HomeMy WebLinkAbout060925 📢 Press Release_ SPORTS PHYSICAL APPOINTMENTS NOW AVAILABLE FOR THE 2025–2026 SCHOOL YEAR!📢Jefferson County School-Based Health Centers (SBHCs) are now scheduling sports physicals to help student-athletes prepare for summer and fall sports. All Jefferson County school districts
require a pre-participation sports physical every two years to ensure students are healthy and ready to play.
Comprehensive physical screenings are available at our SBHCs located at Chimacum, Quilcene, and Port Townsend High School. Appointments are open to all interested student-athletes in
Jefferson County and can also be scheduled at the Quilcene Public Health Clinic.
To be seen, students must complete the Pre-Participation Sports Physical Evaluation Form and the General Consent for Services Form before their appointment. Both forms are available
