MINUTES: By mo~ion 9f Commissioner O'Meara, seconded by Commissioner Pitts and unanimously passed, the minutes for the meeting on January 19, 1983 were approved. APPEAL OF SEPTIC S STEM APPLICATION DECISION: C.A. McCONNACHIE: Attorney enn A ra am repr sente Mr. c onnac ~e. an y urant rev~ewed evidence regarding this matter, beginning in January, 1981 when application was made for a septic pe:tit in conjunction with plans to remove the existing house and rebuild on is one-acre lot on Mats Mats Bay. The application was de- nied due to hi h water table, lack of suitable soil, and inadequate area for a drain field, given the locations of the bay, a creek, and two wells. As a result of an appeal to the Health Officer in March, 1981, in which Mr. McConnachie indicated the existing system was failing at times, Mr. McConnachie was informed that, as long as the existing structure was being used, repairs to the existing septic system would be approved, but that a mound system would be the only type which could be used to replace the existing drainfield. He was also informed that remodeling or replacement of the existing home would be considered for approval following repair of the existing drainfield. A report from an engineering firm indicated that the existing system is not failing and that, with specified precautions, a conventional septic system would be adequate on this lot. Mr. McConnachie submitted another permit application in November, 1982 based on remodeling and improving the existing house. The Environmental Health Director and the Health Officer maintained their position of March, 1981, and the permit was not approved. (Documents: Attachment I) Commissioner Brown noted that any failure in the existing system could be due to the tank not being pumped and to disrepair of the drainfield, stating that, given the age of the system and the number of years it had operated acceptably, repair of the existing system could be feasible. Mr. Abraham added that the proposal is to reduce the house from three to two bedrooms and install water-saving fixtures, It was clarified that WAC 248-96-040 permits repair of an existing system, but stipulates that repairs be the best possible, Although the Health y Health Board, February 16, 1983 Page 2 Department staff feel a mound system would be the best replacement system, such a system could also fail. Commissioner O'Meara made a motion to permit repair of the existing system with a conventional septic system designed by an engineer and approved by Mr. Durant, Commissioner Pitts seconded the motion. Unanimous. APPEAL OF SITE EVALUATION: JOSEPH MURRELL: Ed Benn represented Mr. Murrell on this matter. Mr. Murrell wishes to sell his Bridgehaven lot, but feels he is essentially prevented from doing so because an August, 1982 site evaluation indicated site conditions are unacceptable for a septic system, This is in conflict with an October, 1978 site evaluation which condition- ally approved the site, The lot does not provide an acceptable area for a drainfield which is both 100 feet from a neighbor's well and 75 feet from the beach, (Documents: Attachment 2) There was discussion of the similar problems involving a number of lots at Bridgehaven, and of the Shorelines Permit application now in process in which affected lot-owners are seeking permission to use some fill and place bulkheads in order to prevent further erosion along the bank. In some cases, this may also increase lot size, and could possibly, although not necessarily, affect septic permits. Mr. Murrell's lot is one of those in- cluded in the Shorelines Permit application, The Board decided to take no action regarding Mr, Murrell's appeal until the Shorelines Permit application has been processed, Mr. Benn agreed that this was reasonable, APPEAL OF SERTIC SYSTEM APPLICATION DECISION: CHARLES LAWS: Mr, Laws was accompanied by John Ekstrom, septic system designer, during this discussion of his proposal to install a mound system on his lot at Cape George Village. In this case, conditional approval of the site had been given during site evaluations in 1975 and 1977, but the site was found to be unacceptable when a permit application was made in July, 1982. This is another area where soil conditions are generally poor and there have been a number of septic system failures as the area has developed, (Documents: Attachment 3) Points made regarding this particular lot included: l)The soil is slowly permeable, requiring a greater area and depth to accommodate a septic system than permeable soil. 2)Mr, Ekstrom has designed a mound system, with fill carefully taylored to this site, which he feels would be adequate for this lot and will meet State regulations. 3)Mr, Ekstrom feels there is a 100 percent reserve area available should replacement of the system become necessary, Mr. Durant does not feel there is an adequate reserve area, 4)The house Mr. Laws intends to build would have only one bedroom, with water-saving fixtures and a two-bedroom mound. 5)Mr. Laws is willing to add a covenant to the deed which would preclude enlargement of the house and would require water-saving fixtures. 6)Mr, Laws is willing to have the system closely monitored, and agrees to immediately undertake repair or replacement of the system should failure occur. 7)Installation of this system, with careful collection of data, could provide valuable information , , Health Board, February 16, 1983 Page 3 regarding a mound system on this small a lot (12,000 square feet) with slowly permeable soils, and would be pertinent to future sewage disposal in this Cape George area, Mr, Durant stated that the major factors in mound system failures have been construction in the area, lack of control of surface water intrusion, and slowly permeable soils, He further stated his belief that a replacement system could not successfully be installed in the same area, should this mound system fail, He pointed out that if the proposed system is installed, it could affect future decisions regarding other borderline or unacceptable lots in the area. Mr, Durant recommended that, if the Board chose to allow installation of this system, the design be submitted to the State Health Board representative for review and approval, Dr. Fischnaller said there is no way for Mr. Laws to hold the County harmless should the system be approved and fail, and neighbors who might be affected by any failure would probably have legal recourse against Mr, Laws and the County. Commissioner O'Meara made the motion to approve installation of a mound system on this lot contingent on approval of the State Health Department. Commissioner Pitts seconded the motion, Unanimous. Mr. Laws was instructed to submit a current system design to Mr, Durant for transmittal to the State, ENVIRONMENTAL HEALTH SPECIALIST CONTRACT: JOHN HAYES: Mr. Stuart reported that an ~nquiry to Clallam County regard~ng the possibility of Clallam County employing Mr. Hayes, then allowing him to work in Jefferson County three days a week by contract, so that Mr, Hayes would be eligible for insurance and other benefits through Clallam County resulted in their proposal that his salary scale be increased to that of Clallam County (Letter: Attachment 4). Clallam County has a higher scale: $13.67 per hour, as opposed to the $13,18 Jefferson County now pays. He stated that he felt it would not be fair to other contract employees of comparable status to pay Mr, Hayes more, and therefore recommended against a contract for services with Clallam County, Commissioner Pitts made a motion not to pursue a contract with Clallam County, but to present the contract at $13,18 per hour between Mr. Hayes and Jefferson County to Mr. Hayes. Commissioner Brown seconded the motion, Unanimous. WAIVER OF 100 FOOT CONTROL RADIUS: QUILCENE LITTLE LEAGUE PARK WELL: Mr. Durant reported that the State Health Services Division would consider approval of the 100 foot radius waiver. The well is 85 feet from one pro- perty line. However, they want the County to try to get a restrictive covenant on the 15 feet of the neighboring property which completes the 100 foot radius, first, The Board instructed Mr, Durant to contact the property owner and request a restrictive covenant. NURSING DIRECTOR REPORT: Mrs, Bee submitted the monthly report for January, 19~3 (Attachment ~) and the annual report for 1982 (Attachment 6). Mr. Stuart requested that Mrs. Bee be allowed to present her report at the , - , .' Health Board, February 16, 1983 Page 4 beginning of the meeting in the future so that she can be excused from the meeting during the environmental health matters, The Board also felt this would be advantageous, and will hear her presentation first when no one is waiting to meet with the Board. They also suggested that her written report would be adequate unless she wanted to report additional information, and that if they have questions regarding the written report, they will ask her to appear as early in the meeting as possible. Otherwise, she may be excused from the meeting entirely. ENVIRONMENTAL HEALTH DIRECTOR REPORT: the month ot February, 19~3 (Attachment for 1982 (Attachment 8), Mr. Durant reviewed the report for 7), and submitted the annual report HEALTH OFFICER REPORT: Dr. Fischnaller reported that he and Mrs. Bee have applied to attend a seminar regarding treatment of tuberculosis next month. This is. not a case of new construction, but of re~odeling an existing hume. In fact. the new plan \.-JQuld ~liminate one bedroom that new c":xists in the house. Mr. McConnachie is quite willing to improve the system, but feels that the plan insisted upon by the Health DeparL~e~t is not workable, and he has engineering opinion which confirEs that. We must again reaffirm the conclusions that were previously made in February and March 1981. The site is basically unacceptable for on-site sewage disposal. We have had continuing problems in the area with existing sewer systems "hich have failed and or fail seasonally. The system next door failed last year. Mr. McConnachie informed us th1 existing system ~as inadequate and we felt it did snrface "ith increased useage, (drainfield in blackberry patch). Basea on site conditions observed, I'm sure this is or would be the case if the home was remodeled. Therefore, as per Dr. Fischnaller's letter of March 18, 1981, a mound is the only repair option we can consider. McConnachie to review the sewage disposal system serving his residence at Mats Mats Bay and to recommend acceptable modifications to accommodate his remodeling plans for the existing structure, The proposed new construction consists of providing a new double garage/boat storage area located on the southerly side of the existing structure, leveling the foundation structural members of the house, and some interior/exterior re- modeling of the existing structure. The contemplated remodeling would result in the conversion of the existing 3 bedroom residence to 2 bedrooms. On August 15, 1982 I visited and reviewed the soils on the subject parcel: Lots 93 and 94 Plat of Olympus Beach according to the plat thereof recorded in Volume 3 of Plats Page 19 records of Jefferson County. -I The existing soils are comprised of the following two classifications: "S...antown Gravelly Sandy Loam, 0 to 8 Percent Slopes This nearly level to gently sloping soil is in lowland seep spots of terraces along the toeslopes of moderately steep, hilly glacial terraces. The depth to the cemented layer is 20 to 30 inches, The A horizon ranges from dark brown to very dark grayish brown in color. The B horizon ranges from brown to dark brown and dark yellowish brown. This soil is somewhat poorly drained. Permeability is moderately rapid above the cemented layer and very slow in the cemented layer. Roots penetrate to the cemented layer. This soil holds about 2 to 4 inches of water available for plants. Runoff is slow, and the hazard of water erosion is slight. A perched water table is at a depth of 1/2 to 1 foot during the rainy season. Annual precipitation ranges from 17 to 25" inches. " -;!.~- 121 5th Avenue North. 5uite 200. Edmonds. Although a complete percolation test was not completed during my site inspection, these soils types exhibit a typical percolation rate of approximately 10 min./in, to 15 min./in. in the A horizon and significantly slower perc rates at deeper depths. In most places slopes . . These rates suggest that the horizontal and vertical flow via percolation into the groundwater aquifer is slow enough to allow purification to meet health standards. Also, from visual inspection of the site topography, the equal potential lines of the underground aquifer do not flow toward the creek to the west but instead essentially follow the existing 7.5% slope toward the bay, approximately- 120 feet a,way. .~ ' _<_From my field investigation it is apparent that the existing drainfield system is ~J~;~-not failing, as was indicated in Dr. Fischnaller's letter to Mr. McConnachie dated -~ March 18, 1982. The system has apparently been in continuous satisfactory service for the past 30 years and shows no visible evidence of leaching into Mats 1'1ats Bay or sewage surfacing from the existing septic tank. ~Although this site does not meet all of the present standards for soil depth and depth to a high water table, we believe that these shortcomings can be mitigated by shallow installation and installation of interceptor/french drains with visqueen vapor barriers on the downhill side. Thus, given the existing conditions and the owner's plans, we feel that by com- pleting the items listed below an acceptable system can be provided for this site. These items, which must be strictly adhered to for installation of a modified conventional drainfield system, are as follows: 1. Construction of the drainfield and new 1,000 gallon septic tank shall be completed during the summer months to prevent undue compaction. 2. Interceptor lines wi.th visqueen/barriers must be installed to insure the lowerih~ or the existing water table at least l~ feet. 3. Select fill/topsoil to a depth of at least 1 foot shall be imported to cover the additional lineal footage added to the existiog system. 4. - Monitoring ports constructed of rigid 4" PVC per SOR 3034 to be constructed to periodically check system service ability and water quality. 5. Maximize use of low water consumption devices, including: a.' Low flush toilet. b. Shower head nozzel with flow restrictors. . . , " ~ . "'.: . _... .... Page 3 Mr. Randall M. We have considered the use of a mounding system for this site and conclude that the installation of such a system would serve no apparent purpose, especially since leaching at the toe of the fill could easily occur on this 7.5% slope. The mound systems work much better on more level topography. . . We request that you approve the concept of rehabilitating and expanding the existing conventional system based upon the foregoing considerations. This department would approve a repair as per WAC 248-96-040. Site conditions previously observed were: 1. High water table 2. Lack of suitable soil for on-site sewage disposal 3. lilck of adequate area due to lot size and close proximity to Mats Mats Bay, a creek on west property line, your proposed well site and the neighbor's existing well. These conditions do not meet the standard requirements of WAC 248-96 and any repai r may be subject to fail ure. Because of these conditions a mound system is the only system we can consider for replacement of the existing failing drainfield. These guidelines were provided to you on this date. Submit plans to r.lr. Bill"Liechty, DSHS and this office for review. Work to divert surface and sub-surface water away from the drainfield area should be done at this time. Since August of 1982 the owner has attempted to acquire an on-site sewage system permit. This attempt has included four meetings with Mr. Randall Durant, Environmental Health Director and two with Dr. J.E. Fisc hnaller, Health Officer, to satisfy the conditions of approval. ~r. John Ekstrom, o~-site sewaEe disposal designer in Kitsap County, has developed an~ desiEned plans for a fill system e~gineered to the soil con~itions of that lot. He also has met with ~r. Durant and Dr. ?ischnaller and at this point in time it a~~ears that acquirin~ a permit is impossible under any circum- stance. As the site evaluation has been and is presently a part of the permit application process, then it would seem consistant that an apDroved status on the first part would be carried on to the permit, provided that all required conditions are met or exceeded. There- fore. this is a request ~hat the conditionally approved site- evaluation (1-]1-77), Which falls within the 1976-82 permit ~equirements, be allowed as the basis of a valid approved permit because of the following: A. The topo~raphy, surface water and soil conditions have not changed since 1975. B. The soil classification of 1975 has been verified in 1982 and labeled as slowly per~eable ( during wet season). C. The conditions as recommended in the site-evaluation will be met or exceeded by a beeter curtain drain and a smaller house. D. Additional conditions will be included that meet the criteria for this soil classification, double tanks, fill system drain field (two bedroom house size) and low volume water faucets. E. In addition, the fill system would be subject to periodic i~s~ections and i~mediate repair if the drain field requires improvement. r 2 ( ~ ,. F. The fill system may be ~onitored and the data collected to be used by the local health deDartment for future reference to the Department of Health and Social Services. This/on-site fill system is designed for one particular lot which is consistant with the local health board's poLicy of individual evaluation of each site. Another logical approach might be the oonsideration of a variance for fill system site condi~ions. Specifically the 24 inch mini~um of original, undisturbed and unsaturated soil, as recom~ended in Guide-lines for Fill Systems (~ay, 1980) be modified as follows, As the system is designed, the top of the natural soil is to be plowed. leaving 12 to 14 inches of undisturbed natural soil. To this could be added ad1itional plowed natural soil from the house site. which would then be of suf:icient depth to meet the guide- line rece~mendation. ~r. John Ekstrom has already adjusted the fill material formula to compensate for this condition. It has been observed and docu~ented that fill O~ Dound systems that ;;cve bee~ correctly designed have built-in forITulas to co~pensate for excessive water infiltration. p~or quality native soil and high water ~sage. This ~i~ht account for the ve~y h~gh success rate of these sys terns. The fill svstem would be installed durin€ the dry season as recom~en~ed. Please note that percolation rates for Dounds should be measured at 18 to 24 inches minimum, Percolation boles on the site ueasured 'only 10 inches. Also, "slowly permeable" soils require greater area and depth to remove the effluent from the fill area than do permeable soils. As mounds are new and alternative Yith liInited use and follow-up in Washington State, any site considered for a mound in Jefferson County must have 2 feet of permeable soil for considera- tion. We might consider acreage size parcels with slowly permeable soils as per the guidelines. Also note a 30 foot setback between a mound and lower property lines and/or any cut is necessary. There is a cut area right at the back lower lot line that ranges from l~ feet to appro:ri1nately 5 feet. I Yish I could be of more assistance in this matter. What this area really needs is a sewer. Due to the limited lot size I am sorry I have no recommendation at this time. I MATERNAL CHILD HEALTH , The intent of Maternal Child Health clinical and oreventive services is to reduce infant mortality and the incidence of preventable diseases and handicapping conditions among children, to reduce the need for in- oatient and long-term care services, to increase the number of children (especially preschool children) appropriately immunized against disease, and the number of all children, especially low income children receiving health assessments and follow up diagnostic and treatment services, and otherwise promote the health of mothers and children. Jefferson County Health Department Nursing Division applied for and re- ceived a grant of $32,728.00 from DSHS to provide programs in t~ese areas. We received an additional $14,944.00 of Federal money for our Child Development program, and $2,000.00 to purchase an Impedance Audiometer, making a total grant of $49,672.00. Well Child Clinics, Crippled Children's services, W.I.C., Infant and Child Development program, and Newborn Services are activities covered under our grant proposal. The other income in this program is $3,163.24 from fees in Well Child Clinic, a grant of $3,575.83 from Developmental Disabilities for the Infant and Child Development program, adding up to total income for these programs for 1982 of $56,411.07. . In addition CCS paid directly to provider for examinations and treatment of handicapping conditions for 44 children from Jefferson County, A little over $20,000.00 was spent for these Jefferson County children by the State CCS services. The total is not complete yet as all bills were not received and paid by December 31. The applications, arrangements, counselling and follow-up is made through the nurses in the Health Department. ~. Well Child Clinics These clinics are held in PortTllwnsend, Quilcene and Chimacum. There were 847 infants and children seen in the clinics. The services offered in these clinics include: Medical History Blood and Urine Tests Oevelopmental Assessment Vision Screening Audiometry Nutritional Evaluation and Counselling Infant and Child Care Teaching and Counselling -1- ~ Twenty-four per-cent of the children seen had abnormal findings which warranted a referral for further care to a physician or other health care professional, or a revisit for follow-uo in the Health Department, The total number of referrals was 198. The number of conditions exceeds the number of referred children as some children were referred for more than one problem. In addition over.100 children were referred to immuni- zation clinics for continuation and/or completion of their immunization series. Criopled Children's Services (CCS) Forty-three applicants' (age birth to 21) were screened for diagnostic and therapeutic services for handicapoing conditions. Of these, 15 were new applications. The eligible children received both in- and out-patient care including medical, surgical, laboratory and other diagnostic services. A Public Health nurse is assigned 10% of her time in this program. . Funding in CCS at the state level was reduced in 1982, and restrictions were put on coverage for some conditions, principally otitis media and surgery for myringotomies. The majority of the referrals from Jefferson County were the children with these disorders, so we have attemp- ted to assist parents to find other funding sources for these problems. -2- Conditions evaluated and/or treated in 1982 are: / Communication disorders 24 Musculosketal 7 Heart defects 7 Cleft palate and lip 1 Cystic fibrosis 1 Other birth defects 3 . c, WIC (~omen, Infants and Children) This program is largely funded by the U,S. Department of Agriculture through the State and is known as the Nutritional Program for Women, Infants and Children (W.I.C.) Clients identified in WIC clinic as hav- Tng nutritional defects and low income receive correctional foods through a voucher system. Nutritional counselling is provided by a Qualified nutritionist and the Clinic nurse. Clients are evaluated physically by measurements of height, weight, and blood test. Their present food in- take is evaluated, along with physical findings, and any high risk factors present to determine eligibility. Repeat visits for recertification are every six months. Under grant contract we have 185 slots now. During the year of 1982 we had 200 slots available for people in Jefferson County. We were unable to find enough eligible clients to fill the openings. Our caseload averaged about 165 participants. Outreach and oublicity, including a feature newspaoer article was done to attempt to reach more eligible people. . Clients are seen and evaluated on self referral, referral from physicians, other agencies, and other Health Department programs, The total number of WIC clinic visits in 1982 was 438 clinic and 153 nutritionist visits for a total of 591 client visits. A Registered Nutritionist serves under contract for counselling and teaching WIC participants. This teaching is in addition to the counsell- ing and teaching by the Clinic nurse at the time of certification. This program benefits the client by providing nutritional food (i.e. eggs, milk, juice, cereals, cheese, dried beans, baby formula) by individually prescribed vouchers each month. Physical orogress is monitered, and the client or Dar~nt receives good nutritional and health teaching. d. Infant and Child Development Program .This program is funded by a Federal grant for Washington State, and by funding from Developmental Disabilities. The goal of the Infant and Child Development Program is to provide each child in eastern Jefferson County between birth and three years of age with the opportunity to achieve his maximal developmental level, physical and behavioral. . The program objectives are to foster and support prevention measures; provide early identification of need through Screening and Assessment -~ . ~ ~~~ . services; provide follow-along evaluation and parent guidance services; and provide short and long term developmental and health intervention. Consultation and ancillary services will be obtained and coordinated as part of the child's program as appropriate. In order to achieve the program goal, these services will be provided in a way that will maximize, not diminish, the parent-child relation- ship and family involvement. The parents will have the primary respon- sibility for implementation of the program. Parental support will be provided as necessary to help them carry-out this responsibility. The program will be maintained in the home, appropriate to this age group, to provide an optimally nurturing environment. This program is done by a clinical nurse specialist in MCH/DD. There were 37 referrals made to the program in 1982. Thirty-three of these children required complete screening exam, 30 required follow-up care in their homes, and three were referred to other agencies for care. Four required on-going intervention services by MCH/CHN. In addition to direct service with clients the nurse orovided educational inservices and consultation to improve observational skills to people who are examining newborns and infants. Office nurses 5 6 Other nurses 2 4 Parents 1 10 Other agencies 5 42 The Clinical Nurse Specialist gives consultation services to Health Department nurses, the PNP in l'lell Child Clinic and WIC, teachers and others worki ng with chi 1 dren. Soci a 1 vlOrk servi ces, mental health pro- fessionals, physical therapists, and communication disorder specialists give services to the children under this program under the contract. e. Newborn Services Public Health nurses made 202 contacts on referral from hospital staff, physician, patient request, or record surveillance. The PHN visited 96 mothers and new infants to assess-the condition of both, and ,the need for teaching and/or guidance. There were nine home deliveries in Jefferson County in 1982. These were often high risk or premature infants, or post partum teenage mothers. The visits were made in hospital after delivery, at home, or by telephone. Forty-six mothers were sent letters offering services as personal or telephone contact was not possible. ,Many new mothers were seen in WIC and WCC. . -4- . Jefferson County Health Department provides informational packets for new parents, These packets contain information on immunizations, nutrition, infant safety, infant feeding, growth and development. how to obtain birth certificates, post partum care, and a brochure on Health Department services. , One hundred fifty of these packets were distributed by PHN through in- patient obstetrical facilities and on home visits, f. Other Child Health A variety of activities is included in this category. Audiometry (tympanograms) was done for 46 individuals on referral from physicians or clinics. We received a grant of $2,000,00 for the purchase of a new Impedance Audiometer. We have the only one in Jefferson County, For this reason we get many referrals from physicians for this screening service, A staff nurse attended a 5.1.0.5, (Sudden Infant Death Syndrome) to get current information on this condition. She also worked with staff at Jefferson General Hospital, sheriff and police deoartment, prosecuting attorney, funeral directors, and physicians telling them of the Public Health nurses role, and setting up an informational meeting in November, 1982 with a speaker from 5.1,0.5, N.W. Regional Center at COHo There were no 5.1,0.5. deaths in Jefferson County in 1982. g. Information and education, sometimes through home or office visits were provided to families of children with illnesses, communicable diseases, nuisance diseases, etc. Consultation with mental health workers, pro- tective children's services is also part of the nursing section's services to children and their parents, Pregnancy Testing Pregnancy testing is done by appointment by PHN's in the office. Counselling, and pamphlets on fetal development, changes during preg- nancy, nutrition, etc. is provided to those with positive tests. Clients wishing to terminate their pregnancy are referred to Family Planning Clinic for counselling. . In 1982 we did 54 pregnancy tests.. lhese clients were given counselling, referred to physician, WIC and other appropriate services, II COMMUNICABLE DISEASE CONTROL (Other than TB) a. Immunization Clinics . Immunization clinics are held every Tuesday at the Port Townsend office. At these clinics 2,622 doses of vaccines of various sorts were given, Some of these were for international travel and inclijded certificate validation. Because of the state regulations governing the reporting requirements for use of this vaccine most physicians elect to send their patients to Jefferson County Health Department clinics for immunization. -~ . . . lie have continued our irrmunization audit program sending reminders to parents of children who fail to return within two months after scheduled visits are due. This program continues to be effective with an increase in the number of children completing their immunizations in the recommen- ded time. The effectiveness of this is shown in the fact that we have h~d no reported cases of diphtheria, tetanus, whooping cough, polio, ~kasles or rubella in 1982. Iomunizations are offered at the Quilcene and Chimacum Well Child Clinics on a drop-in basis. This assists the parents in those areas to have their children's immunizations done without the trip to Port Townsend for our Tuesday clinics. School immunization laws were revised for the 1979-80 school year to require exclusion of elementary school children who were inade~uately immunized or whose parents failed to file a waiver because of medical exemptions or religious or ohilosophic objection. This law now includes all school children in elementary and secondary schools. The cooperation of all the public and private schools in Jefferson County has been excellent. Because the great oush to bring children into compliance is over we have s~aller numbers of school age children in our clinics for immunizations. b. Co~unicable Disease Investigating and Reporting Poor public compliance of reoorting the minor communicable diseases such as chickenpox, imoetigo, mumos, makes an accurate account of these im- oossible. We are closely monitoring and investigating reports of rubella and measles as these diseases are being erradicated by the use of vaccines. In 1982 there was one reported case of rubella in school--it was not serologically confirmed. We are planning to offer all adult school em- ployees and volunteers measles-mumps-rubella vaccine in 1983 to attempt to eliminate the :>eople susceptible to these diseases. c. !os i an Refugees Public Health nurses, in the course of their community activities not infrequently find refugees. Additionally, reports of arrivals are received fro~ Federal Immigration Services, and/or D,S.H.S. Since the name of the sponsor is usually provided, contact can often be accomplished. Refugees frequently have been resident in camps in Asia prior to travel to the U.S.A. and have commonly had some health screening of variable quality prior to deoarture. In spite of this, the incidence of tuberculosis is relatively high. Refugees are usually given general health screening by private ohysicians. Evaluation for TB including X-Rays is done by the Health Officer. Refu~ees soeak or read various lanquaaes. To aid communica- tions, some orinted ~aterial in the language"of"the Asian countries is available, but some refugees are illiterate. Often interpreters are available. Because of the language barriers, obtaining patient coopera- tion is difficult and very time consuming. We are particularly fortunate in that Dr. Fischnaller speaks several languages. -f- . .~ , _._._..U... ..".,.",.,.....~,..._ . . . The volume of refugees seen in our clinics has greatly decreased in 1982. The refugees we screened were negative for tuberculosis and as of December, 1982 we have no refugees in Jefferson County receiving Health Department services. Many refugees move to Seattle or other large cities where they can associate with more of their mm countrymen. d. / Venereal Disease Clinics The V.D. clinic is held most days bebleen 3-4 p.m. In 198'J....a total of 28 patients were seen. Patients are examined and soecimens taken as necessary. The specimens are sent to Jefferson General Hospital or to the State Laboratory in Seattle. Medications are provided as needed for the treatment of gonorrhea and syphilis. Clients with other sexually transmitted diseases are referred to physicians. III ADULT HEALTH a. Foot Clinics for Senior Citizens Clinics are held in Port Townsend, Chimacum, and Quilcene. These are very highly utilized. To prevent long waiting time for appointments we have increased the number of clinics to four times a month in Port Townsend, three times a month in Chimacum, and two a month in Quilcene. They are conducted by a Licensed Practical Nurse who has had snecial training in foot care. She is assisted by volunteers. This is one of the rKlst popular programs for seniors. Many elderly citizens are unable to do this care for themselves and have experienced much oain and discomfort until this service became available throuoh the Health Department. In 1982, 925 clients were seen at our foot' clinics. This represents an increase of 3% over 1981. The age of people seen in our foot clinics varies from 60 years to 94 years, with the average age being 74. The regularly scheduled clinics ea.ch month are: ~clinics in Port Townsend 3 clinics in Chimacu.n 2 clinics in Quilcene -7- . There are regular volunteers who assist in each clinic. We are indebted to them for their regular faithful attendance and assistance. They con- tribute 792 hours a year, In 1982 we asked for a 52.00 donation from clients--on January, 1983 tpis will be raised to 53.00. This donation will pay less than half the cost, but it is all we feel these senior citizens can afford, Donations collected in 1982 Salary and transportation in 1982 Supplies . Senior Citizens Health Screening These clinics are done by nurses. from Clalla~ County on a bi-county grant and are held in Port Townsend, Chimacum, Quilcene and Gardiner. Eligibil- ity consists of attainment of ace 60. Services are PHN conducted and con- sist of general medical history and exa~ which includes audiometry tonor~try, stool for blood, urine, pap smear for women, multiple chemical screen on blood sample, plus blood count and he~atocrit. There were 43 clinics and 231 Health Screens done. The progra~ has had funding cuts and had to eliminate some of their clinics in both counties. The program is funded through the Office of Area Agency on Aging and will continue to be done by a grant from OAAA through Clallam County. . c. Other Adult Health Public Health nurses provide service in response to a variety of expressed needs. Referrals are received from physicians, other agencies, from families and self referral. Examples of service include teaching on nu- trition, medication, and health problems, investigation of various other needs in order to Make appropriate referrals to other community resources. Nurses give talks to groups on various health needs and services as requested. A nurse is usually available in the office for counselling, blood pressure monitering, and to help with finding the appropriate source of help, or other services as needed. . Services to West End of Jefferson County Under contract with Clallam County Health De~artment, nursing services are provided by a Public Health nurse based in Forks. She visits Western Jefferson County one or two days per month. She made 68 home and office visits, and visited the school 18 times giving 291 units of service to school children. These were vision, hearing, scoliosis screen- ing and follow-uo and health counselling. IV TUBERCULOSIS CONTROL a. TB Patient Services Basic functions are diagnosis, treatment, prophylactic treatment plus follow-up of patients with quiesent disease who are at risk of develop- ing tuberculosis. Tuberculosis is classified as follows: Class I Class II Tuberculous exposure, no evidence of infection. Tuberculous infection without disease. Positive skin test with no symptoms or other clinical evidence (X-Ray, bacteriologic, etc.) of disease. Tuberculous infection with disease with specification as to various clinical findings. Class III 4It During the year 86 people had TB skin tests. Most of these were for employment. Eighty of the tests were negative and required no follow-uo. Nine peoole were Class II and were put on prophylactic medication and follow-up. Five of these were clients referred by private physicians and nursing home and we supplied medication and follow-up at no cost to the client. Prophylactic medication is usually ordered for one year. There were no Class III active clients identified in Jefferson County in 1982. b, X-Rays Twenty-two X-Rays were taken for Jefferson County Health Department and read by Dr. Fischnaller along with several films from outside sources. X-Rays for patients in this service are taken under an arrangement Jefferson County Health Department has with the Port Townsend Medical Clinic. For those oatients who are suspect or known cases, Jefferson County Health Departments pays for the X-Ray, X-Rays taken in compliance with state codes requiring chest X-Rays for certain occupa- .tions such as school personnel, nursing home personnel and read by our Health Officer are charged for. c. Skin Testing Eighty-six skin tests were given and 6% were positive and investigated and followed as needed. 4It -9- . . . d. TB Hospitalization It was not necessary to authorize hospitalization for any TB patient from Jefferson County in 1982. e. Tll Laboratory Smears and/or cultures were done for diagnosis of tuberculosis. We send these specimens to the State Laboratory in Seattle where they are done at no cost to Jefferson County Health Department. f. Nursing Service Nurses supervised home care for patients recelvlng treatment. This suoervision is essential because of the relative frequency of drug re- actions, and possible need for a change in or termination of treatment. Monthly vision checks for acuity and color vision are necessary when a patient is on one particular TB drug. Hearing tests are necessary with another. Monthly evaluations to detect any evidence of liver damage as a result of a TB drug is another evaluation the PHN's do. Home visits are also necessary as a part of the effort to identify sources of TB infection and to assess spread to contacts of known active disease. V SCHOOL NURSING PHN visit the schools and do vision screening for all children in grades K, 1, 2, 3, 6, 9. Color vision screening for grade 5 students. a. L When the Home Health Agency was transferred, the two nurses who worked in both schools and Home Health transferred to Jefferson General Hospital, one on a full time and one on a part-time basis. Because of this change we now only have two nurses to do all the general Health Department programs and school nursing. It is impossible to provide the same amount or nursing services to the schools, The Director of Nursing and the school superintendents agreed to have volunteers do the vision and hearing screening, and the nurse screen all who did not pass.and make referrals. The P.E. teachers will take care of the scoliosis screening and referral after they have been taught by the nurses. We began this program and have trained volunteers. The response was very good. It is taking longer than anticipated for the nurses to do the rescreening and referrals, so the scoliosis program will not begin until February or March. Other health services are provided by telephone consultation, or if necessary a visit. The addition of a part-time nurse for the school pro- grams is needed. VI INFLUENZA CLINICS Influenza vaccine is purchased under State contract. In previous years Jefferson County Health Department received a grant to cover the cost of the vaccine and supplies. This year there were no grant monies available, but since it is a very needed program it was continued with a charge of $2.00 each which covered the cost of the vaccine. The nurses time was provided by Jefferson County Health Department. Influenza vaccine is available to anyone in Jefferson County, but is recommended for those 60 years or over and for "high risk" individuals of any age. Those defined as high risk are those having heart conditions, chronic lung conditions, chronic renal disease, diabetes, etc, The influenza vaccine is composed of killed influenza viruses and is given by injection. The vaccine administered in 1982 offered protection against three influenza strains which recently caused disease in North America--A/Brazil, Bangkok, B/Singaoore. One dose of vaccine provides protective levels of antibodies against these three strains of influenza in 70 to 90 oercent of adults, Clinics were held at: Chimacum Community Center Port Townsend Recreation Center Quilcene Community Center Brinnon Booster Club Jefferson County Health Department Vaccine was also available at our weekly inmunization clinics, and at other times by ap~ointment. Other services, such as schools, foot care, and programs to meet community needs are done as time and money permits. Emphasis is placed on teaching clients and others in the community skills and knowledge to aid them to take more responsibility for their health, as well as to cope with health related problems. Staff members have the opportunity for inservice and continuing educa- tion provided locally and by other agencies such as WIC, MCH, Immuniza- tion Unit, etc. The members of the nursing staff appreciate the support of the Jefferson County Commissioners as our Board of Health. Regular contact with the Water Supply and Waste Section of DSHS was made in regards to alternative methods of on-site sewage disposal. F, TWenty commercial septic tank installers and two septic tank pumpers were licensed. G. Reviewed and approved City of Port Townsend and Jefferson County Building permits when related to on-site sewage disposal, Provided consultation to the Jefferson County Planning Department for sub- divisions, shoreline development and environmental assessments. H. Annual review of on-site management reports-Kala Point Condominiums and Garden Court Apartments I. In 1982 there were 57 restaurants of which 14 have cocktail permits, 5 meat markets, 12 tavernS, 4 bakeries, 39 grocery stores, and 4 school kitchens, Total number of permits issued was 121. / B. Total inspections-229, Restaurants are inspected 2 to .3 times per year, twice a year for taverns, one or more times for grocery stores~ and at least twice a year for meatmarkets and bakeries. C, Tempory food service permits issued-71 with 28 inspections. There was no fee charged for most of these permits because they were non- profit organizations and no fee is charged for them. D. Plan reviews for new establishments or remodeling of existing establish- ments-23 including consultation, E. Food Handler tests and permits-557, F, Epidemiology-4 epidemiologic surveys. G. PSP-Paralytic Shellfish Poisoning Surveillance Program. Participated with DSHS in monitoring beaches, providing public information, and submitting samples to the State Lab. III. Water Program Performed 9 proposed public well sites and 12 private well construction inspections. Information was provided on request to the public regarding development of public and private water supplies, We had hacteriological water sample bottles available to the public from Clallam and Kitsap County's water labs of which 170 were sold. Upgrading and follow-up of existing public water supplies was done in coordination with DSHS. IV. Other Environmental Health Programs A. Solid Waste-inspected and evaluated operation of drop boxes and sanitary landfill. B. School and County Swimming Pool-provided inspection of school food service facilities and checked chlorine and PH levels at swimming pool. Reviewed and approved one additional private school. C. Rabies follow-up on all reported animal bites-approximately 25. Also coordinated with State Lab the shipment of animal heads when necessary for testing-2 in 1982. D. Chemical and Physical Hazards-gathered information on air quality complaints and passed them on to the Olympic Air Pollution Authority. We also gathered information on complaints on cases of herbicide spraying of timerlands and referred them to DSHS. We provided information on formaldehyde and it's health effects, and information on who provided tests on formaldehyde levels. -2- ~ ,. V. General Environmental Health staff level for 1982 was an average of 1.5 f. toe,s. There were 6,891 office and telephone contacts handled by the secretarial staff and sanitarians. This was an average of over 500 contacts per month. The'total does not include foodhandler tests or field work. There were 101 complaints related to miscellaneous environmental situations such as sewage, garbage, animals and other nuisances~ We provided information on request in other public health areas such as birds, turtles, pesticides and "no smoking" regula tions ~ The environmental health staff represents Jefferson County in the exchange of information with other'State and County agencies that have health- related requirements within their regulations~ Theses agencies are the Washington State Department of Social and Health Services, Washington State Department of Ecology, State and Federal Departments of Agriculture, State Fisheries, and Olympic Air Pollution Control Authority, The approximate percentage of time spent in each environmental health program was: Sewage 58% Food 23% Other programs 11% Water 8% -3- , j :, .
