, 1986 at 2:00 p.m. in the Hospital Day Room. Tom Beavers, ERA Real Estate, et al reo Discussion of Environmental Health Issues: Tom Beavers, Broker for ERA Real Estate as well as Shirley Rudolph and Bobbie Mustatia, came before the Health Board to discuss concerns regarding the environmental health services provided by the County. Health Board Minutes: October 21, 1986 Page: 4 Mr. Beavers reported that out of nine on-site septic permit and site evaluations applications submitted over the last month by his office, everyone was denied. In his opinion the health department staff has not offered any help or suggestions regarding what can be done in this type of situation with regard to making the property usable. Six years ago, Mr. Beavers added, he came before the Board with a petition with over 3,000 signatures seeking a solution to the same problem, and nothing has been done during those years. He asked what can be done to change this situation? Commissioner Dennison asked if a list of alternative systems was given to the applicant, or realtor, in these instances. Mr. Beavers stated that to his knowledge that was not done in these cases. Randy Durant advised that the alternative systems list is provided on request and where the Health Department feels, such a system would fit the needs of the site. There are soils in this County that are not suitable for on- site sewage systems or alternative systems and there is no answer to the dilemma this causes for the property owner. The use of alternative systems and the implementation of a Designer Program for on-site sewage disposal systems are steps the County is taking to help with this problem, but even these steps will not help in every instance. Mr. Beavers then submitted a list of properties that have had site evaluations performed since January with an indication that on-site sewage disposal will not work. He added that the State regulations say (Page 15) that the County Board of Health has the right to make their own restrictions on properties over five acres in size. Dr. Geerlofs added that what Mr. Beavers cited in the regulations is true and the County is looking into this issue. The bottom line issue of all the regulations is the protection of health. The State is relatively conservative in their guidelines, because it is hard to come up with the specific numbers that will assure that a system will work and be safe health wise for an indefinite period of time. The discussion continued regarding the current regulations of the State and the County; the alternative systems available and the regulations for their use; the need for the State to research and approve more alternative systems for use; the expenses associated with monitoring alternative systems; the appeal process; and the adversarial position of the Health Department with regard to issuing on-site sewage disposal permits. Randy Durant responded to Mr. Beavers' statement about the adversarial role of the Health Department by noting that when a site is found to be unacceptable for on-site sewage disposal, but an alternative system may be appropriate, it is the responsibility of the staff person to help the property owner in every way possible to find a way to use their property. Randy Durant asked that specifics be provided on sites that have been denied by the Health Department so they could review the files on them. No specifics on these sites were provided. The Health Department staff has to work within the rules and regulations set out for them by the State and the County and have to rely on what they find Health Board Minutes: October 21, 1986 Page: 5 on a site. The staff does not view their role as adversarial, but one of assistance. Dr. Geerlofs added that the Health Department Administration feel that Randy Durant is competent in soil analysis and interpretation of the regulations. Other issues discussed were: Creating a better image for the Health Department; the Health Department's role and the property owners perception of that role; the understaffing of the Health Department; the future implementation of a Designer Program and how that will affect the appeal process; and the subjectivity of the permit approval process. Mr. Beavers concluded by suggesting that the Health Department develop a packet of information regarding on-site sewage disposal regulations, the appeal process, and information on alternative systems. This could be given to each applicant and the cost of the work required to develop such a packet could be covered by increasing the permit fee. Chairman Brown thanked Mr. Beavers, Mrs. Rudolph and Mrs. Mustatia for taking their time to discuss these issue with the Health Board. MEETING ADJOURNED JEFFERSON COUNTY BOARD OF HEALTH d~,-;&) B:G. This agreement ecomes effective on the first day of September, 1986 and continues until the last day of August, 1987. WITNESSETH Whereas, School District # 48 is in need of professional nursing services to meet state regulations regarding the health of students and to promote the health of students and, Whereas, School District # 48 is in need of environmental health services to meet state regulations regarding food handling and safety of students and, Whereas, Jefferson County Health Department referred to as the Contractor is prepared to provide professional nursing and environmental services and, Whereas Jefferson County Health Department is willing to assume the responsibilities as set forth within this agreement: It is agreed between both parties and named herein as follows: I. The contractor shall perform the following duties related to assessing health as an extent ion of the child's primary health care provider: A. The following duties will be the primary responsibilities of designated nursing personnel and will be accomplished to the fullest extent possible. 1.) Review health history and developmental questionnaire (distributed by school) at enrollment. Assist teacher with appropriate health plan- ning and update as necessary when changes occur. Identify abnormal health findings and indicate these on student health record. 2.) Coordinate required screening programs for vision and hearing and scoliosis and kyphosis. Hearing screening for following grades: Kindergarten, 1, 2, 3, 6, and 9. Vision screening for following grades: Kindergarten, 3, 5, 7, 10. Scoliosis screening for grades 5 - 10. The nurse will train volunteers and/or designated staff to assist in the above screening projects. Included in screening procedure are: - rescreening students who failed initial screening. - referral of students found to have abnormalities for medical evaluation at student's expense. - records results of screening and follow up activities on students' health records. 3.) Collect and evaluate cumulative data to identify communicable disease outbreaks, safety/environmental hazards and school climate. - Monitor absentee records for communicable disease surveillance and control. - Monitor and evaluate patterns of reported health problems for timely reporting of contagious diseases to the health department. 4.) Nurse, or environmental health specialist, recommend changes in the school environment such as lighting, noise control, and sanitation needed to maintain an environment which is safe and conducive to learning. 5.) Participate as the health advocate on the child education evaluation team interpreting medical recommendations for the Individual Education Program (IEP) placement. B. The following duties will be secondary responsibilities and will be accomp- lished on a prioritized time-available basis by designated nursing personnel: 1.) Participate in planning and implementation of school health manage- ment protocols for the child with acute or chronic health problems, such as asthma, skin problems, injury, or handicap. 2.) Monitor the school health rooms to see that they are equipped with the appropriate supplies and equipment; that emergency Protocols are posted and understood by the person responsible for the health room. The nurse may train the health room staff. 3.) Initiate and make home visits when necessary to further assess the family's needs as related to the child's health. 4.) Serve as a resource person to the classroom teacher and staff for health education and participate as a member of the health curriculum development committee. 5.) Make recommendations to teachers and assist in implementing changes necessary to accommodate individual student disabilities which are health related. 6.) Counsels pupils, parents and school personnel in the area of general health and well-being, and make referrals for further intervention as deemed necessary. The contractor shall perform the following duties related to environmental concerns: A policy and procedure. aCl ltles according to Health B. Make at least one inspection of all school buildings and facilities to assess the safety and adequacy of buildings, personal hygiene facilities, ventilation, heating, sound control and lighting; and to evaluate specific safety practices in industrial arts and science facilities, and to make recommendations for improvement. ~ III. The Contractor will assure the professional preparation and competence of the nurse and environmental consultants. IV. The Agreement may be amended by written consent of both parties and all amendments shall be attached to this Agreement and made a part therein. V. The School District 1/ 48 shall compensate the contractor for agreed upon services at a rate of $4.50 per student based on enrollment as of October 1, 1986, payable by December 1, 1986. VI. The Agreement may be terminated by either party upon 60 days written notice. Make at least one inspection of all school buildings and facilities to assess the safety and adequacy of buildings, personal hygiene facilities, ventilation, heating, sound control and lighting; and to evaluate specific safety practices in industrial arts and science facilities, and to make recommendations for improvement. ~ - /' ,. III. The Contractor will assure the professional preparation and competence of the nurse and environmental consultants. IV. The Agreement may be amended by written consent of both parties and all amendments shall be attached to this Agreement and made a part therein. V. The School District # 46 shall compensate the contractor for agreed upon services at a rate of $4.50 per student based on enrollment as of October I, 1986, payable by December 1,1986. VI. The Agreement may be terminated by either party upon 60 days written notice. CONTRACT FOR SERVICES between MICHELE ISSEL and JEFFERSON COUNTY HEALTH DEPARTMENT This agreement is made and entered into between Michele Issei and the Jefferson County Health Department. This agreement is effective on October 3, . 1986 and shall terminate November 28, 1986 or two weeks following the hire of the nursing director, whichever comes first. WITNESSETH WHEREAS, the Jefferson County Health Department is operating without benefit of a director of nursing; and WHEREAS, Michele IsseI has extensive knowledge of the position of director of nursing for Jefferson County; and WHEREAS, Michele IsseI has specific knowledge related to public health nursing, programs and organization; THEREFORE, it is agreed Michele IsseI will act as a private nursing consultant to provide guidance in nursing program implementation and facilitate changes within the nursing section; I. Specifically, the consultant will provide the following services in coordination with the Health Officer, the Health Administrator and the Acting Director of Nursing services: I. Meet with nursing staff to provide professional nursing consultation regarding case management, identified clinic problems and other c1ient- centered problems. 2. Participation with the Health Officer and the Health Administrator in selecting, training and orienting the new director of nursing. 4. Participate with the Health Officer and the Health Administrator in planning the Board of Health Retreat and then attend the planned retreat. 5. Provide specific additional consultation services as may be requested by the Health Officer or the Health Administrator. II. Michele IsseI shall be present in the 'Health'Department for at least four hours per week, and be available for telephone consultation. III. Michele IsseI shall be compensated at a rate of $150.00 per week inclusive of expenses. IV. This agreement may be terminated at any time by mutual consent and agreement.
