O'Meara was the only member Approval of Minutes: The minutes of the July 18, 1984 meeting were approved as iead by motion of Commissioner Pitts, seconded by Chair- man Brown. CONTRACT rei Early Intervention Program (Amendment) 1984-85; Mary Tudor: Commissioner Pitts moved to approve-the contract for the professional services of Mary Tudor, R.N. as recommended by Gael ,,~tuart, Administrator. Chairman Brown seconded the motion. Commissioner Pitts moved and Chairman Brown seconded the motion to approve and sign the contract for each school distric~ as well as a letter of explanation to each district. Preliminary Budget narrative explanation: The new computer printout forms tor submission ot thef-)reliminary DU:dget do not provide space for an explanation. Gael Stuart reported, and presented a four page 1985 Budget Explanation. He noted the request for several new positions and a change in the amount of rent anticipating the deDartment being given the space now used by Community Action. Randy has an appointment to meet wirh Minutes, August 15, 1984 Page 2: a representative of the Department of Social and Health Services to look at a preliminary location of the system on the ten acre parcel it may serve. Sand Filtration system for Cape George Village; Charles Laws: In response to a query from Commissioner Pitts, Randy-Uurant advisieQ':enat his last contact with tir. Laws regarding this project was in February. New soil logs were evaluated to determine if a sand filtration system could be utilized on this property. The soil was found to be unacceptable and ~he water table too high, Randy reported.. Commissioner Pitts advised that the Board will write a letter to Mr. Laws. ENVIRONMENTAL HEALTH DIRECTOR REPORT: Randy noted the following items in his repor~: * Contact in July were up from same time last year. * Septic tank permits were down while site evaluations were up over last year. * Paralytic Shellfish Toxin Surveillance Program- Killisut Harbor closed for Butter Clams, Discovery Bay closed for all shellfish and a portion of Oak Bay is closed for scallops. * One dogs head was submitted to Olympia for rabies testing. * The review of the final proposal for the City of Winslow's sludge disposal program is being conducted. * The new State Bulk Foods Regulations have been received. * The review of the proposal for a mound system at the Alcohol Plant is being completed. ADMINISTRATOR'S REPORT: The revenues are higher than is reflected on the tinancial report due to the way payments are received from some grants Gael Stuart advised. Expenditures are being held in line. MEETING ADJOURNED Next meeting will be September 19, 1984 < --., ',/?~ ~ //'-~~<!/ C;;L<~ /I C()~'~TkACT FOk PRO}"" :~~~ C.] O\i:..L SERr 1 CL THIS CONTRACT is between the JEFFERSON COUNTY HEALTH DEPARTMENT (Department) and Mary Tudor, R.N., M.N. The contract is to be effective as of July 1, 1984. WHEREAS, the Department is in need of the services of a Nurse Consultant, Child Health Development; WHEREAS, at the present time the Department is unable to provide the services from personnel within its employment; and WHEREAS, Tudor states that she is a oualified person to perform the duties and responsibilities set out in the body of this contract; Now, Therefore, IT IS AGREED: (l) Tudor shall act as a Nurse Consultant in Child Health Development conducting consultation with parents of children who have special health and de- velopmental needs and with the staff of the department. Tudor will conduct staff workshops, provide assistance to staff in case finding, follow-up~ intervention, consultation and teaching. Tudor will also keep necessary records and data and refer clients to appropriate medical personnel, confer with the Public Health Nurse and County Health Officer or Public Medical Doctor regarding findings and recommendations related to patients with potential problems. (2) The services to be performed by Tudor will be in accordance with the standards of the professions and will follow the outline of those services as set out in the M.C.H. Contract with the State of Washington, which is attached hereto and by this reference incorporated into this contract as if fully set forth. (~) Tudor shall provide the Department a detailed accounting of the work performed and the mileage driven at the conclusion of each month of this contract. The Department shall make payment for the work performed on or before the 20th day of the month following the month in which the work was performed. (5) Tudor is employed by the Department to perform certain required services which cannot be provided by Department personnel. It is understood that Tudor acts as an independent contractor and that she will receive no fringe benefits from the Department or Jefferson County. It is further understood that neither the Department nor Jefferson County will withhold FICA taxes or other obligations for which Tudor may be responsible. (6) Tudor shall provide malpractice insurance coverage for the work which she performs under the terms of this contract in a minimum amount of $100,000.00 per injury. (7) Tudor shall not discriminate against any person presenting themself for service based on race. religion. color, sex, age or national origin. The Department shall take affirmative action to insure that persons receive services without regard to race, religion, color, sex, age or national origin. (8) This Contract shall continue until terminated by one or the other of the parties hereto. Termination shall require sixty (60) days written notice by the terminating party. ., .c " : .
