The final large lot subdivision record of survey map shall be prepared in accordance with the provisions of Subsection 7.405 for Large Lot Subdivisions. The map shall be labeled as a survey for the large lot subdivision. A vicinity map shall be shown on the face of the large lot final survey map. The Record of Survey Map shall be prepared on an 18" by 24" acceptable mylar. A right-of-way sixty (60) feet in width for Mclnnes Road contiguous to the subject property shall be conveyed, by statutory warranty deed, to Jefferson County. The Auditor's File Number (AFN) shall be shown on the face of the Final Record of Survey Map for the large lot subdivision. A certification checklist shall be placed on the face of the large lot record of survey map. The checklist shall provide for all approvals as outlined in the large lot subdivision provisions. Road Approach Permits must be obtained from the Jefferson County Department of Public Works prior to construction of any approach onto the County road. Prior to final large lot plan approval the proponent shall submit a drainage plan and report to the Jefferson County Department of Public Wor~ks for review' and approval. The drainage plan must incorporate the storm drainage critena of the Was,hington State Department of Ecology, Storm Water Management Manual (SWMM) for the Puget Sound Basin and Washington State Department of Fisheries, Draft Storm Water Guidelines, November 1, 1990. A temporary and permanent erosion control plan shall be reviewed and approved by the Jefferson County Department of Public Works. The plan shall be approved prior to final large lot subdivision plan approval. The temporary and permanent erosion control plan shall incorporate the applicable sections of the Storm Water Management Manual. The storm water management system shall be designed to ensure that runoff entering Donovan Creek, the adjacent wetland and Quilcene Bay from the project will not reduce the present water quality of either water body. The septic system primary and reserve drainfield areas for the lots in the large lot subdivision shall be located west of the County road, Mclnnes Road. The applicant shall assemble any existing pertinent data and information related to groundwater and the aquifer for the site and the surrounding area. This data and information, accompanied by the recommendations of a groundwater specialist, shall be submitted to the Jefferson County Board of Commissioners prior to final approval by the County. No development of any type that would have an adverse affect on water quality shall take place within the wetland of Donovan Creek. Within the conservation easement area, only water wells shall be developed. To protect the important habitat and unique ecological value of Donovan Creek and its associated wetland, a dedicated conservation easement may be established with either the Jefferson County Land Trust or another conservation/trust group which will preserve the integrity of this important habitat and unique ecological feature of this site. The conservation/trust group must be acceptable to the Board of Commissioners. The Auditor's File Number for the dedicated conservation easement shall be referenced on the face of the large lot subdivision mylar. The common open space/dedicated conservation easement may not be further divided unless approved by Jefferson County. The Northwest Institute for Historic Preservation shall not be involved in the establishment or management of the conservation easement. It is understood that the proponent has offered this easement as a method of avoiding probable significant adverse impacts to the property. If such an easement is not created and recorded prior to final approval, then further mitigation may be necessary. Prior to final large lot subdivision approval, the applicant shall demonstrate in writing that the area of the proposed lots complies with the provisions of Subsection 7.306.1 of the Large Lot Subdivision requirements far a density subdivision. Notice shall be given to potential purchasers that individual water wells for proposed building sites shall be evaluated for salt water intrusion using the same standards as for a community well. The applicant shall be required to pay all costs of work incidental to approval of the subdivision before final approval is granted. The applicant shall comply with all conditions of the Mitigated Determination of Non- Significance issued by the Jefferson County Board of Commissioners. The following notices shall be placed on the face of the final large lot subdivision map: NOTICE TO POTENTIAL PURCHASERS: Jefferson County requires the dedication of common open space areas for cluster large lot subdivisions. The common open space for this large lot subdivision is important habitat, has unique ecological value and is preserved by means of a dedicated conservation easement. Any development of the open space, including livestock grazing, is prohibited. bo Applications for On-site Sewage System Permits shall be accompanied by soil profile description, site characterizatior~ and septic system plan prepared by a certified desi~l~e?;"f'egistered sanitarian, or licensed professional engineer. Any removal of or major disturbance of soil within the proposed drainfield areas may create site conditions that are unacceptable for the installation of sewage disposal systems. Approval of this subdivision does not constitute approval of building or sewage disposal permits on each lot. Permit approvals will be subject to building plot plans, type of use, contours, and soils on individual lots, Permits will be reviewed in accordance with site conditions and regulations existing on the date the permit is applied for. Individual water wells for these building sites shall be evaluated for salt water intrusion using the same standards as for a community well. SUBMISSION: It is the responsibility of the applicant or authorized representative to ensure the final large lot survey map and supporting data is submitted in accordance with the conditions of summary approval.
