Joseph Fischnaller, M.D, Gael Stuart, Administrator Board Members: B.G. Brown, Chairman A.M. O'Meara, Member John L. Pitts, Member All members were present when Chairman Brown called the meeting to order. MINU1'ES: With one minor correction, the minutes for the meeting of May 18, 1983 were approved on the motion by Commissioner O'rteara, seconded by Commi.ssioner Pitts. Unanimous, FOOT CARE CLINICS: Catherine ~errill, who has operated these clinics with the assistance ot several volunteers for five years, discussed this pro- gram with the Board. Nine clinics per month are conducted at three locations (Port Townsend, Chimicum, Quilcenel. The average age of the clients is 70 years, with a large number in their 80's and 90's. The current case load is 440 clients, with at least 1-3 new clients being added each month. The purpose of the clinics is to provide necessary foot care for individ- ualSWh:o are unable to perform such care for themselves due to physical limitations. and to educate clients in the Droper care of the feet, Treat- ment includes nail and skin care, treatment of blisters, bunions, and corns, basic hygiene, and referral to a physician when indicated. ~/rs. Merrill noted that a number of clients who originally attend the clinic on cruj:;cQ.es or with a walker or wheelchair are able to regain full mobili.ty with. proper , consistent foot care. While some clients who are unableto,do their own foot care attend a clinic monthly, others check in quart~rl:Y Or yearly or when they have a problem. Proper and thorough foot care is essential for these elderly people to maintain optimal mobility. Mrs. Bee.reported that, for some clients whose contacts are limited, these clinics provide an additional social benefit. Mrs. Merrill's and the volunt~ers' professional attitude and genuine concern and interest in their clients provides clients with an opportunity to discuss other con- cerns while receiving foot care. NURSING DIRECTOR REPORT: Mrs. Bee's written report for the month of May, 1983 (attachment 1) was briefly reviewed. She noted that the decrease in the number of children seen at well-child clinics was due to the nurse who conducts the clinics being absent due to illness in the family, Health Board, June 15, 1983 Page 2 During the month of May, Mrs. Bee and Mrs. Handly attended a Crippled Children's Services workshop and a workshop on sexually transmitted diseases. PROPOSED MOUND SYSTEM: CHARLES LAWS: Mr. Durant reminded the Board that, on February l6, 1983, they had approved a mound system for Mr. Laws' Cape George lot contingent on Department of Social and Health Services approval. Following inspection of the lot on May 25, 1983, a letter was received from the DSHS Sanitarian (attachment 2) stating that the site is not acceptable for a mound system because it does not have the minimum 24 inches of original topsoil above the seasonal water level or a strata of impermeable soil. Mr. Laws was present, and stated that he would like to engage an engineer to work with DSHS staff to determine whether any other alternative system, such as the intermediate sand filter system, would be acceptable on the lot. He asked that the Board indicate their support of his plans to DSHS in order to encourage their cooperation. During the discussion that followed, the following points were made by the Board and staff members: l)Any alternative system which the State is likely to approve would probably require 24 inches of topsoil. 2)Some alternative systems, such as the sand filter, still require a drain field, and soil conditions would still be critical. 3)Although an engineer may design a system which he feels will work at a particular site, the State may not approve it. 4)Money invested in engineering services may not result in a State-approved system, and may result in a financial loss. 5)While the State is also very interested in alternative systems, and their cooperation could be expected, they are understaffed and may be slow to respond. Mr. Laws stated that information he obtained from the State of Oregon, where sand filter systems have been installed (with varying success), indicated that some systems have been approved for sites with 18 inches of topsoil. Commissioner Pitts moved to communicate, via Dr. Fischnaller, the Board's support of any joint effort by Mr. Laws' engineer and DSHS staff to find an acceptable alternate system. Commissioner Brown seconded the motion. Unanimous. DANGEROUS WASTES AND AGRICULTURAL APPLICATION OF SEWAGE SLUDGE: Mr. Stuart presented a letter (attachment 3) trom the State Association of Public Health Officials stating that these topics will be discussed during meetings held concurrently with the Washington State Association of Counties meetings which the Board will attend in Yakima. Health Depart- ment staff will not be attending, but it was suggested that one or more of the Commissioners might choose to attend these meetings. LETTER OF APPRECIATION: Mr. Stuart read a letter of appreciation for assistance provided by Randy Durant and John Hayes (attachment 4). Health Board, June 15, 1983 Page 3 ADDITIONAL ENVIRONMENTAL HEALTH PERSONNEL: Mr. Stuart reported that Clallam County has inquired about the possibility of sharing the services of another Environmental Health Specialist on the same basis as John Hayes is employed. He indicated that, if the staff is to be increased, hiring a part-time person (possibly shared with Clallam County) is prefer- able to hiring a fulltime person because it would provide more flexibility and make covering vacations, etc. easier. He also acknowledged that the environmental health workload probably justifies additional staff. How- ever, from the standpoint of his position as Administrator, he is compelled to resist increasing the staff unless revenues increase. At present, revenues do not justify additional staff. He stated his opinion that to increase staff at County expense would mean the County would be funding additional services to the public. Mr. Durant stated that an additional staff person would not provide any extra services, but would enable the Department to perform duties and services which they are required to provide. At present, food service inspections are four months behind. Owner septic tank installation inspections are not always performed in a timely manner, and only about 20 percent of professionally installed systems are inspected. He pointed out that, in addition to routine duties such as site evaluations, sewer and water system review and inspection, plan reviews, permits, coordination with the Planning and Building Departments on projects, and related con- tacts and paperwork, the number of special projects and requests is increasing. During the month of May, there were eleven different matters which required a considerable amount of time and effort, such as plan reviews for a school remodeling, a new school facility, and the Crown Z solid waste site, requests for information regarding the status of Protection Island, two public septic system repairs, and the situation at Olympic Testing Labs. He noted that there is no fee charged for many of these less routine activities. The Board indicated that the County's financial situation is such that an additional employee cannot be employed unless there are additional revenues. There was discussion regarding the possibility of charging for services for which there is not currently a fee and of increasing current fees to reflect the actual cost of the service. ADMINISTRATOR'S REPORT: Mr. Stuart reported that actual expenditures are lower than budgeted and actual revenues, due to a Home Health Program payment which was received, exceed the budgeted amounts. He agreed to investigate the possibility of Federal funding to compensate for expenses incurred due to the current situation on Protection Island and the possi- bility of any funding which might be available due to the new jobs programs. PROFESSIONAL SERVICES CONTRACTS: Commissioner Pitts seconded Commissioner O'Meara's motion to approve and sign contracts with Danielle McClanahan and Beverly Rich (attachments 5 & 6) to provide staff coverage during vacations, etc. Unanimous. Fischnaller, M.D. County Health Officer Jefferson County Health Department 802 Sheridan Port Townsend, Washington 98368 Subject: Charles Laws Proposed Mound System, Lot 12, Division 4, Cape George Village Dear Dr. Fischnaller: As per the February 16, 1983, Jefferson County Board of Health minutes and Randy Durant's May 6, 1983, letter, Ethan Moseng and I visited lot twelve of division four of the Cape George Village subdivision on May 25. At that time, we evaluated the site and soil for a proposed mound sewage disposal system. The evaluation was made within the framework of the March 1980 Rules and Regulations of the State Board of Health, On-Site Sewage Disposal Systems, and the May 1980 Guidelines Governing the Design, Application, and Operation of Fill or Mound Systems. Based upon the soils information provided this office and my site evaluation, the location of the proposed mound does not meet with the requirement that a minimum twenty-four inches of original, undisturbed and unsaturated soil exist above the maximum seasonal water level or a strata of impermeable soil. The contract is to be effective , June 1, 1983 and will continue until December 31, 1983. WHEREAS, the Department is in need of the part-time services of a Public Health Nurse; and WHEREAS, at the present time the Department is unable to provide the services from personnel within its employment; and WHEREAS, the Contrator states that she is qualified to perform the duties of a Public Health Nurse as set forth in the body of this contract; NOW THEREFORE, IT IS AGREED: (l)The Contractor shall act as a Public Health Nurse assisting the Pediatric Nurse Practitioner in conducting Well Child Clinics. She will serve under the direction of the Director of Nurses in conducting home visits, school programs and general nursing duties. (2) The services to be performed by the Contractor will be in accordance with the standards of the profession, the State of Washington and the Jefferson County Health Department. (3) The Department shall pay the Contractor the amount of Eleven dollars and sixteen cents ($11.16) per hour for professional services rendered and shall reimburse her for mileage driven in her personal vehicle at the rate established by Jefferson County. (4) The Contractor shall provide the Department with a detailed accounting of the work performed and the mileage driven at the conclusion of each month of this contract. The Department shall make payment for the work performed on or before the 20th day of the month following the month in which the work was performed. ( ( . (5) The Contractor is employed by the Department to substitute for Department personnel during leaves and vacations. It is understood that she acts as an independent contractor and that she will receive no employee bene- fits from the Department of Jefferson County. It is further understood that neither the Department nor Jefferson County will withhold F.I.C.A. taxes or other obligations for which the Contractor may be responsible. (6) The Contractor shall be considered an agent of the Department and will be covered by the terms of the Department's insurance policy. (7) The Contractor shall not discriminate against any person presenting themselves for service based upon race, religion, color, sex, age or national origin. The Department shall take affirmative action to insure that persons receive services without regard to race, color, sex, age or national origin.
