Connecting people to resources & services
Olympic Connect delivers on our name: connecting people in our region to qualified, available care and services through trusted, trained, and highly skilled community-based workers.
These community-based workers are individuals from our region, with lived experience and cultural knowledge. They work with a variety of organizations, Tribes, industries, and sectors
dedicated to providing clients with one-on-one support so they can achieve their health and other life goals.
Olympic Connect coordinates health care and social service providers. We connect regional resources and track real-time data for healthier individuals, families, and communities. Olympic
Connect also strengthens the community-based workforce and the network of partners that employ them by providing training and technical assistance, addressing service gaps, and collecting
centralized data.
Resources hub, Any person (no criteria- any age, any income, etc) can refer themselves on-line and then they are assigned to the best-fit provider (case managers/ resource navigator
or coordinator)
Case managers reach out to people who have signed up-
case managers provide a needs assessment and then then case manager reviews that assessment with person and then they work with them to access resources to meet their needs & goals.
It’s all client paced and optional enrollment
No cost
Target : Residents of Clallam, Kitsap and Jefferson
The case manages will have a search engine of resources
The intention is to bridge the gap between the client and the service they need. Hope to increase desired follow through
It’s more then just referrals- its assistance getting successfully connected to those referrals.
