One set of the Road Index Maps is for indicating thereon the revisions. as to road realignment, surface type changes, roads. added to the county road system, and roads transferred out of the system during the. year 1960. This set of maps is to be submitted with your LM-O report as they are a part of your reporting data. The other sets of maps are for your files. For each revision entered on the LM-O, the location of the revision is also to be shown on the map, giving the exact limits of the road change and assigning an identification number to the change. The identification numbers for road changes shown on the maps should correspond with the item numbers listed in the LM-O report. A print of the new plats. adding roads to the county road system should also be submitted with your report in order that the correct location of the roads in the plat can be correctly shown on the maps. Annexations to the cities during the year 1960 should be shown on the map and the county roads or portions of a county road, transferred to the cities due to the annexation should be recorded on the LM-O report.
