Dear Property Owner,
A monitoring inspection of the septic system serving the above referenced property was completed
November 26,2013. The septic permit on record had never been 'finaled'because only a portion of the
system was installed at the time of inspection by Health Department staff and an'as-built' drawing was
required. There is no evidence in the file that the as-built was submitted,therefore the permit was never
finaled. This office was contacted in April 2015 in regards to completing the permit. A site plan showing
the septic system components and structures was required and septic system components were required to
be uncovered to verify their location. A site inspection was completed by this office on May 6,2015 and a
site plan was submitted by Justin Wells,O&M Specialist,on May 27th,2015.
Based on the inspection and report submitted the permit cannot be finaled because the drainfield was not
completed per the approved permit. A total of 150 feet of 2 foot wide trench was required. Less than
118'was installed. The system is in violation•of the permit. No further approvals for activity or building
on the site shall be granted until the violation is corrected.
This letter is intended to serve as formal notice that no further approvals shall be granted until corrections
are made and approved by Health Department staff.
A permit is required for any repair or modification of an onsite sewage system,per Washington
Administrative Code 246-272A and Jefferson County Code 8.15. A list of designers that have submitted
work here is enclosed.The code sections referenced are attached for your information.
