Joseph Fischnaller, M.D. Gael Stuart, Administrator CITY OF PORT TOWNSEND REPRESENTATIVE: Glenn Ison Commissioner John L. Pitts called the meeting to order in the absence of Chairman B.G. Brown. Commissioner Larry W. Dennison was present as well as all of the staff members, and the City of Port Townsend representative. APPEDVAL OF THE MINUTES: The Minutes of October 16, 1985 were approved by motion ot Commissioner Dennison, seconded by Commissioner Pitts. Commissioner Pitts asked about the short course on On-Site Wastewater Treatment that Randy Durant attended in Seattle, regarding what is in the offing in the way of new alternative systems and what was discussed in regard to financing options. Randy Durant reported that the financing discussed was low interest loans for repair of systems and as far as new systems in the future there will be increased use of mound systems and sand filters. DIRECTOR OF NURSING REPORT: The Flu Clinics were concluded during the month of October, with 16 City employees receiving shots which was more than last year. ;, Smokeless tobacco information was distributed to area Dentists, atu:LHigh School teachers. * Smoking and Pregnancy information was given to all the physicians in the community with several of them asking for more material. .k The "Buckle Up" program continued in the Chimacum Creek Pre School and Wanda Cotton's Pre School. * Kah Tai, City of Port Townsend and Community Action Council employees will receive information on the Great American Smoke Out in their pay envelopes. Posters are also up in various places around the community on this project. Health Board Minutes, November 20, 1985 Page 2: * Staff attended the WIC (Women, Infants and Children) Conference in Portland. Major changes will be made in the program in terms of measu:reable objectives to document what the program is actually doing for poverty level families. * A consultant has been assigned to Jefferson County for the Maternal Child Health Program. A State audit of the MCH program is scheduled for January. During the time that Gretchen Gephart has been with the Health Department there has been over a 100% increase in the volume of people who have been seen, with MCH being the largest service area. The State may go to a "Unit of Service" payment for this program, which Gretchen stated may benefit the County to a greater degree than the current method of payment. * A second pilot study is being considered by the State with regard to Crippled Children Services, being handled at the local level with funds provided by the State. * There were 38 cases of Pediculosis and only 2 cases of Scabies. A large number of hemucults are still being done. * Screening for vision, hearing and scoliosis was done at the Schools. * A total number of 793 flu shots were given this flu season. * A newsletter at Grant Street School has been incorporating health and safety information provided by the Health Department. * Two "Buckle Up" signs were installed over the past month, one on the highway into town and one on the highway leaving town. The Leader has been asked to print a picture of these signs, but there has been no response to the request yet. Commissioner Pitts reported that the State Insurance Trust is looking into a differential rate on health insurance for smokers and non-smokers. The eastern State County commissioners asked for more information on Jefferson County's experiences with smokeless tobacco. Commissioner Pitts asked if the Health Department would put together their experiences with smokless tobacco so they could be sent to these County Health Departments. The New England Journal of Medicine has an article on smokeless tobacco that could also be copied and sent to these counties Gretchen Gephart reported. Commissioner Pitts also asked if the Health Department would contact the Schools about showing a film on pregnancy and smoking. Gretchen Gephart advised that any information that goes into the schools must be cleared by the School Superintendent. Gael Stuart will talk with Margaret Berry about the possibility of showing such a film in the Port Townsend Schools. HEALTH OFFICER'S REPORT: To update on the TB case in the West End of the County, Dr. Fischnaller reported that the State Welfare may pay for all of his hospital and medical expenses. A second person has shown symptoms and the Health Department is currently waiting for the results of the chest X-ray. Commissioner Pitts reported that he has been contacted by a group who is goingctocpropose new legislation on the use of illegal aliens for tree planting in the forests of Washington State. Dr. Fischnaller advised Health Board Minutes, November 20, 1985 Page 3: that any inquiries about these cases should be directed to the Clallam County Nursing Director. ENVIRONMENTAL HEALTH DIRECTOR'S REPORT: October was a busy month with John Eliasson serving his last day on October 11. There have not been too many delays since this reduction in the Environmental Health staff, because of the extra hours worked by Randy Durant. * There were 15 telephone complaints on items such as: On-site sewage problems, well protection, animals, garbage, food quality. Some of the "other" activities during the month included work on the Port Townsend Paper Company Sludge site, contacts with the Soil Con- servation Service regarding Quilcene Bay, preparing for interviews for the Environmental Health Specialists position, follow up on the Olympic Testing Labs in Quilcene and work on the City of Winslow sludge application site. Follow ups were done during the month on the following: two new mounds systems that were recently installed, and a couple of old abandoned dumps. Six final inspections of on-site sewage disposal systems were completed during the month. There were no short plat reviewed during the month and the septic tank permits and site evaluations were both less than for the same period last year. Randy Durant and Dr. Fischnaller toured Indian Island at the Navy's invitation. The Navy has contracted to have a Hazardous Waste Assess- ment done at the Bangor base and at Indian Island. The three areas on Indian Island that will be assessed are: 1) An old garbage dump on the northwest end of the island which is along the shoreline, 2) near a pier on the east side of the island where excess explosives were burnt at one time and 3) near a bunker where it is reported that a contractor buried some paint barrels. * * * * * * After the assessment report, which should be done by January 1986, a determination on proceeding with sampling will be made and a correction phase will follow. Randy Durant inspected the Port Townsend Paper Company sludge application site three times during the month. The application period ended October 15, 1985. ADMINISTRATOR'S REPORT: A new phone system from AT & T for the Health Department has been researched and will cost $509.00 for in- stallation as well as an hourly fee for the actual installation time. The monthly cost for this new push button "Merlin" system is only slightly greater than the current monthly cost. The Health Department budget will cover this if it is installed as soon as possible. Commissioner Pitts advised that the phone services for every County department will be analyzed in 1986. The Board concurred that the Health Department proceed with the installation of this system. Health Board Minutes, November 20, 1985 Page 4: Gael Stuart finished his reported by noting that the revenues are more and the expenditures are less than anticipated and this trend is expected to continue for the balance of the year. INTRODUCTION OF NEW STAFF: Randy Durant then introduced Jeanne Chao the new ~nv~ronmental Health Specialist who began work on Monday. Appeal: Lowell Larson re: Water System: Commissioner Pitts reported that Mr. Larson had contacted him and cancelled his appointment with the Health Board. Mr. Larson advised that he had been in contact with the State regarding his water system and asked if he could meet with the Board at a later date to relate his experiences with the State and the County. Letter of Resignation from Director of Nursing, Gretchen Gephart: Gael Stuart read the letter ot resignation submitted by Gretchen Gephart, Director of Nursing. Commissioner Pitts reported that the Board of County Commissioners have accepted Ms. Gephart's resignation. He then thanked Gretchen Gephart for all of the hard work and program develop- ment that she has done and added that all of the community input regarding her work has been very positive. Experimental On-site Devices for sewage disposal: Gael Stuart then read a letter trom Bill White and Dave Lenning ot the Department of Social and Health Services regarding experimental on-site devices, which advised that the State does support the application of experimental con- cepts or devices and that the authority and responsibility to permit and monitor these alternative systems lies with the local Health Department. Commissioner Pitts added that the Board has expressed the desire to pro- vide alternatives for the people within the community, but they also recog- nize staff limitations. Family Planning Grant Agreement: The Board concurred that the Chairman could sign the Agreement between the Health Department and the Clallam Jefferson Counties Family Planning Services as approved on October 16, 1985. Waiver Request; Norma Cronin, Lot 25 Squamish Harbor: Mrs. Cronin reported that she and Bob Reed were on this lot on November 13 and found that there were discrepancies in the rreasurEITIents m ere Heal th Depart- ment~ report as follows: * The structure is 135 feet (not 125 feet) at the northeast corner, from the high water level, and 140 feet from the south- east corner to the high water level. * It is 103 feet from the high water level to the first perc hole. * It is 67 feet (not 60 feet) from the well on Lot 24 (owned by a Mr. Hill). * There has never been any flooding on this property as was referred to by the Planning Department. * The soil description was not marked off on this report. There was 24 inches of soil before hardpan was found. Health Board Minutes, November 20, 1985 Page 5: * The place where the septic tank would be installed is toward the southeast portion of the property which has a natural slope to the northeast. * Mr. Hill (Lot 24) has given Mrs Cronin written permission to install her drainfield ninety feet from his well. Commissioner Pitts advised Mrs. Cronin that even though the neighboring property owner has no objection to this, the County has to abide by local and State regulations. The Planning Department will have to be consulted regarding the questions on the flooding. Mrs. Cronin responded that this property has been in her family for a number of years and she is retired now and intends to put a permanent home on the site if at all possible. Randy Durant then explained the differences in the distances as noted by Mrs. Cronin. The measurements for the distance of the drainfield from the well are approximate because the Hill's were not home to show Randy where the actual well head was located. The critical difference is the setback from the beach. Ordinary high water is defined in the statewide Health Regulations as, where a noted change occurs in the actual type of foliage growth. In discussion of any other options for placement of the home on this lot, Randy noted that the drainfield placement on this lot has to run North/South; add that to the setback requirements from the adjacent properties wells and there is not sufficient area from the beach to place the drainfield. There may be an area 40 feet wide by the width of the lot, contiguous to the property line on South Point Road where a drainfield could be placed, and is a possible area to explore. Commissioner Pitts noted that there may be other possibilities to explore if Mrs. Cronin is willing to change the position of the house and the septic system that may accommodate all of the setback requirements for the wells and the shoreline. Mrs. Cronin stated that if the house was setback to far it would not be in line with the view. The required drainfieW setbacks are as follows: 100 feet from the shoreline, 10 feet from the foundation and 100 feet from any well. Mr. Bob Reed who made the measurements, was then introduced by Mrs. Cronin. Randy Durant reiterated his suggestion that measurements be made from the well to the south to see what area would be available to the west toward South Point Road. A map drawn to scale, with stakes on the site where the measurements are taken from would allow Randy to review these measurements. Well logs for the two adjacent wells would also help if they are available. In response to Mrs. Cronin's question regarding a description of the soil, Randy noted that he had observed four soil logs and once hard pan was discovered in the hole the soils were not described any further. Once all of the information is received from the Planning Department on the flooding matter and the Health Department on the setback requirements, Commissioner Pitts advised that the Board could then make a determination on the variance request. There would be a re-evaluation fee on this permit. Health Board Minutes, November 20, 1985 Page 6: Quilcene Bay 205j Grant Proposal: Gael Stuart reported that the final budget has been submitted on this proposal and it has been approved. Environmental Health Position: The deadline for applications for the vacant Environmental Health position is Novmeber 27, 1985. In- terviews will be scheduled during the second week of December. WHEREAS, the Jefferson County Health Department is in need of a Department Health officer, and WHEREAS, at the present time, the Jefferson County Health Departmen~ hereafter referred to as the Department is unable to employ a full time Health Officer, and WHEREAS, Peter Geerlofs, M.D., hereafter referred to as the Contractor is a licensed physician and is prepared and willing to assume the responsibilities set forth within this agreement. IT IS AGREED BETWEEN BOTH PARTIES AS NAMED HEREIN AS FOLLOWS: 1. The Contractor shall act as Public Health Officer and perform the following duties required by that office. A. Enforce the public health statutes of the State, rules and regulations of the State Board of Health and the State Director of Health, and local health rules, regulations and ordinances within his jurisdiction; B. Take such action as is necessary to maintain health and sanitation supervision over the territory within his jurisdiction; C. Control and prevent the spread of any dangerous contagious or infectious diseases that may occur within his jurisdiction. D. Inform the public as to the causes, nature, and prevention of disease and disability and the preservation, promotion and improvement of health within his jurisdiction; E. Prevent, control or abate nuisances which are detrimental to the public health; F. Attend all conferences called by the State Director of Health or his authorized represen~ative; G. Collect such fees as are established by the State Board of Health or the local Board of Health for the issuance or renewal of licenses or permits or such other fees as may be authorized by law or by the rules and regulations of the State Board of Health. H. Take such measures as he deems necessary in order to promote the public health, to participate in the es- tablishment of health educational or training activities, and to authorize the attendance of employees of the \ocal Health Department or individuals engaged in community health programs related to or part of the programs of the local Health Department. 2. For said services rendered under this agreement, the Department shall pay the Contractor the sum of $500.00 per month, and mileage at the rate established for employees of Jefferson County. ~ Contract between Jefferson County and Peter Geerlofs, M.D. Page 2: 3. Payment shall be made on or before the twentieth (20th) day of the month following one full month of service, and it shall be understood that the Contractor shall be responsible for reporting the mileage travelled on Department business on the proper forms. 4. As an independent Contractor, it is understood that the Department is not responsible for payroll deductions such as Income Tax, Social Security, etc., and the Contractor must make arrangements, according to Internal Revenue Service directives, for payment of such tax as may apply. 5. The Contractor shall not discriminate against any person presenting himself/herself for service because of race, religion, color, sex, age or national origin. The Department shall take affirmative action to ensure that persons receive service without regard to race, religion, color, sex, age or national origin. 6. This agreement shall continue and be binding upon both parties herein unless terminated by giving sixty (60) days written notice by either party. 7. This agreement may be amended by written consent of both parties and all amendments shall be attached to this agreement and made a part thereof.
