The property owner is responsible for the accurate location of all property lines.Any removal of or major disturbance of soil in the primary or
reserve drainfield area may create site conditions that are unacceptable for the installation of a sewage disposal system.Any change in
drainfield or tank location may invalidate this permit unless prior approval is obtained from the Jefferson County Environmental Health
Division. If during excavation or development of the site an area of potential archeological significance is uncovered,all activity in the
immediate area shall be halted,and the UDC Administrator shall be notified at once.
HEALTH DEPARTMENT MUST BE CALLED FOR FINAL INSPECTION.

CONDITIONS OF APPROVAL- PERMIT NO.: SEP08-001 82
1.) This permit is to add a pretreatment unit to help a septic system that is showing early
signs of failure. Monitoring of the system through the winter season is required.
Monitoring to be conducted by a licensed professional or county staff.
1 2.) H -Monitoring ports are required to be installed at each end of each lateral.
3.) H - AS PER WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 ALL SYSTEMS
REQUIRE MONITORING. THIS SEWAGE DISPOSAL SYSTEM WILL REQUIRE AN
ACTIVE MONITORING AGREEMENT RECORDED TO THE PROPERTY TITLE PRIOR
TO FINAL APPROVAL OF THE SEWAGE DISPOSAL SYSTEM.
'4.) H -The signed Notice for Maintenance and Monitoring of an Onsite Sewage System shall
be recorded to the property title prior to final approval. Non-compliance with the contract
or maintenance requirements is a violation of this permit.
5.) As per Jefferson County Code 8.15 all onsite sewage systems require monitoring.
Monitoring shall be provided by an approved entity. THIS SYSTEM IS REQUIRED TO
BE INSPECTED PER JEFFERSON COUNTY CODE 8.15 AS ADOPTED OR
AMENDED.
6.) Health Dept. required to observe pressure test with system designer when system fully
installed/complete, 48 hours notice to be provided for scheduling.
7.) Designer must be contacted prior to start of construction and for inspections during
installation. DESIGNER IS REQUIRED TO DO A PRECOVER INSPECTION ON ALL
TYPES OF SYSTEMS.
8.) Notification of the start of construction shall be faxed or emailed to Jefferson County
Public Health ONE WORKING DAY prior to start.
9.) H -An asbuilt drawing and certification of completion by the designer is required prior to
final approval.
10.) Before final approval is given, the designer shall provide an operations and maintenance
manual to the property owner and the Health Department. The manual must instruct the
owner of the on site sewage system on the ways to properly operate and maintain all
components of the system.
11.) Divert all sources of drainage, including roof drains away from septic tank and drainfield
area.
12.) Approval/issuance of a sewage disposal permit does not guarantee the approval of other
development or a building permit on this site. Compliance with other Jefferson County
and Washington State Codes is required.
13.) Risers are required to grade with secured lids over both compartments and over the
outlet of the septic tank for inspection and maintenance/monitoring. Minimum 20" riser
over inlet.
14.) Approval of this sewage disposal permit does not preclude the permit holder from
complying with the Unified Development Code for other/future development on the site.
15.) A maintenance and management contract is required with the manufacturers authorized
representative. Maintenance of the system shall be completed by a manufacturers
authorized representative at the frequency required by the manufacturer.

16.) The project shall adhere to the Best Management Practices (BMPs)to control
stormwater, erosion and sediment during construction. BMPs shall address permanent
measures to stabilize soil exposed during construction, and in the design and operation of
stormwater and drainage control systems.

Project: Septic System Monitoring Results, Parcel Number 939601433, 160 Colman
Loop, Port Townsend
Hi Randy;
The purpose of this report is to provide a history of the project along with conclusions
regarding the results of drainfield and groundwater monitoring after installation of an
aerobic treatment unit (ATU).
History of Project
According to the owner, severe ponding was evident in the drainfield based on
observations of soggy areas in the yard during Winter months. On 8/29/08, two soil holes
were observed in the area of the drainfield by this office and Jefferson County Health
Department (see attached soil logs). With the goal of alleviating the severe ponding, a
septic system permit for an ATU was submitted and approved in 2008 and installed on the
existing drainfield under SEP08-00182.
In 2012, the d-box was located and replaced along with outlet levelers all in a 24 inch
diameter PVC riser installed with a gasketed bolt down lid. Observation (monitoring)ports
OP1, OP3, and OP4 were also installed in each drainfield lateral (see attached Wet
Season Evaluation site plan)with ground monitoring port OP2 installed between the upper
two laterals.
This office monitored these ports from 7/17/12 through 12/5/13 with your office performing
a"Wet Season Evaluation"to monitor levels in February and March of 2014. It should be
noted that this area experienced above normal rainfall from mid-February, 2014, through
the first week or two in March which likely had a significant impact of the water levels in
OP1 & OP2. See attached table reflecting the summation of monitoring results.
Conclusions
Based on the monitoring results, it appears that the drainfield laterals remain mostly dry
throughout the year and that any water levels appearing in the wet months appears to have

lowered considerably and stabilized (the yard no longer gets soggy in the Winter months).
Also, based on the available information it appears that the drainfield laterals are not
disposing of effluent directly into a water table. However, this drainfield likely does not
meet current code in relation to the required three feet of permeable unsaturated soil below
a conventional gravity drainfield. It must be noted that any future permit applications to
Jefferson County may require this system to be brought up to current code along with
establishing a reserve drainfield area.
STANDARD DISCLOSURE STATEMENT
This report has been prepared exclusively for David Fess for his use in evaluating the
impact of the permitted ATU on the severe ponding in the existing drainfield. Any
observations, conclusions, and recommendations in this report apply only to the property
that was examined and they are not applicable to other areas. This report is based upon
soils and system evaluation and monitoring described in this report and upon the site
conditions as they existed at the time of our site inspections. Further soils evaluation,
monitoring, and/or system modifications or replacements may be required as part of any
future permit applications to Jefferson County along with any other requirements stipulated
by Jefferson County and Washington State DOH.
Within the limitations of scope, schedule and budget, our services have been executed in
accordance with generally accepted professional on-site wastewater treatment system
design principles and practice. This warranty is in lieu of all others, either expressed or
implied.
