Pitts, Member The meeting was called to order by Chairman B.G. Brown with all members present. Approval of the Minutes: The minutes of the January 18, 1984 meeting were approved on t':le motion of Commissioner O'!1eara, seconded by Commissioner Pitts. Unanimous. NURSING DIRECTOR REPORT: that Bud Gerber, Superintendent ot District, ':lad advised her that the at the school. Gretchen Gephart, D.O.N., reported the Queets-Clearwater School scabies problem is now under control During January, in-service education for pepole who administer medications at the area group homes was accomplished. Gretc':len also advised that the Shagella that is listed on ~he monthly report was reported by a private physician who is taking care of the treatment and the follow- up treatment. Waiver requests rei Jefferson. County Food Service Ordinance No. 2-77, Public Restrooms Section 4.1a: 1) Request trom San Juan Grocery: Gael Stuart read t':le variance request from Sue Dillingham, Owner of the San Juan Grocery. Upon the recommendation of the Health Department staff, Commissioner Pitts moved to approve the variance as requested by San ,Juan Grocery provided they meet all of the other Health Department' r~quirements for this type of establishment. Comm- issioner O'Meara seconded the motion, unanimous. 2) Time Oil Company request: The letter explaining the reasons for this requested variance to allow one restroom at the proposed Store and Service Station', to be located at the Corner of Ness Corner County Road and Irondale County Road in Hadlock, was read by Gael Stuart. Randy Durant reported that Time Oil has not submitted food service plans to date. Commissioner Pitts moved and Commissioner O'~1ea'::"a seconded ~he motion to approve the variance as requested by Time Oil, Inc. provided they meet all of the other Health Department and State requirements. Minutes January 18, 1984 -- Health Board Page 2 Health District formation report: Administrator, Gael Stuart, advised that in order to form a Health District a pooling fund with dedicated millage would have to be estalished. Port Townsend Mayor, Brent Shirley, was appointed as a member of the Health Board which will give him equal voting rights on the Health Board. Senior Health Screening: Commissioner Pitts reported that the Area Agency on Aging (03A) will be discussing the possiblity of any money being available from them for Senior Health Screening at their March 2, 1984 meeting. Contract employees eligibility for the 2.5% raise: Contracts for employees that are not tied to a specific contract amount have, in the past been adjusted in January to reflect any raise given to County employees, Gael Stuart advised. This adjustment for the 2.5% raise being given in January and July of 1984 will cost approximately $1,000 for all of the contracted employees affected. Commissioner O'Meara moved to approve the application of the 2.5% raise in January and July to contracts not tied to a specific amount in the Health Department. Commissioner Pitts seconded the motion. Unanimous. City of Port Townsend contract for services of Health Department: Gael Stuart advised that the contract with the City of Port Townsend for the Health Services in the amount of $25,000, to be paid quarterly, has been approved by the Prosecuting Attorney and the City's Attorney and will be presented to the City Council for their approval at their meeting on February 21, 1984. Commissioner Pitts moved to accept and sign the contract with the City of Port Townsend subject to approval by the City Council. Commissioner O'Meara seconded the motion. Unanimous. that the front of Septic ENVIRONMENTAL HEALTH DIRECTOR REPORT: Sewage Disposal Permit renewals are now the Activities Report and the income is Tank Permits on Page 2. Randy Durant reported being listed on the included with the January's Food program figures include 24 inspections for the DSHS audit and the food program inspections are almost completed for the first quarter of 1984. The site evaluations have increased from 9 to 23 for January while the sewage disposal permits decreased from 12 to 9. John Eliason will be starting to work with the sewage permit program which will help as there is more field activity in the spring and summer. Health Board/Minutes February 15, 1984 page 3: HEALTH OFFICER REPORT: Nothing special to report for January. ADMINISTRATOR REPORT: The actual revenue figures are not as high as budgeted because the money expected from the schools and the City has not come in yet, but the expenditures are on target. One problem that was not anticipated was the actual estimated cost for the change over of the telephone system. $1,500 was budgeted for this and the last estimate given for a system that would fit the needs of the department was $6,200. Gael Stuart recommended keeping the present system until further information can be obtained and/or a different plan for the change over can be devised. The next regular meeting of the Health Board will be March 21, 1984. MEETING ADJOURNED - CONTRACT FOR PROFESSIONAL SERVICES THIS CONTRACT is between the JEFFERSON COUNTY HEALTH DEPARTMENT (Department) and Myrtle Corey. The contract is to be effective as of January I, 1984. WHEREAS, the Department is in need of the services of a Foot Care Practitioner; and WHEREAS, at the present time the Department is unable to provide the services from personnel within its employment; and WHEREAS, Corey states that she is a qualified person to perform the duties and responsibilities set out in the body of this contract; Now, Therefore, IT IS AGREED: (1) Corey shall act as a Registered Nurse conducting Foot Care Clinics for Senior.Citizens and Handicapped Persons. Corey will give foot care including trimming nails, treatment of calluses, pad feet to alleviate pressure, check pedal pulses and perfusion, inspect general condition of feet, counsel and instruct, refer patients to appropriate medical personnel, confer with the Public Health Nurse and County Health Officer or Private Doctor regarding findings and recommendations related to persons with potential problems. (2) The services to be performed by Corey will be in accordance with the standards of the Jefferson County Health Department and will follow the outline of those services as set out by the Department. (3) The Department shall pay Corey the amount of Seven dollars and two cents ($7.02) per hour for services rendered and shall reimburse her for mileage driven in her personal vehicle at the rate established by Jefferson County. (4) Corey shall provide the Department a detailed accounting of the - ' - CONTRACT FOR PROFESSIONAL SERVICES (Corey) Page 2 work performed and the mileage driven at the conclusion of each month of this contract. The Department shall make payment for the work performed on or before the 20th day of the month following the month in which the work was " , performed. (5) Corey is employed by the Department to perform certain required services which cannot be provided by Department personnel. It is understood that Corey acts as'an independent contractor and that she will receive no fringe benefits from the Department or Jefferson County. It is further understood that neither the Department nor Jefferson County will withhold FICA taxes or other obligations for which Corey may be responsible. (6) The Department shall provide insurance coverage for the work which she performs under the terms of this contract. (7) Corey shall not discriminate against any person presenting themself for service based on race, religion, color, sex, age or national origin. The Department shall take affirmative action to insure that persons receive services without regard to race, religion, color, sex, age or national origin. (8) This contract shall continue until terminated by one or the other of the parties hereto. Termination shall require sixty (60) days written notice by the terminating party. This contract is to be effective January I, 1984. i WHEREAS, the Department is in need of the services of a Registered Physical Therapist, and WHEREAS, at the present time the Department is unable to provide these services from personnel within its employment, and WHEREAS, the Contractor is qualified to perform the duties and responsi- bilities set out in the body of this Contract, NOW THEREFORE IT IS AGREED: (1) The Contractor shall act as a Registered Physical Therapist providing services for children eligible under the requirements of the Chiid Development Program upon proper referral from the Clinical Nurse Specialist of the Depart- ment and the Public Health Officer who is a physician. The Contractor shall conduct assessments and followup where indicated for eligible children with special problems. Services will be coordinated with each child's total program which is parent implemental and will consult and advise other staff members. (2) The services to be performed by the Contractor will be in accordance with the standards of the profession and will follow the outline of those services required by the Child Development Program which is attached hereto and by reference incorporated into this contract as if fully set forth. (3) The Department shall pay the Contractor the amount of Fifteen dollars ($15.00) per hour for professional services rendered. Mileage costs will be reimbursed at the prevailing Jefferson County rate. (4) The Contractor shall provide the Department a detailed accounting of the work performed at the conclusion of each month of the contract. CONTRACT FOR PROFESSIONAL SERVICES AGREEMENT BETWEEN JEFFERSON COUNTY HEALTH DEPARTMENT AND THE CITY OF PORT TOWNSEND , This agreement is made and entered into between the Jefferson County Health Department (Department) and the City of Port Townsend (City). This agreement became effective January I, 1984 and will terminate December 31, 1984. WITNESSETH WHEREAS, the City is required to provide public health services to its citizens and, WHEREAS, at the present time, the City is unable to provide public health services from regular members of its staff and, WHEREAS, the Department is willing to assume the responsibility as set forth in this agreement: IT IS AGREED BETWEEN BOTH PARTIES AS NAMED HEREIN AS FOLLOWS: T. The Department shall perform the following specific duties: A. Enforce the public health statutes of the state rules and regulations of the State Board of Health and local health rules, regulations and ordinances within the City; B. Take such action as is necessary to maintain health and sanitation supervision within the City including: 1. Crippled Children's Services 2. Maternal Child Health 3. Control of communicable and infectious diseases 4. Inform the public as to the causes, nature, and prevention of disease and disability and the prevention, promotion and improve- ment of health within the city. 5. Prevent, control or abate nuisances which are detrimental to pu blic health. 6. Inspect food establishments and require observance of standards necessary to protect the public health. ..~ Contract for Professional Services Agreement-City Page 2 7. Enforce laws and regulations regarding disposal of liquid and solid waste and the protection of water supplies. 8. Collect such fees as are established by the State Board of Health or the Local Board of Health for the issuance,or renewal of licenses or permits or such other fees as may be authorized by law or by the rules and regulations of the State Board of Health. 9. To maintain all records including vital statistics according to the laws of the State of Washington. 10. To provide monthly reports to the appointed representative of the City concerning the activities of the Department including fiscal reports. C. For said services under this agreement the City shall pay the Depart- ment Twenty five thousand dollars ($25,000) for the period January 1, 1984 to December 31, 1984. Four equal quarterly payments of six thousand two hundred fifty dollars ($6,250.00) shall be made during the months of March, June, September and December. The final payment shall be made prior to December 31, 1984 in order that it may be included in the Department's 1984 revenue. D. It is understood that the Department is responsible for payroll deductions for Income Tax, Social Security, Washington State Retire- ment, and other employee benefits and that the City has no liability for payment of such taxes as may apply. E. The Department will pay the premium for all insurance necessary for the operation of the Department including professional liability insurance for the Health Officer, nurses and environmental health specialists. F. The Department shall not discriminate against any person presenting himself/herself for service because of race, religion~ color, sex, age or national origin. The Department shall take affirmative action to ensure the above. G.
