COUNTY ENPLOYEES OF JEFFERSON COUNTY
IN COOPERATION WITH OTHER POLITICAL SUBDIVISIONS
OF THE STA';l'E OF WASH~NGTON THROUGH THE F.ACILITIES
OF THE WASHINGTON STATE ASSOCIATION OF COUNTY
COMMISSIONERS, AS TRUSTEE
BEFORE THE BOARD OF COMMISSIONERS, JEFFERSON COUNTY, WAiHINGTON.
The Board of COMMISSIONERS, convened in regular sesBion this 24th. day of May, 1971,

and having duly considered a Resolution establishing a plan of group life, accident, hospotal, and surgical
insurance for eligigible employees of JEFFERSON COUNTY, in cooperation with several other political

subdivisions of the State of Washington, do hereby adopt the following Resolution:
WHEREAS, It appears that at least seventy-five percent (75%) of the eligible employees of JEFFER-
SON COUNTY have expr.essed their desire to join a plan of group life, a.ccident, hospital and surgical inauranc~;

WHEREAS, The plan of group insurance offered to political subdivisions oj the, State of Washillgton, :i

through the Wahington State Association of County Commissioners, as Trustee, has been reviewed and appears!

to offer a sound program of employee group coverage; and
WHEREAS, The establishment of a program of group insurance for these employees is a proper
employee bene,fit under the provis~ons of'Ch?-pt4;lr 41; 04. Laws of State of Washington, and
WHEREAS, In adopting a program of grolip insurance for these employees, it is necessary to incur
::ertain expenses,

NOW, THEREFORE, BE IT RESOLVED by BOARD OF COMMISSIONERS of JEFFERSON County,
Washinton, pursuant to the provisions of Chapter 41. 04, Laws of State of Wa'shington that JEFFERSON
COUNTY adopts as the official plan of group life, accident, surgical and hospotal insurance for the eligible
employees of JEFFERSON COUNTY the program of coverage provided for under the terms of the carrier's
agreement with the Washington State Association of County Commissioners, as Trustees,' said association of ,I

full contract of coverage and the trust agreement are on fole with the clerk of this Comrnissioners, Olympia,
Washington; and the state insurance commissioner, Olympia, Wahington.
BE IT FURTHER RESOLVED: that in adopting this program of group insurance, the Board agrees:
(1) To pay all or a part of the total premium for each covered employee for grou.p life, accident,

hospital and surgical insurance premium for a single employee.
(2) T'o make the nece,ssary payroll deduction to cover any portion of the premil\m not paid by the
political subdivision and to maintain membership rolls and related records as may be required by the plan.

(3) To transmit the total premium payment required under the conttl.1.ct in accordance with the numbe,
