Dear Mr.Archbald:
This letter is to inform you that Jefferson County Public Health (JCPH) intends to file a Notice to
Title on your property for failure to comply with the Onsite Sewage Regulations. Since 2006,JCPH
has conducted site visits and sent letters requesting action be taken to correct a potentially failing
septic system on the property.
The following is a brief summary of the actions that JCPH has taken to achieve compliance:
09/14/2004 JCPH received an evaluation identifying 2 feet of scum/sludge in the tank
02/28/2006 JCPH issued a System in Violation letter requiring permit for repair/upgrade
01/29/2013 JCPH sent a letter requesting proof of repairs
05/10/2013 JCPH sent a second letter requesting proof of repairs
10/26/2015 JCPH sent final notice requesting proof of repairs
In 2004,JCPH received a copy of an EES conducted identifying your septic tank that had two feet of
scum/sludge in the bottom half of the tank. The O&M professional concluded that the tank was
leaking. Since February 28, 2006 the Jefferson County Health Department has repeatedly requested
you obtain a permit and bring the system up to code. If the violations are not corrected by
February 28, 2016,we will proceed with the recording the Notice to Title in the Jefferson County
land records.The Notice to Title will describe the existing violations of the County's Onsite Sewage
Code present at your parcel and will be a "cloud on the title."
In order to remove the Notice to Title,you must document that you have corrected the violations
and pay the Notice to Title fee, currently set at$525.00.

As a result of the violations listed below,the real property described above is out of
compliance with Jefferson County Code JCC 8.15.060 Adequate Sewage Disposal Required and
has created a condition that is a potential public and/or environmental health threat and/or
nuisance.
The following item(s) on the above-listed real property have/has been observed to be in
existence there by a Jefferson County Certified Operations and Maintenance Specialist and thus
it is more likely than not that they exist at the above-listed real property in violation of Chapter
246A-0010 WAC and Jefferson County Code Section Jefferson County Code JCC 8.15.060

The landowner has not corrected the violation despite receiving a "Notice and Order to Correct
Violation" in February 2006,January of 2013, May of 2013 and October of 2015.
Any interested party may apply to Jefferson County Public Health to rescind this Notice to Title
for Non-Compliance by demonstrating resolution of the violation. Corrections must be
completed in compliance with current code, satisfactory to Jefferson County Public Health for
the identified violation and payment of the required fee
