The application was reviewed by the Jefferson County Department of Community Development staff on 11/25/02 for the potential presence of Environmentally Sensitive Areas (ESAs) under the provisions of the Unified Development Code (UDC). After an initial Geographic Information Systems mapping review and an investigative site inspection, the following ESAs were confirmed to be present on the subject property: Suburban Shoreline; Type 1 Marine Waters; landslide hazard area; Flood Area -Zone A; Seawater Intrusion Protection Zone (Coastal SIPZ). 2.) Critical Aquifer Recharge Areas are defined as Seawater Intrusion Areas, and Special Aquifer Recharge Protection Areas and the following land uses located within Susceptible Aquifer Recharge Areas (1)All Industrial Land Uses (2)All Commercial Uses (3)All Rural Residential Land Uses requiring a Discretionary Use or Conditional Use Permit(4)All Rural Residential Land Uses with nonconforming uses that would otherwise require a Discretionary Use or Conditional Use Permit(5) Unsewered Planned Rural Residential Developments (6) Unsewered residential development with gross densities greater than one unit per acre. All of the above listed uses shall be subject to the applicable requirements and standards of the Jefferson County Unified Development Code. 3.) To help prevent seawater from intruding landward into underground aquifers, all new development activity on Marrowstone Island, and within 500 feet of any marine shoreline shall be required to infiltrate all stormwater runoff, to the maximum extent practicable, onsite. 4.) The applicant is proposing to create or add zero square feet of impervious surface and 465 square feet of land disturbing activities. Sections 6.6 and 6.7 of the Jefferson County Unified Development Code requires that your proposal submit a Stormwater Calculation Sheet. The applicant submitted a calculation sheet. 5.) The project shall adhere to the Best Management Practices (BMPs) to control stormwater, erosion and sediment during construction. BMPs shall address permanent measures to stabilize soil exposed during construction, and in the design and operation of stormwater and drainage control systems. 6.) A geotechnical report was not required or submitted for the remodel permit. 7.) Jefferson County determined that this proposal is categorically exempt from review under the State Environmental Policy Act(SEPA) pursuant to WAC 197-11-800(1)(b)(i). 8.) Any future permits on this site are subject to review for consistency with applicable codes and ordinances and does not preclude review and conditions which may be placed on future permits. 10.) The site plan as submitted with the building permit application on November 12, 2002 has been reviewed for consistency under the UDC. It has been approved by Jefferson County Department of Community Development. Any modifications, changes, and/or additions to the stamped, approved site plan dated November 12, 2002/December 6, 2002 shall be resubmitted for review and approval by Jefferson County Department of Community Development. 11.) A building permit application (BLD01-00617) for a carport demolition was applied for and withdrawn September 5, 2002. 12.) An application for Evaluation of the Existing System (SEP02-00209)was issued 9/19/2002. It has yet to be finaled. 13.) The applicant provided a Site Evaluation dated November 20, 2001, which was prepared by Krazan &Associates, Inc. Based on visual observations and Krazan &Associates experience it appeared that groundwater seepage encounters impermeable soils at rather shallow depths and flows along the slope toward the shoreline. The geology of the subject property and adjacent parcels would make on-site and upland off-site infiltration impractical. The firm of Krazan and Associates specializes in geotechnical engineering, environmental engineering & construction testing & inspection. is\F_BLD_Permit_Propane.rpt 10/29/19 03 UILDING PERMIT APPLICIION MRAOW00p 86 Jefferson County Department of Community Development 621 Sheridan Street Port Townsend, WA 98368 PERMIT #: BLD02-00673 Received Date: 11/12/2002 SITE ADDRESS: 250 MAPLE LN PORT LUDLOW, 98365 OWNER: F NEIL L LAMKA PHONE: (206)933-1389 KATHRYN LOUISE LAMKA 2349 HARBOR AVE SW #705 SEATTLE WA 98126-2126 SUBDIVISION: OLYMPUS BEACH Block: Lot: 55&5E PARCEL NUMBER: 978900038 Section: 4 Township: 28 N Range: 01 E CONTRACTOR: LANDER CUSTOM DESIGNED HOMES PHONE: (360)437-2501 PO BOX 65437 PORT LUDLOW WA 98365 Contractor's License LANDECDO22JT Expires 3/12/2004 REPRESENTATIVE: RICK LANDER PHONE: (360)437-2501 PO BOX 65437 PORT LUDLOW WA 98365 PROJECT DESCRIPTIOP DEMOLITION TYPE OF WORK RES SQUARE FOOTAGE: TYPE OF IMP DEM VALUATION MAIN: CODE EDITION: 1997 ADD'L: HEAT TYPE: OCCUPANCY: HEAT BASE: HEAT TYPE: OCCUPANCY: UNHEATED: #OF STORIES: OTHER: CONST TYPE: GARAGE: SHORELINE: CONST TYPE: DECK: SETBACK: BANK HEIGHT: SEWAGE DISPOSAL: WATER SYSTEM: PARCEL TAGS: YES NO BEDROOMS: BATHROOMS: STORMWATER: YES NO AREA Exist: Exist: Wetland Erosion Prop: Prop: Seismic Streams Total: Total: Flood Way Floodplain Routing Dater )/ F&W Landslide / P Shoreline Aquifer Forest: Commercial Rural Type Amount Paid Bv: Date: Receipt: APPROVED vOVE D Permit $47.00 MAM 11/12/02 51326 1 r State Building Code $4.50 MAM 11/12/02 51326 DEC 1 3 2002 Total: $51.50 Jefferson County Planning & Building Departmen A,7,, 0 . - 5g, JEFFERSON COUNTY COMMUNITY DEVELOPMENT,621 SHERIDAN ST,PORT TOWNSEND WA 98368 PH: (360)3/9-4450 7 ( 'T r 1 it DEMOLITION PERMIT APPLICATION NOV 12 2002 1_ SITE ADDRESS: 911#IROAD NAME ' ''S-D OACL,9 a_ Lam,.
