SPECIAL PROVISIONS:
AN APPROACH CULVERT WILL NOT BE NECESSARY AT THIS LOCATION AT THIS
TIME HOWEVER, IF DRAINAGE BECOMES A PROBLEM HERE IN THE FUTURE THE
COUNTY RESERVES THE RIGHT TO REQUIRE ONE. THE GRADE OF THE APPROACH
SHALL BE SUCH THAT ALL SURFACE WATER SHALL BE DIRECTED INTO A
DITCHLINE AND NOT ONTO THE COUNTY ROAD. THE APPLICANT SHALL
CONTACT THE PUBLIC WORKS DEPARTMENT FOR A FINAL GRADE INSPECTION
PRIOR TO COMPLETING THE DRIVEWAY. THE DRIVEWAY SHALL BE SURFACED
WITH CRUSHED ROCK A SUFFICIENT DISTANCE BACK FROM THE COUNTY ROAD
TO INSURE THAT NO MUD SHALL BE TRACKED ONTO THE COUNTY ROAD. THE
OWNER HIS SUCCESSORS AND ASSIGNS SHALL BE RESPONSIBLE FOR
MAINTAINING THIS APPROACH FOR THE LIFE OF ITS USAGE, INCLUDING, BUT
NOT LIMITED TO, DRAINAGE AND SURFACING. THE APPLICANT BY SIGNING
THIS PERMIT IS AGREEING TO ALL OF iTS CONDiTiONS INCLUDING THE
PROVISIONS ON THE BACK OF THIS FORM.
A bond in the amount of $ is required for the protection of Jefferson County as set forth in the terms of
the bond. No work shall be done under this permit until theparty or parties to whom it is granted shall have communicated with
and received instructions from JEFFERSON COUNTY PUBLIC WORKS This permit shall be void unless the work
herein contemplated shall have been completed before 0 4 / 18 / 9 8
In accepting this permit the grantee, his successors and assigns, agree that any damage or injury done to the property of the
grantee or any expense incurred by him through the operation of a contractor, working for the County or any County employee
shall be at the sole expense of the grantee, his successors and assigns.
