THIS.AGREEMENT/Entered into by and between the DEPARTMENT OF
SOCIAL AND HEALTH SERViCES, hOi'oinafter refer:red to as the "Department"
and JEFFERSON COUNTY ,hereinafter referred to as the "Contraotor".
IT IS THE PURPOSE OF THIS CONTRACT TO provide alcoholism
i~fo~tion and referral services in the geographio area of Jefferson County.
IT IS, THEREFORE, MUTUAllY AGREED THAT:
