HomeMy WebLinkAbout1076 CONTRACT NO. l060-PRC-858 STATEMENT OF WORK The Contractor shall accompli8h ~he following tasks in the operation of the ALCOHOLISM INFORMATION AND REFERRAL program in the prescribed gmographio araa: 1) Provide an alcoholism information and referral service at a suitable location. 2) Prov1~e informa~ional services to ~he general public and eduoa~ion services furnisheU by qualified personnel ~o schools, courts, health and ~lfare agenoies, and other appropriate public or priv~te agenoies or groups. 3' W~rk cccvsr~tiqely with othe~ pUblic and priva~e mgenoieg to mainuin, proIl\()te, and coordinate community resources and facilities for the referral, care, guidance, tr6atment and rehabilitation of. ~looholics and problem drinkers, and develop . and WOlintaln Q aomprohonoive plan for the coordination of' all suoh services. 4) Supply a minimum of ten (10) hours of administrative and gervice time per week or a minimum of for~y (40) hours per month for the period of the contract to and in behalf of r9sidents of the geographio area involved who are muCfa~inq from alooholism-related problems. S) Maint~in a reoord-keeping system for acoumulation of such tilooholism-related information as the Department may reasonably require. The Department will assist the Contractor by providing neoessary report forms~o be completed and re~urn6d.to tho Department. 6) Insure thai: all directors and staff members of the Contractor, ~s may be designated by the Dopartment, shall attend any training, ed~oational or information program as may be given by th(d Department from time to time, upon notifioation of suoh program by tho Department. 7) S~b"it monthly to the Department, a report of the aotivities of i~e information and referral servicoD, and an annual finan- cial a~count.inq of its expenditures And shall, in addition, 8Ubmi~ cnnually to the Department, no later than 6 months prior to the normal t~rmination of ~his oontraot, aplen for the propoBed alcoholism program and the proposod expenditures for three (3) Bucoaeding contract yea~8. 6) Pro~i~e servioe. under this contraot for the benefit of neody alcoholios, howover it is the int8n~ of the parties hereto t~t all atat.e, fede~al, community or privAta funds avail- able bo utilise4 to a.sis~ finanoially in tho oaeo of suoh r.OMY per.ons.
